Your Role as a Home Health Physical Therapist</b></div><div>
As a Contract Physical Therapist, you will provide skilled therapy services in patients&rsquo; homes to help them regain strength, mobility, and independence following illness, injury, or surgery.</div><div></div><div><b><br>
Responsibilities include:</b></div><div><ul><li>
Perform in-home physical therapy evaluations and treatments</li><li>
Develop and implement individualized care plans</li><li>
Complete Medicare OASIS documentation, including: Start of Care (SOC), Resumption of Care (ROC), Recertifications, Discharges</li><li>

Educate patients and caregivers on mobility, safety, and recovery</li><li>
