SUMMARY OF POSITION</b><b>
</b></p><p>This case manager position will serve clients as part of Centerstone's Mobile Crisis team in Manatee County. This role will respond to crisis events in the local community and provide deescalation, wraparound support, and follow up services. This position will provide mental health, substance abuse and outreach services to individuals who may have a mental health and/or substance use disorder. This role will work with community partners and local stakeholders to provide comprehensive care. The Case Manager will meet with individuals in the office, hospital, and/or their own environment such as in the client&rsquo;s home, schools/work, etc. This role will ensure continuity of care with clients through case management services as well as link clients and their families to needed resources to help to improve their quality of life. A variety of clinical training opportunities are available based on program and interest.</p><p></p><p><b>
ESSENTIAL DUTIES &amp; RESPONSIBILITIES</b></p><ul><li>
Reports to work as scheduled on a daily basis and performs duties as assigned by supervisor to ensure the smooth operation of the department.</li><li>
Provides individual and group mental health and substance use disorder case management services. Refers, advocates, and links client and family for additional services as identified.</li><li>
Provides supportive and educational counseling services (individual, family, and group) to develop daily living and coping skills. Assist clients with developing natural support systems.</li><li>
Transport clients in company vehicles to necessary appointments as needed.</li><li>
Responsible for developing, implementing, evaluating, and updating treatment plans with input from the client and family.</li><li>
Conduct clinical behavioral health assessments.</li><li>
Enters written documentation into Electronic Medical Record in a timely manner for all services rendered.</li><li>
Act as liaison and communicate collaboratively with local law enforcement, and local stakeholders to maintain coordination of the treatment process.</li><li>

<div><div>The 805UndocuFund started as a grassroots response effort to provide financial assistance to undocumented families in both Ventura and Santa Barbara counties following the Thomas Fire and Montecito debris flows. Established by three community organizations (CAUSE, Future Leaders of America, and MICOP), the 805UndocuFund has become a part of our local safety net. The original model rotated responsibilities among organizations; in 2018, MICOP took helm following the Thomas Fire and Montecito debris flows, CAUSE took the helm in 2019 and Future Leaders of America took leadership in 2020 for the COVID19 response efforts. Since its founding, the 805UndocuFund has provided over $8 million dollars to more than 6,000 undocumented families impacted by local disasters and the COVID19 pandemic. Most of our recipients have been Latinx and indigenous community members struggling to find resources outside 805UndocuFund. Due to the immense pressures for the &ldquo;host&rdquo; organization, the 805UndocuFund steering committee moved to create its own 501(c)3 with its own staff and Board of Directors. In its new iteration, the 805UndocuFund is figuring out how to compel local funders to fund the organization year-round as some do not see the need for support. Programming for 805UndocuFund includes &ldquo;small scale&rdquo; response fund, disaster relief, and plans for a statewide advocacy network, the CA UndocuFund Network.

</p><p><b>PURPOSE:</b> The Crisis Case Manager supports individuals experiencing mental health crises in community settings as part of a mobile crisis response team, which includes licensed therapists, case managers, and peer support specialists. This role provides timely intervention, stabilization, and connection to appropriate care, either through Kanza&rsquo;s wraparound services or higher levels of care, such as psychiatric hospitalization. Typical settings include a Kanza office, local hospitals, local law enforcement offices, local schools, or home settings.<br>

<p><b>ESSENTIAL JOB FUNCTIONS:</b></p><ul><li>
Provide crisis stabilization support and observation as part of a crisis team.
</li><li>Manage crisis situations effectively using positive approaches and maintain a therapeutic setting while assessing triggers &amp; precursors to symptoms and risk factors to assist in stabilizing their crisis symptoms.
</li><li>Demonstrate knowledge of program protocols and behavior management in carrying out suicide precautions, welfare checks, detox protocol, and general treatment models as identified.
</li><li>Respond to crisis situations calm &amp; non-reactive by utilizing non-physical de-escalation techniques to intervene with in crisis in a safe manner to de-escalate situations whenever possible. Assists in facilitating and processing crisis debriefings with individuals after crisis events are resolved.
</li><li>Collaborate with medical and law enforcement personnel as needed while protecting confidentiality in accordance with state and federal laws.
</li><li>Participate as a member of a multidisciplinary treatment team, working in conjunction with therapists, case managers, peer support specialists, medication providers, and crisis care staff.
</li><li>Assist with the admissions, discharges, frequent safety checks, appointments and transport of individuals.
</li><li>Follow safety guidelines and provide training when providing observation and support in both the office and community settings to keep the individual, yourself, and other staff safe.
</li><li>Complete all paperwork required, (i.e., progress notes, incident reports, timesheets, mileage, etc.) by the end of each shift.
</li><li>Timely attendance and punctuality to all scheduled shifts, trainings, supervision etc. Provide appropriate notice and follow program protocol on notification prior to absence and tardiness.
</li><li>Establish appropriate and professional boundaries with individuals and community partners Treat individuals and community partners with dignity and respect.

</li></ul><p></p><p><b>WORKING ENVIRONMENT: </b>The environment differs based on the client&rsquo;s needs, and while some care will be provided in a typical office environment, care is often provided in the client&rsquo;s home or another community setting. This position will require the use of a company vehicle for business and requires travel to multiple worksites including other offices, residences, and various places around the community. The Crisis Case Manager may be exposed to low socio-economic homes as well as aggressive situations and needs to be comfortable utilizing de-escalation techniques.
