Under the supervision of a Mental Health Program Director and Program Coordinator, will assess individuals, identify goals, develop a person-centered plan for treatment, implement treatment, and link and coordinate community services, all using the best practice, strength-based person-centered model identified for the consumer. The position also includes responding to mental health crises through rapid response, crisis stabilization, assessment, and connection to community resources, in collaboration with crisis teams and first responders.</p><p><b>
