Position Summary</b></p><p>
The Homeless Outreach program is a dynamic and unique team. Our staff work autonomously out in the community to assist clients with basic needs, case management and referrals to higher levels of care or community partners. Objectives will be to create supportive relationships with homeless individuals through street outreach and referrals to community partners to prioritize meeting clients where they are at.</p><p>
This position provides outreach services to Salt Lake City&rsquo;s unsheltered individuals and/or families in order to connect clients with supportive services with the goal to decrease Salt Lake City&rsquo;s unsheltered population and connect individuals with safe and stable housing. Our city care coordination team is made up of three individuals. This team will be provided a work vehicle and will be expected to provide services to unsheltered clients across the city limit. Daily tasks may include providing clients with essential supplies, driving clients to community partner appointments, and assisting clients with obtaining vital documents that will later be essential in the housing process. Care coordinators will have the ability to build rapport with clients to assess housing motivation and add clients to HOP&rsquo;s internal case management referral list. This team is expected to respond and assess community referrals. In this position you will also respond to community concerns, assist with community education, and problem solve around homelessness issues.</p><p><b>
Essential Duties?</b></p><ul><li>
Proactively engages and initiates contact with clients in accessing basic needs items and community-based resources, to help them overcome homelessness.</li><li>
Prioritizes client engagement in Salt Lake City, identifying priority areas, coordinate needs, and address barriers.
</li><li>Identify priority needs, (i.e. pregnant, IV/IM drug user, dual diagnosis, and other medical conditions) and refer/coordinate services as needed.</li><li>
Provide direct, Trauma Informed Care to clients experiencing unsheltered homelessness, including crisis intervention, outreach, ongoing assessments (i.e., SPDAT, Lethality Assessment , Crises Response Plans, etc.), creative problem solving, agency and community resource utilization, and network building.</li><li>
Assess medical emergencies, and provides immediate and professional response to any emergency following appropriate policies and procedures. (contacting emergency medical, utilize Naloxone kits, Etc.)</li><li>
Participate in regularly scheduled staff meetings to ensure appropriate case planning and referral of clients to the most appropriate service provider. (i.e. CTG, CE enrollment, clinical staffing, etc.)</li><li>
 Ensuring that all paperwork related to client and program activities is properly completed, managed confidentially and stored in an appropriate location and submitted in a timely manner.</li><li>
Assist in resolving conflicts utilizing de-escalation skills and trauma informed practices.</li><li>
Evaluate own performance to increase effectiveness as a Care Coordinator and maintains willingness to accept supervision and direction.</li><li>
Maintain positive, professional interactions co-workers, and all other community providers including; Homeless Resource Center staff, emergency services, fire, police and sheriff, criminal justice, staff from other community providers, neighborhood residents and businesses, client family members, agency donors, community support groups and referral sources.</li><li>

The primary function of the Community Outreach Hygiene Promotor is to ensure the delivery of the WASH Hygiene promotion and community mobilization activities in the coordination and guidance of the Assistant, WASH - Hygiene Promotion or Officer WASH or WASH Manager or WASH Coordinator.</p>

Conducting hygiene campaigns, house-to-house hygiene promotion, elaborating clear and adapted messages/activities for beneficiaries, and overseeing the hygiene promotion and other WASH activities conducted by the community volunteers.</p>

The role provides data required in close cooperation with the team. Besides the Assistant, WASH - Hygiene Promotion ensures the capacity building of assigned local staff and stakeholders in WASH-related program implementation is delivered.</p>

<ul><li>Deliver the implementation of the aims and objectives of the WASH response program in line with the WASH Coordinator or Manager's WASH guidance and WASH strategy.</li><li>Act as a mediator between IMC and the target communities.</li><li>In collaboration with other actors, deliver suitable hygiene promotion activities that suit the local context (communities, health facilities, camps, and Schools) and would be widely accepted by beneficiaries.</li><li>Conduct household visits to assess and promote household sanitation and hygiene behaviors.</li><li>Sensitize communities on hygienic behavior and disseminate key messages about Acute Watery Diarrhea (AWD)/Cholera through community radios.</li><li>Deliver hygiene promotion messages at strategic times, including during community events or religious gatherings and through once-monthly community broadcasts.</li><li>Educating food vendors and handlers on food hygiene is key to reducing infections. CHPs will organize training sessions, promote healthy environments, and reduce infectious diseases.</li><li>Improve hygiene and sanitation, eliminate open defecation, promote proper use of sanitation facilities, ensure access to clean water, and encourage hand washing.</li><li>Engage WASH committees to identify, analyze, and suggest practical solutions to the IDP and refugee communities&rsquo; WASH-related challenges.</li><li>Work with the WASH committee on the treatment of water facilities and point of use (POU) and post-distribution monitoring of items in their areas of operation.</li><li>Monitor the proper use, cleanliness, and maintenance of communal/shared sanitation facilities.</li><li>Carry out sanitary surveys around water sources to identify possible contaminants and mitigate water source pollution.</li><li>Submit weekly tally sheet reports to the respective supervisor in a timely and appropriate manner.</li><li>Distribute basic hygiene kits to vulnerable communities with the support of a WASH officer as their supervisor.</li><li>Identify locations/sites for WASH infrastructures (latrines construction/rehabilitation, borehole drilling/rehabilitation, etc.) in the beneficiary communities and coordinate closely with communities to identify informal settlements in host communities.</li><li>Participate in hygiene campaigns and drama shows in schools, community and IDP camps.</li><li>Collect health related data and keep records on activities of hygiene promotion.</li><li>Report challenges and successes during the activities and any other issues that require IMC follow-up/action.</li><li>If assigned, collect water samples from households and water sources and conduct quality tests using the Del Aqua Kit Record all results of the water test.</li><li>Timely reporting of Hygiene Promotion activities to the Assistant, WASH - Hygiene Promotion</li></ul></div>

Assists in obtaining support for clients to include but not limited to home care, medical, dental, legal, substance use treatment, mental health, Department of Human Services, clothing, and other entitlement needs.</li><li>
Assist the client in understanding and meeting the expectations of service providers, courts, and employers.</li><li>
Inform clients of opportunities for social support such as recreation, faith-based, or self-help involvement. Make available tickets, transportation, and financial assistance where appropriate.</li><li>
May deliver instructions on various topics that support the client&rsquo;s ability to live independently.</li><li>
Provides ongoing evaluation, supervision, and development of program activities to assure quality and effectiveness of services provided.</li><li>
Implements and maintains procedures related to program operations.</li><li>
Submits required reports and statistics promptly to the program manager as requested.</li><li>
Collaborates effectively with program partners to heighten the efficiency of joint efforts across multiple organizations.</li><li>
Communicate with appropriate program partners, agency staff, community, and criminal justice agencies to ensure ongoing and effective referrals into programs.</li><li>
Serves as Case Coordinator for clients for program care. Works with the Nurse, Peer, and other stakeholders to ensure the client's needs are being met. Coordinates and participates in regular case conferences.</li><li>
Monitors and ensures achievement of program milestones and client outcomes.</li><li>
Oversees client recordkeeping in compliance with appropriate regulatory agencies.</li><li>
Creates and implements life skills training and education opportunities for tenants.</li><li>
Collaborates with referring agencies, funders, and all related internal staff to effectively support individual client needs.</li><li>
Maintains regular contact to coordinate client benefits and address all other treatment needs.</li><li>
Reports to service providers about significant individual behavior or events that will require an immediate response.</li><li>
Acquire concrete resources to meet individual needs.</li><li>
Complete letters of request/referral forms, and other required documents of eligibility or charitable organizations and public agencies to advocate for resources that the client is entitled or eligible for.</li><li>
Makes referrals for education, employment, legal issues, and others as needed.</li><li>
Document the need for financial assistance or other resources such as wrap-around from the project to stabilize client finances.</li><li>
Utilizing various databases and/or online platforms, maintaining all documentation by the agency and appropriate regulatory standards.</li><li>
Participates in relevant Agency meetings and ongoing development of professional knowledge and skills.</li><li>
Actively supports the financial stability of the organization by meeting or exceeding established productivity and documentation standards.</li><li>
Informs supervisor of significant events/issues related to clients as appropriate and follows the program&rsquo;s defined incident reporting process as required.</li><li>
Attends regular program and departmental meetings, case reviews, and CFC quarterly meetings.</li><li>
Maintains and develops professional knowledge by completing recommended education and training, workshops, conference attendance, and readings.</li><li>
Maintains knowledge of regulations and standards that govern client population.</li><li>

Qualifications</b></p><p><b>
Education: </b>Bachelor&rsquo;s degree in social work, human services, criminal justice, or related field preferred. An equivalent combination of education and experience will be considered.</p><p><b>
Credentials:</b> Valid and clean NYS Driver&rsquo;s License. Valid PPD screening is available.</p><p><b>
Experience: </b>Minimum two years of experience with case management and community outreach in areas such as but not limited to persons affected by substance use disorder, prison re-entry, geriatrics, mental health, and refugees. Knowledge of HMIS, Medicaid, SNAP, Social Security, and legal systems is helpful. May require the performance of duties in various community locations and community settings that may not be handicapped accessible.</p><p><b>

<li>Actively acknowledge and engage pedestrians and visitors with a friendly greeting/smile in order to provide directions and/or city information.</li>
Litter removal <li>- While moving through the zone with a rolling trash can that has supplies/tools, remove litter: fast food bags, cups, soda cans, cigarette butts and other debris on the streets/sidewalks and alleys. Remove graffiti and posters from light posts, trash cans, news boxes, etc. Pull weeds.
</li><li>Damp wipe and straighten outdoor surfaces, such as trash can lids, news boxes, etc.</li>
<li>Be watchful for unwanted behaviors or criminal activity and report to the Safety Ambassadors via two-way radios. Must be able to complete basic daily reports of tasks completed which requires being proficient in speaking, reading and writing the English language.</li>
