Job Summary:</b></p>

The Hospice Clinical Director is responsible for ensuring the coordination and management of patient care. RESPONSIBILITES</b></p>

Provides education and advocacy about grief, loss and bereavement to both hospice staff and the community</li><li>
Develops and distributes educational materials about grief, loss, and bereavement.</li><li>
Demonstrates understanding of definitions and principles of bereavement, loss, and grief</li><li>
Address high-risk needs.</li><li>
Assures timely ongoing assessment of individual bereavement needs.</li><li>
Appropriately assesses bereavement needs and evaluates the effectiveness of interventions, adapting the Plan of Care accordingly.</li><li>
Demonstrates timely, accurate, and appropriate documentation of bereavement contacts.</li><li>
Participates in IDT-Interdisciplinary Team</li><li>
Participates in other hospice clinical, operational committees</li></ul><p></p><p><b>
Qualifications:</b></p><ul><li>
Masters Degree in counseling, social work, psychology or other professional with documented evidence of training and experience in dealing with grief required.</li><li>
3 yrs previous Hospital or Hospice experience required.</li></ul><p></p><p><b>
