Job Duties

Position provides comprehensive case management services by ensuring all relevant classification paperwork is timely, accurate and complete. Ensures inmates are appropriately employed during their incarceration. The position focuses on adult inmate release planning and performing case management/counseling which includes classification. Promotes the development of self and staff through providing training. Acts as a role model by providing guidance/direction for inmates in a correctional facility, which enhances facility security and promotes long-term pro-social behaviors.

Minimum Qualifications

Experience in counseling, rehabilitation, or casework management. Experience facilitating rehabilitative programs. Experience preparing reports. Working knowledge of principles and practices of counseling, of casework and treatment methods, of behavior modification, various intervention techniques and inmate restoration rehabilitation. Demonstrated ability to counsel and interact with inmates to develop, implement, and follow through on treatment plans and to communicate effectively both verbally and in writing. Proficiency in Microsoft Office Suite.

Additional Considerations

Advance knowledge in Social Services, Counseling, Criminal Justice or other areas related to human services or equivalent combination of training and experience. Previous experience working in adult counseling or corrections. Working knowledge of Microsoft Office (Outlook, Word, and Excel). Certification in a specialized area, such as Certified Substance Abuse Counselor, CSAC, Certified Sex Offender Treatment Provider, CSOTP. Bi-lingual candidates are encouraged to apply. Description of Work
This position is located at Craven Correctional Institution in Vanceboro, NC, Craven County. The prison is a Medium Custody Adult Male Felon Diagnostic Processing Center which houses 850 offenders. Employees in this class carry a caseload of inmates, which involves supportive counseling, conducting assessments, developing case plans, answering and following up on inmate questions and problems, and making recommendations to the classification committee.

Basic Purpose
Positions within this job family are assigned responsibilities involving providing routine social casework in the classification and assignment of offenders committed to a correctional institution or advanced social casework pertaining to the re-entry of offenders into the community.

Level I:

Level II:
This is the career level where incumbents perform a full range of case management tasks involving the classification and assignment of offenders committed to a correctional institution. Incumbents at this level are responsible for overseeing difficult advanced classification and counseling cases. 
Level III:
This is the specialist level where incumbents are responsible for providing programs such as substance abuse treatment, cognitive behavior and sex offender treatment; and offender re-entry case management functions to include; coordination of strategic social casework; classification; development of transitional/intervention plans; conducting and analyzing offender assessments; developing family and community collaborative support teams, and providing service referrals and follow-up.

Corrections Counselor I

 The overall purpose of case management in the Onslow County Detention Center is to assess an offender’s criminogenic needs and connect them to treatment and services, helping them remain substance and crime free once released into the community. The Case Manager will work within the Onslow County Detention Center and conduct assessments for all assigned non-violent adults with substance use or co-occurring mental illness and substance use. High priority will be given to those with moderate to high risk of reoffending to reduce the risk of criminal justice recidivism. The Case Manager will coordinate with the current correctional behavioral health providers along with the medical unit and therapy staff to prepare individualized case plans, including referrals to treatment and services upon release from the Onslow County Detention Center. In addition to recovery-based services, the Case Manager will address housing, employment, education and health and wellbeing. Case management services and support takes place both in the Onslow County Detention Center and in the greater Onslow County community and surrounding counties if indicated. Case management will continue for a minimum of 90 days after release with 6 month and beyond data tracking. The Case Manager II/Trainee- COPES provides clinical case management services to adults receiving long-term term services and supports through an Aging and Long-Term Care of Eastern Washington (ALTCEW) program. Evaluate client strengths and needs, complete a comprehensive assessment, determine level of functioning, develop and implement service plans that provide a reliable and safe plan of care that enables clients to achieve a maximum level of health and independence and targets case stabilization, ensuring independence. Complete administrative duties and meet required program deadlines. Coordinate with and perform advocacy with relevant parties and community agencies.
ESSENTIAL DUTIES AND RESPONSIBILITIES includes but is not limited to the following:<ul><li>
Adhere to program standards and guidelines and follow case management program protocols.</li><li>
Provide long-term care services responsive to the needs of elderly and adults with disabilities who are eligible for Title XIX Program services.
</li><li>Conduct face-to-face comprehensive assessments at the client's residence to determine level of care, including determining nursing facility level of care needs.</li><li>
Develop ongoing care plans which provide the support necessary to allow clients to live independently.</li><li>
Facilitate the coordination of in-home caregivers (Individual Providers and Agency Providers) for client care needs.</li><li>
Coordinate with ALTCEW and provide advocacy, coordination, consultation and support to clients, providers and stakeholders across multiple disciplines and systems.</li><li>
Perform program charting requirements, correspondence, and other paperwork accurately and in accordance to program deadlines.</li><li>
Authorize service payment through Provider One/Individual Provider One.</li><li>
Conduct interdisciplinary case reviews and consultations.</li><li>
Establish and maintain a good working relationship with FBH staff and community partners. Perform other related duties as required.</li><li>
Comply with all State WAC requirements and FBH policies and procedures.</li><li>
Requires travel to see clients in the community and travel between work sites.</li><li>
Other duties as assigned.</li></ul>
QUALIFIED CANDIDATES WILL HAVE:
REQUIRED:<ul><li>
High School diploma or its equivalent and one year paid full time on-the-job social service experience or one year full-time equivalent verifiable hours through an internship or volunteer work in social services. Part-time hours may be considered if hours total one year full-time equivalent.</li><li>
Must apply for a Counselor Agency Affiliated &ndash; Registered credential from the WA State Department of Health within 30 days of hire.</li><li>
Must have an unrestricted and valid driver&rsquo;s license, active car insurance and must be willing to use their personal vehicle in the course of work.</li></ul>
PREFERRED:<ul><li>
Master&rsquo;s degree in behavioral or health sciences and one year paid on-the-job social service experience; OR</li><li>
Bachelor&rsquo;s degree in behavioral or health sciences and two years of paid on-the-job social service experience; OR</li><li>
Master's degree in any other field, and three years of paid on-the-job social service experience. OR</li><li>
Bachelor&rsquo;s degree in any other field and four years of paid on-the-job social service experience.</li></ul>
Applicants who do not meet the PREFERRED qualifications will be hired as a &ldquo;Case Manager Trainee&rdquo;. Case Manager Trainees will no longer be considered a Trainee when all conditions below have been satisfied regardless of whether their length of employment as a Case Manager Trainee would qualify them under the preferred qualifications to be a Case Manager II.<ul><li>
Participate in a two-year, on-the-job case manager training program under direct supervision;</li><li>
Participate in a monthly supervisory review of a sample of client assessments and service plans;</li><li>
Participate in annual performance evaluations conducted by the supervisor; and</li><li>
Perform competently as a case manager at the end of two years as determined by the supervisor&rsquo;s performance evaluation.</li><li>
Agency has the discretion to reduce the timeframe to one year for exceptional performance by the Case Manager Trainee as determined by the supervisor&rsquo;s performance evaluation.</li></ul>
KNOWLEDGE, SKILLS &amp; ABILITIES:
<ul><li>Ability to communicate and engage effectively with individuals of varied backgrounds, cultures, education level and socio-economic status.</li><li>
Knowledge of elder care and disabled adult caregiving issues.</li><li>
Ability to perform face-to-face assessments and screenings.</li><li>
Knowledge of data entry and computer systems to complete tasks.</li><li>
Ability to complete administrative tasks and organize and prioritize tasks.</li></ul>

JOB SUMMARY

Performs advanced case management work. Work involves coordinating case management 
activities developing program goals, objectives, and procedures; and evaluating outcome measures 
for the program. Works under minimal supervision with extensive latitude for the use of initiative and 
independent judgment.

ESSENTIAL FUNCTIONS

A. Performs complex pre-release case management for the reentry and integration of inmates 
discharging from a correctional facility; assists in the development of program policies and 
procedures; assists with program monitoring and evaluation; and assists with coordinating 
activities to produce an effective reentry transitional process.

B. Reviews inmates referred to the program and tracks progress through the case management 
process; conducts inmate assessments to identify workforce and reentry barriers; implements 
criteria for identification of inmate needs; and develops and implements case plans to address 
removal of inmate deficiencies.

C. Coordinates with appropriate agencies in obtaining necessary inmate identification documents; 
assists in inmate eligibility and application for various reentry benefits and services; and works 
with and provides liaison with program staff, government agencies, inmate families, faith-based 
support groups, and other organizations.

D. Develops and maintains records on participating inmates; documents case records and reviews 
reports; and compiles and analyzes data and prepares summary reports.
<ul><li>Performs a variety of marginal duties not listed, to be determined and assigned as needed.</li></ul></div><div>
MINIMUM QUALIFICATIONS

A. Education, Experience, and Training

1. Graduation from an accredited senior high school or equivalent or GED.

2. Four years full-time, wage-earning case processing, case management, social services, 
criminal justice, or human services experience. Fifteen semester hours from a college or 
university accredited by an organization recognized by the Council for Higher Education 
accreditation (CHEA) or by the United States Department of Education (USDE) may be 
substituted for each six months of experience.

3. Reentry programs, transitional employment planning, parole, or probation experience 
preferred.

4. Computer operations experience preferred.

5. Governmental agency experience preferred.

B. Knowledge and Skills

1. Knowledge of case management principles, objectives, standards, and methods.

2. Knowledge of community and government service delivery systems and case management 
delivery systems.

3. Knowledge of agency programs that address inmate assessment and inmate needs 
preferred.

4. Knowledge of job market and job training procedures preferred.

5. Knowledge of agency and departmental organizational structure, policies, procedures, 
rules, and regulations preferred.

6. Knowledge of motivational interviewing concepts and techniques preferred.

7. Skill to communicate ideas and instructions clearly and concisely.

8. Skill to coordinate with other staff, departments, officials, agencies, organizations, and the 
public.

9. Skill to interpret and apply rules, regulations, policies, and procedures.

10. Skill in problem-solving techniques.

11. Skill in the use of computers and related equipment in a stand-alone or local area network 
environment.

12. Skill to prepare and maintain accurate records, files, and reports.

13. Skill to assess inmate needs and coordinate inmate services.

14. Skill in interviewing and conducting individual needs assessments.

15. Skill in motivational interviewing preferred.
