<div>Responsibilities</div>
<ul><li>Provide individualized Medical Nutrition Therapy (MNT) and counseling for patients with chronic conditions (e.g., diabetes, hypertension, obesity, hyperlipidemia, CKD).</li>
<li>Develop and lead group nutrition education sessions and classes.</li>
<li>Work closely with physicians to build efficient referral workflows within the EHR.</li>
<li>Track and document encounters for billing and quality reporting.</li>
<li>Educate staff on the role of nutrition in chronic care management.</li>
<li>Contribute to patient engagement efforts, health fairs, or outreach initiatives.</li></ul>

<ul><li>Registered Dietitian (RD/RDN) credential required.</li>
<li>Strong understanding of insurance billing (CPT 97802/97803/97804, G0270/71) and ICD-10 codes supporting MNT.</li>
<li>Entrepreneurial drive and a collaborative approach to building services within a practice setting.</li>
<li>Excellent communication and interpersonal skills</li>
<li>The ability to compassionately counsel people making major life changes</li>
