POSITION SUMMARY</b><br>
The caseworker for the Kenosha Permanent Housing Connections Program provides comprehensive supportive services to people who are experiencing chronic homelessness in scattered site living arrangements. Using a person-centered approach to service delivery the caseworker is responsible for assessing need, developing and working with a housing stability plan and applying individualized interventions to assist participants achieve their self-defined goals.</p><p><b>
RESPONSIBILITIES</b></p><p>
 1. Provide individual case management to program participants on a regular and consistent basis as determined by the individual&rsquo;s case plan</p><p>
 a. Assess, plan, coordinate, implement and evaluate the overall service delivered to the participant</p><p>
 b. Assist clients obtain safe secure living arrangements by teaching apartment searching skills</p><p>
 c. Assist clients move in and out of apartments/houses</p><p>
 d. Help participants learn to live in housing, maintain their housing in a safe manner, and get along with fellow tenants and the landlord</p><p>
 e. Create a comprehensive Housing Stability Plan with the participant and provide appropriate interventions to assist with goal achievement</p><p>
 f. Help participants create support systems and participate in the community, as they desire</p><p>
 g. Provide individualized budgeting and money management services to program participants as needed</p><p>
 h. Teach basic, intermediate and advanced social skills based on need</p><p>
 i. Collaborate with landlords and other community service providers</p><p>
 j. Coordinate opportunities for educational advancement, such as GED preparation and attainment, post-secondary training, and vocational education</p><p>
 k. Assist with job preparation and attainment, such as career counseling, job preparation training, dress and grooming, job placement and job maintenance</p><p>
 l. Provide assistance in accessing mainstream benefits, including food stamps, childcare assistance, and health insurance</p><p>
 m. Assist participant access Mental Health, AODA and Medical services as needed</p><p>
 n. Provide assistance with ensuring participants have access to food and clothing</p><p>
 o. Provide transportation and assist in developing transportation skills</p><p>
 2. Ensure documentation is complete and accurate</p><p>
 a. Use Client Tracking System to document client contacts and progress</p><p>
 b. Complete data entry into Wisconsin Service Point (WISP) as required</p><p>
 c. Complete check requests on a monthly basis and submit on time</p><p>
 d. Complete and maintain client files and ensure the information matches the data entered in WISP</p><p>
 e. Complete and file discharge plans appropriately</p><p>
 3. Participate in the program admission process and established goals</p><p>
 a. Review the Coordinated Entry Assessment and VI-SPDAT in Wisconsin Service Point.</p><p>
 b. Provide clients with a program orientation at intake meeting</p><p>
 c. Acquire necessary releases of information to gather background information and share information for the required collaboration of services and mainstream resources</p><p>
 d. Obtain third party verification of homelessness that determines eligibility for people experiencing chronic homelessness that includes documentation of the mental health diagnosis from a qualified licensed professional</p><p>
 e. Understand and assist with the management of program and system performance measures</p><p>
 f. Participate in the bi-annual Point-in-Time Event</p><p>
 g. Collaborate with Emergency Shelters, Homeless Outreach Workers and other service providers working with people who are experiencing chronic homelessness</p><p>
 4. Meet with supervisors to review program, paperwork and case management issues</p><p><b>
QUALIFICATIONS: Bachelor&rsquo;s Degree in a human service-related field and/or experience working with individuals </b><b>and families experiencing homelessness and/or other issues that put individuals at risk of housing instability.</b></p><p>
 1. Ability to effectively communicate, both written and verbally, in the English language</p><p>
 2. Must have reliable transportation and valid driver&rsquo;s license with good driving record</p><p>
 3. Must carry own auto insurance with at least 100,000/300,000/100,000 coverage</p><p>

Key responsibilities of the position include answering customer phone calls and assisting with case inquiries, case maintenance, determining eligibility for financial assistance, Medicaid, SNAP, LIHEAP, nursing home care, and waivers, as well as any additional programs the department administers for applicants and recipients in accordance with state regulations. In order to accomplish these duties, you will interview customers over the phone; review, interpret, and correctly apply policy; data enter information into an automated eligibility determination system, as well as perform manual calculations; type clear and thorough case comments about client circumstances and changes. All work must be completed within deadlines in accordance with federal and state policy. In this role, it is essential to comply with regimented policies and procedures and stay current with major changes impacting knowledge and skill. These job duties are carried out under the general supervision of an Income Maintenance Casework Supervisor.<br>

Responsibilities:
<p><b>
Summary</b></p><p>
Provides counseling services, assists the inmates/detainees in adjusting to facility life and obtaining needed services or changes in assignment. Develops and implements programs within the facility to meet the individual needs of inmates/detainees, to include individual and group counseling.</p><p></p><p><b><br>
Primary Duties and Responsibilities</b></p><ul><li>
Interviews newly admitted inmates</li><li>
Conducts facility orientation for newly assigned inmates/detainees explaining rules and regulations of the facility, available services and programs, and the disciplinary and classification process.</li><li>
Provides individual counseling to inmates/detainees on an informal basis regarding work and housing assignments, adjustment problems, and family problems.</li><li>
Resolves day-to-day problems with counseling. Refers inmates/detainees with serious problems to professional counseling staff.</li><li>
Provides assistance to the inmates/detainees in obtaining needed services. Serves as a representative for the inmate/detainee at classification committee meetings.</li><li>
Monitors inmate/detainee custody, housing, job and program assignments to ensure all cases are reviewed in a timely manner.</li><li>
Conducts visits to inmates/detainees assigned to caseload who are housed in special housing.</li><li>
Reviews inmate/detainee medical status to determine eligibility to work in volunteer job assignments.</li><li>
Performs other duties as assigned.</li></ul>
Qualifications:
<p><b>
Minimum Requirements</b></p><ul><li>
High school diploma or equivalent certification required.</li><li>
Bachelor's degree in criminal justice, sociology, or related field is preferred.</li><li>
One (1) year in caseload management of offenders in a correctional setting required or an equivalent combination of education and related work experience required.</li><li><b>
MUST have worked an Arizona Department of Corrections security post, or completed COTA, within the last 2 years.</b></li><li>
Must have valid AZ Drivers License</li></ul><div>
