This job serves as the single point of contact for members to coordinate all of the member&rsquo;s care needs across the various service delivery systems and community supports. This is a full-time community-based position requiring frequent travel within the assigned territory in DE. A significant portion of this role involves working directly with members in their homes and also requires providing case management services within nursing facility settings. The incumbent will travel to members&rsquo; homes, nursing facilities, and other community-based settings for individuals enrolled in DSHP Plus LTSS and DSNP.
ESSENTIAL RESPONSIBILITIES
<ul><li>Conduct regular in-home and nursing facility visits: Travel to members&rsquo; homes, nursing facilities, and other community-based settings to complete face to face needs assessments with subsequent telephonic contact with the member in accordance with state and national guidelines, policies, procedures, and protocols. This includes actively working within the nursing facility environment and participating in NF care plan conferences to ensure member needs are met.
</li><li>Assess, plan, coordinate, implement and evaluate care for eligible members with chronic and complex health care, social service and custodial needs in a nursing facility or home and community-based care setting.
</li><li>Coordinate care across the continuum of services and assisting members physical, behavioral, long term services and supports (LTSS), social, and psychosocial needs in the safest, least restrictive way possible while considering the most cost-effective way to address those needs. This includes ensuring appropriate care transitions between home, community, and community-based care settings.
</li><li>Authorize LTSS services based upon completion of a comprehensive needs assessment. Coordinate HCBS services, Medicaid and DSNP benefits and assess appropriateness of care and services in community.
</li><li>Facilitate transitions to alternate care settings such as hospital to home, nursing facility to community setting using an integrated care team to address the member&rsquo;s specific needs.
</li><li>Educate members or caregivers regarding health care needs, available benefits, resources and services including available options for long term care community or facility-based service delivery.

About the Role

The Case Manager will have the prime responsibility for organizing comprehensive service planning and assessment activity and assuring service plan implementation through direct case management or co-provider services. The Case Manager will also have either primary or shared responsibility, depending upon the specialty/discipline, in providing the residents with specific linkages/advocacy services with co-providers of housing, mental health, substance abuse, and healthcare services etc. as well as with parole boards. Together with the residents, the Case Manager will formulate residents' comprehensive service plan in conjunction with specialty program staff. For some residents the Case Manager will be the major provider of a particular service and, for others, a secondary support agent.

Position is responsible for services including, but not limited to, assistance to eligibleMedicaid participants receiving waiver services to enable them to remain in the community setting. You are looking to be part of a team that goes the extra mile to accomplish more for each individual and family we work with.Schedule:Monday &ndash; Friday 9am-5pmPrincipal Responsibilities:Managing a caseload of up to 16 assigned individuals/families experiencing chronic homelessness with a concurrent diagnosis of substance use or mental health needs.Engage in rapport building with participants and provide service from a &ldquo;choice&rdquo; perspective while utilizing participant-centered, harm reduction, and strengths-based approaches.Provide case management services including assessment, referrals to community supports, and creation and monitoring of an individual action plan to accomplish participant identified housing and treatment goals.Schedule and maintain consistent, meetings with assigned individuals to have structured, intentional conversations focused on obtaining housing and supportive service options.Document case management services through detailed and thorough case notes in Ehana for each participant to include bi-weekly meetings, client and collateral contacts each month, as well as any significant events and/or interactions.Update assessment and action plans in accordance with the CSP-CHI requirements using Ehana.Maintain organized client files both hard copy and electronic in compliance with contract requirements.Develop a network of social service providers and refer participants to appropriate services such as financial assistance, entitlements, addiction services, advocacy, employment assistance, financial counseling, rental assistance, deposits, Domestic Violence services, Mental Health services, etc. and assist with follow up for those referrals.Educational Requirements:<ul><li>Bachelor's degree in Mental Health, Psychology, or Social Services.</li><li>Experience working with individuals and/or families who are experiencing chronic homelessness and have a concurrent diagnosis of substance use or mental health needs.</li><li>Lived experience and recovery coaches with case management experience will be considered in lieu of a degree.</li></ul>We offer competitive benefits for full-time employees including: <ul><li>Generous paid time off: 11 holidays, 2 personal days, 3 weeks for vacation, 10 sick days, and your Birthday!</li><li>Two health plans with 4 tiers each. OVERVIEW
Position Summary
Yellowstone Boys and Girls Ranch Targeted Case Managers manage all aspects of a youth&rsquo;s case while providing critical community resources to support youth and families. Targeted Case Managers provide wrap-around support for families while working collaboratively with current and prospective services to meet any youth&rsquo;s complex needs.
RESPONSIBILITIES
Documentation
Complete, update, and maintain all necessary documentation within the guidelines set by COA, Montana ARM's, Contracts, and YBGR Policy and Procedures. Complete, submit, and verify accuracy of paperwork, including but not limited to progress notes, intake paperwork, etc., while maintaining Performance Quality Improvement (PQI) and financial viability of the program.
Collaboration
Collaborate with the youth, the youth's family, and other professionals involved to develop a treatment/service plan that addresses the specific needs of the youth including, facilitating, and leading the treatment team meetings. Maintain contact with other service providers to ensure the unique needs of the youth and family are best met.
Support
Advocate, support, and guide youth and families in obtaining the needed services including, but not limited to, advocacy, assessing youth and family&rsquo;s needs, referring to services, creating and implementing service plans, and monitoring services of the youth and family. Research the various services offered within the community to identify services that would best meet the unique and individual needs of youth and families.
REQUIREMENTS
Qualifications
21 years old or older. Criminal / Child Abuse Clearance: Must pass criminal record and child abuse registry background check as required by the State of Montana. Education: Bachelor&rsquo;s degree in human services related field of study. A combination of previous professional experience and formal education from an accredited university may be considered in lieu of degree.

The ability to complete forms within an electronic Medical Health Record System typing at least 35 words per minute while using a keyboard and mouse.
