Key Responsibilities<ul><li>
Maintain the cleanliness and curb appeal of all exterior areas including common spaces, grounds, amenities, and parking areas</li><li>
Remove trash and debris from offices, building exteriors, trash chutes, and compactors on a daily or as-needed basis</li><li>
Assist in preparing vacant apartments for new residents, including trash removal and light make-ready tasks</li><li>
Support the maintenance team by cleaning available apartments and model units 
Help troubleshoot and assist with light maintenance and repair tasks (mechanical, carpentry, electrical)</li><li>
Communicate and report any maintenance concerns to the Maintenance Supervisor promptly</li><li>
Support community emergency response and weather-related clean-up when needed 
Maintain cleanliness and organization of maintenance shops and work areas</li><li>
Work collaboratively across teams and contribute to a positive, respectful community environment</li><li>
Collaborate with office staff on community projects or resident needs as requested</li><li>

<li>Provides transportation by driving participants to and from work sites for orientations, training and other activities, as needed.</li>
<li>Attends job development/ job match meetings, and staffing&rsquo; /follow along meetings with referral sources, participants and the customer support team to review progress of the participants.
</li><li>Updates customer case files by completing and filing documents and monthly reports.

SUMMARY DESCRIPTION OF WORK

The Care Team Specialist serves as the behind-the-scenes point person who ensures consistent communication among a client&rsquo;s care team, both within the organization and with external providers, to support optimal care. Community Support Specialist

As a Community Support Specialist with Thresholds&rsquo; Substance Use Treatment program, you will help people with substance use disorders through an array of harm reduction services.

This team serves a population at high risk of opioid overdose and involvement in the justice system. With your team, you will do outreach at shelters or on the street at community events to distribute and educate clients on naloxone, the opioid overdose reversal medication. You will also make direct referrals to a wide variety of services, including substance use or mental health treatment. After a client has been engaged in services, you will remove barriers to housing, employment, and mental healthcare through case management. You will help clients solidify their recovery and strengthen daily living skills as you drive them to doctors&rsquo; appointments, to benefits offices, or to the grocery store. <li>Master&rsquo;s degree in Psychology, Social Work, or related field</li>
<li>CADC preferred</li>

Our agency is looking for an experienced and skilled CBRS worker to provide case management and community based rehabilitative services to participants in our mental health agency. This position utilizes a variety of approaches and interventions that promote recovery and successful community integration to children and adults with emotional, behavioral and psychological problems. CBRS will provide individualized support and skills teaching in the community to promote new role development and assist individuals in gaining access to needed psychiatric, medical, social, residential, financial and other services appropriate for the individual. If you have a license-able degree, you must be licensed. Applicant must be willing to work with adults as well as children in the community and their homes.
