Position Overview:
<ul>
<li>The Community Support Specialist (CSS) is a vital care service based on the individual needs of children and their families.</li>
<li>The CSS is responsible for delivering an array of accommodating community services, assessing and linking children and families with social, educational, vocational, and recreational services.</li>
<li>Collaboratively working with children and families, the CSS provides precise, measurable and individual services that focuses on the child&rsquo;s and family&rsquo;s abilities to assimilate within their community as well as to identify and access community services.</li>
<li>Since the CSS is an integral resource for children, families and their communities and because it reflects Canopy Children&rsquo;s Solutions core values, the work functions are comprehensive and include, but are not limited to, identifying and accessing needed community services, implementing evident-based practices specific to individual children and families, connecting them within their communities and tracking their progress.</li>
<li>This role in the PMHC also supports the clinic by answering incoming calls; greets and directs visitors who enter the CBHS Outpatient Clinic; takes and processes payments from clients. Community Supports

<li>Assists children and families to gain access to needed community support services including, but not limited to, education, medical, social, vocational, recreational, therapeutic and other social services.</li>
<li>Assess the strengths and service needs of the children and families.</li>

<li>Provides transportation for families in a safe and responsible manner.</li>

Professional Standards

Administrative Support
<ul>
<li>Attends weekly staff meetings.</li>
<li>Works in collaboration with supervisor, therapist, and other Canopy Children&rsquo;s Solutions staff to monitor progress.</li>
<li>Assist with aftercare planning to ensure referrals are made, appointments are made, and services are available/accessible.</li>
<li>Sits in on telehealth/telepsychiatry during scheduled times.</li>
<li>Schedules medical appointments for families when needed.</li>
<li>Relays changes in medical information including medicine changes to the PMHC nurse.</li>
<li>Assures any new psychotropic medications have a release signed by the guardian consenting to the medication prescribed.</li>
<li>Other duties as assigned.</li>
</ul>
Required Qualifications
<ul>
<li>Minimum of Bachelor&rsquo;s degree in social work or related field and at least two (2) years of experience working directly with children and families.</li>
<li>Must meet the minimum requirements to be certified through the Mississippi Department of Mental Health as a Community Support Specialist.</li>
<li>Must demonstrate competence implementing evident-based practices, best practices working with children and families.</li>
<li>Must demonstrate competence in completing reports related to payment for services provided.</li>
<li>Computer skills required.</li>
<li>Must be at least 21 years of age, have a valid Mississippi driver license, a good driving record, proof of legally appropriate auto liability insurance, and must be willing to use personal vehicle, as needed, for agency business</li>

Peer Case Managers working with clients enrolled in Foundational Community Supports program will be expected:<ul><li>
Provide screening and assessment of people seeking Foundational Community Supports utilizing needs assessment and prioritization tools as well Housing First/client-centered practices.</li><li>
Assist client in developing a comprehensive Housing Stability Plan: Assess barriers to housing as well as the client&rsquo;s strengths and needs, and develop a strength-based and client-centered plan for housing and housing retention.
</li><li>Provide 1:1 supportive service as outlined in client&rsquo;s Housing Stability Plan and complete required documentation within established timeframes.
</li><li>Attend Foundational Community Supports related trainings
</li><li>Attend regular staffing to discuss Foundational Community Supports outcomes with other partners and community agencies that specialize in Foundational Community Supports Services.</li></ul> 
Individualized Service Planning: Utilizing motivational interviewing strategies, the skilled Peer Case Managers will demonstrate the ability to:<ul><li>
Assist the consumer in assessing options.</li><li>
Reach agreement with consumers about activities that will contribute to healing and support and assist in communicating that to others.</li><li>
Review and analyze the plan and make suggestions for improvement/ modifications that would improve consumer participation.</li></ul> 
Implementing Services: The skilled Peer Case Manager will demonstrate the ability to:<ul><li>
Provide a range of supportive/helpful interventions and activities as agreed to with the consumer and documented in the plan.</li><li>
Adapt activities and interventions to enhance focus on strengths, needs and creativity without changing the basic nature of agreements and plan.</li><li>
Assist consumers in analyzing progress toward vision and goals and encourage their feedback to their team.</li><li>
Communicate ideas by using own life as a learning and teaching tool (when helpful to consumer and plan).</li><li>
Communicate progress and concerns to the appropriate co-workers.</li><li>
Participate in continuous improvement efforts by sharing perspectives in agency activities such as Quality Improvement reviews, staffing or other means.</li><li>
Seek assistance and report incidents to the right people at the right time.</li></ul> 
Transitioning Services to Natural and Other Community Supports: The skilled Peer Case Manager will demonstrate the ability to:<ul><li>
Negotiate and facilitate transitioning to other services and natural support systems, providing contacts of who will be helpful, and providing follow up.
</li><li>Reach agreement with the consumer about their transition.</li><li>
Mediate agreement between the consumer and the agency about transition timing, when needed. 
HIPPA: The skilled Peer Case Manager will demonstrate the ability to:<ul><li>
Responsible for upholding a culture of privacy and security in highly confidential work environment (HIPAA covered entity service area).</li></ul>
Complies with all CCSWW policies and procedures that involve access to and safeguarding of client Protected Health Information (all forms).</li></ul></li></ul>
Qualifications:

Minimum Qualifications<ul><li>
Must have Recognition as a Peer Specialist by DBHR.</li><li>
High School/GED.</li><li>
1year experience working with homeless populations.</li><li>
Availability to work flexible hours (evenings, nights, weekends).</li><li>
Proof of negative TB test within past 12 months.</li><li>
Obtainment of an Agency Affiliated Counselor License (HIV/AIDS training required).</li><li>
Ability to obtain a CPR/First Aid Certification.</li><li>
Applicant must successfully pass required background checks prior to an offer of employment.</li><li>
Must have reliable transportation, valid driver&rsquo;s license, and automobile insurance.</li><li>
Must have an acceptable driving record per CCS&rsquo; driving policy. (required to provide current driving abstract).</li><li>

<div>ATTIC Correctional Services is currently recruiting for multiple part-time Transport and Monitoring Specialists, to provide community based services to persons with a history of sexual violence following their release and transition back to their home community.
The Transport and Monitoring Specialist is responsible for providing monitoring checks at supervised residence(s), as well as other supervision services such as transportation to authorized activities and supervised visits on an as-needed basis. Hours for this position are variable, a majority of hours are during the weekday but can vary including evenings/ weekends etc. - However there will be notice given for planning and part time hours if out side of usual weekday hours.
Duties include, but are not limited to:<ul><li>
Obtaining schedules and assignments of day-to-day tasks from the Scheduler.</li><li>
Transporting clients to activities.</li><li>
Monitoring client(s) attitude, appearance and behavior.</li><li>
Completing required log entries and apartment checklists each monitoring check.</li><li>
Ensuring that client(s) observe program rules, policies, and procedures.</li><li>
Monitoring the physical residence to ensure the health, safety, and security of the client(s), community, and property.</li><li>
Documenting and reporting client violations to direct supervisor and DHS.</li><li>

Education &amp; Work Experience
