OVERVIEW
 
Provides case management for a caseload of general probation clients to help them successfully complete probation, by applying appropriate behavioral change techniques while providing supervision and monitoring compliance according to Intervention policies and procedures.
 
ESSENTIAL DUTIES AND RESPONSIBILITIES

<li>Perform assessments/reassessments as required.</li>
<li>Develop individualized supervision case plan for each client based upon established protocols.</li>
<li>Detail and explain verbally to the client, the client's probation conditions and case plan to ensure the client understands his/her responsibilities, court orders and expectations.</li>
<li>Meet with client per policy to evaluate progress in accomplishing case plan and probation terms, administer rewards and sanctions according to established policies and procedures.</li>
<li>Document all direct and collateral contacts with all clients.</li>
<li>Conduct orientation meetings with new clients and explain program rules and regulations.</li>
<li>Assess and collect client fees.</li>
<li>Enter client data into electronic database accurately, timely and completely.</li>
<li>Report all program violations to the referral agency via the established Intervention policies.</li>
<li>Complete all written reports concerning clients in an accurate, legible and timely manner. Prior to sending any reports to the courts or referral agency approval must be given by a supervisor.</li>
<li>Refers clients to community resources as appropriate.</li>
<li>Monitor drug and alcohol tests as required.</li>
<li>Work with courts, probation and criminal justice agencies regarding client staffing/compliance.</li>
<li>Travel to the jail to meet with clients.</li>
<li>Provide own transportation to travel to court, jail and/or other sites as required, including in adverse weather.</li>
<li>Testify in court regarding client compliance.</li>
<li>Attend required trainings and meetings; apply skills and information.</li>

<li>EDUCATION:  Bachelor's degree (BA/BS) from accredited four-year college or university in criminal justice, psychology, social work, counseling, a social science or other closely related field. </li>
<li>SUBSTITUTION FOR EDUCATION:  Work experience in the areas of counseling, social work, law enforcement, corrections or community supervision which provided the same kind, amount, and level of knowledge acquired in the required education, will substitute on a year-for-year basis for the bachelor's degree. </li>

 Purpose of the Role:
 
 
 
 
 The Criminal Justice Case Manager will provide time-limited (12 months) wrap-around services to support clients who experience barriers because of a criminal history. These services include: administering risk and needs assessments; assisting clients with their job search and connecting them to job training; legal advocacy; accessing public benefits and housing assistance; navigating community supervision (probation); and other case management needs. The Criminal Justice Case Manager will work closely with internal programs and external partners to empower clients of the program to have the skills and confidence to thrive as active participants in the community.
 
 
 
 
 Essential Functions of the Role:

 Criminal Justice Case Managers will have the vision and passion to support the development, implementation, and growth of all aspects of the Re-entry Intensive Case Manager Program and provide the following services:

 Assist clients during sentencing to remain active in the community and be able to link clients to services to provide skill building and independent living skills.

 Conduct follow-up outreach of direct/or self-referred clients who are eligible and in need of re-entry services.

 Complete initial assessment and follow-up re-assessments.

 Provide direct services to clients through regular case management meetings.

 Help clients navigate the challenges that come with having a criminal background: possible homelessness, unemployment, employment and housing discrimination, domestic abuse, recovery, family issues, parole/probation issues, and legal difficulties.

 Provide supportive service linkages to community resources, such as homeless and housing, occupational training, public benefits, healthcare, mental health services, support groups, etc.

 Follow up with clients to ensure their situations have improved.

Provides intensive, complex and professional level case management services to assigned clients in a criminal justice program; interviews clients; completes state mandated assessments; analyzes data from assessments and uses them to develop a performance based case plan; monitors client&rsquo;s progress; and prepares reports and documentation for case file, reports to referrals agencies and the courts. May also be assigned a specialized caseload, including sexually violent offenders, domestic violence offenders, or those in a residential drug treatment program. ESSENTIAL DUTIES AND RESPONSIBILITIES
 Conducts client intakes including orientation, paperwork, psychometric assessments, assessment review, service recommendations based on client’s risk, needs, strengths and responsivity factors, and client-centered Individualized Service Plans (ISPs) within established timeframes.
 Provides individual case management sessions to increase the probability of service completion, goal attainment and effective life functioning. Duties include community linkages, use of standardized case management approaches (e.g., Strategic Case Management, Life Skills) and tools (e.g., Carey Guides), and vocational and educational support.
 Develops Transition Plans that include connection to relevant community resources and prosocial supports designed to enhance and maintain client success.
 Prepares, plans, and facilitates individual and group sessions, including booster sessions, with integrity and fidelity to curriculum in a solution-oriented manner.
 Coordinates client care within agency and with other community providers.
 Assesses clients’ basic needs, providing essential items and connecting clients to services and agencies that assist with basic needs. Maintains and updates community resource guide. Networks in the community, as determined by CSSD.
 Utilizes motivational interviewing techniques to engage clients and their natural supports in AIC programming.
 Monitors client behaviors and uses behavior management systems. Holds clients accountable and addresses ambivalence and resistance while helping clients maintain motivation to change.
 Maintains accurate and timely records of case management activities through entry into agency and funder data systems.
 Prepares reports that are accurate and timely, and updates referral sources on client activities on a regular basis.
 Actively participates in quality assurance activities including audio taping sessions and meeting with a quality assurance coach, attending trainings, and data report analysis and review.
 Participates in efforts to meet and maintain performance-based measures set by agency and CSSD.
 Participates in client-centered, strengths-based, presentations/discussions in staff meetings and attends teaming meetings with Probation, as required.
 Models pro-social behaviors and serves as a role model to clients and staff.
 Operates as part of a team by providing program coverage for client services (including urinalysis) outside of normal work schedule, typical job function or primary work site, as requested (e.g., additional evening shifts may be required to ensure operational coverage).
 Supervises toxicology testing. Supervised urine specimens shall only be collected from a client by a staff member of the same gender.
 Continues to develop knowledge and understanding about the history, traditions, values, family systems, and artistic expression of groups served as well as uses appropriate methodological approaches, skills, and techniques that reflect an understanding of culture.
 Regulatory and Corporate Compliance Responsibilities
 Understands and adheres to Wheeler Clinic’s compliance standards as they appear in the clinic’s Corporate Compliance Plan, Code of Conduct, and Conflict of Interest Policy.
 Completes all training in a timely manner as required by state and federal regulations (e.g., Compliance, Fire Safety, HIPAA HITECH, Infection Control, Sexual Harassment, etc.) and specific to the department and position.

What drives the core purpose of this job:
The Forensic Case Manager provides services and identification, screening, treatment planning, case management, and court reporting of eligible participants living with mental illness or co-occurring disorders.
What are the key responsibilities:
<ul><li>Act as liaison to the courts and correctional facilities. Provide case-finding and screening services for cases within the Court in order to ascertain appropriate service plans for participants.</li><li>
Follows and implements a comprehensive screening to effectuate referrals to treatment and accompanies and/or arranges transportation for clients to the appropriate facility when necessary.</li><li>
Submit monitoring reports to the MMHC Resource Coordinator in a timely manner prior to each scheduled court appearance describing participants compliance with court approved treatment plans.</li><li>
Immediately notify the court if participants commit major infractions.</li><li>
Maintain case documentation available for review by supervision at any time, i.e. client records, including progress notes and all other relevant information to assist clients and to comply with program requirements.</li><li>
Assist clients in applying for and receiving public and private benefits.</li><li>
Maintain a caseload and monitor client participation in the treatment program through case management appointments, reviewing progress reports and conferencing cases with treatment personnel.</li><li>
Make periodic visits to treatment facilities and maintain accurate log of visits.</li><li>
Prepare and present written and oral status reports to the criminal justice system in a timely manner regarding client's progress.</li><li>
Ensure weekly, bi-weekly, or clinically appropriate level of contact with program participants and treatment providers.</li><li>
Participate in case conferences weekly or as needed.</li><li>
Investigate and arrange supportive services and/or entitlements for clients.</li><li>
Attend and engage in relevant trainings or continuing education opportunities as directed by Program Director.</li><li>

What key knowledge, skills, and abilities are required to excel in this role:
