Position Purpose

The Case Manager performs all the duties of the Social Service Assistant&rsquo;s and provides Case Management services for men, women, and families enrolled in the Transitional Housing Program and/or in the ASSETS program. Case management services include screening/intakes, goal planning, appropriate referrals, regular meetings with clients, facilitation of life skills and other groups, and implementation of policies and procedures for record keeping. 
Responsibilities and Duties:

Qualifications: Bachelor's Degree in HDFS, Psychology, Social Work, or a related field. Must have knowledge of issues concerning the homeless population and have experience working with low-income and homeless individuals and families. Knowledge around issues of Domestic Violence, Mental Health, and Alcohol and Drug Abuse are highly desirable. Must have excellent abilities to collaborate and maintain positive relationship with other providers. Good organizational and communication skills are a must.

Program Description
Brooklyn Community Services (BCS) operates family shelters providing safe, supportive, and dignified transitional housing for families with children experiencing homelessness. Our shelter programs are guided by a trauma-informed, strengths-based philosophy and offer a comprehensive continuum of services&mdash;including case management, housing placement, recreation, and supportive programming&mdash;to help families achieve stability and permanent housing as quickly as possible. All shelter staff are expected to uphold BCS's commitment to equity, cultural humility, and client-centered care.
Position Overview
The Social Worker provides clinical social work services to families residing in the shelter, offering psychosocial assessments, crisis intervention, mental health and domestic violence screening, and brief therapeutic support. Clinical services are provided under the LCSW supervision of the Director of Social Services, in accordance with NYS Education Law Article 154. Working in close collaboration with case managers and the Director of Social Services, the Social Worker addresses the clinical and behavioral health dimensions of family functioning and ensures families are connected to appropriate specialized services, while maintaining the confidentiality standards required under NYC Local Law 35 (2023).
Key Responsibilities
Clinical Assessment and Intervention
<ul><li>Conduct biopsychosocial assessments for newly admitted families and ongoing clinical assessments as needs emerge, under LCSW supervision.
</li><li>Screen families for domestic violence, mental health concerns, substance use, and trauma history; complete required screenings (e.g., Columbia Protocol, DV screening) in accordance with DHS standards.
</li><li>Provide crisis intervention and short-term clinical support to families in acute distress.
</li><li>Facilitate brief individual and family counseling sessions as clinically indicated, under the supervision of the Director of Social Services (LCSW).
</li><li>Inform residents that mental health services are voluntary and that certain disclosures may require mandatory reporting under Social Services Law Title 6 and Mental Hygiene Law &sect;9.46.
</li><li>Assess for child welfare concerns and make mandated reports in accordance with law; coordinate with ACS as required.
</li></ul>Confidentiality and Information Management (Local Law 35)
<ul><li>Maintain strict confidentiality of all information shared during the provision of mental health services; information from clinical sessions may not be disclosed to shelter staff, DHS, or contractors without the resident's express written consent.
</li><li>Apply mandatory reporting exceptions to confidentiality as required by Social Services Law Title 6 and Mental Hygiene Law &sect;9.46, and document such disclosures appropriately.
</li><li>Maintain clinical records separately from shelter case management records to protect resident privacy.
</li><li>Contribute to LL35 annual reporting data, including types of services provided and caseload information.
</li></ul>Care Coordination and Referrals
<ul><li>Develop clinical referral plans and connect families to mental health treatment, substance use services, domestic violence programs, and other specialized resources.
</li><li>Maintain collaborative relationships with community-based mental health providers, hospitals, and behavioral health systems.
</li><li>Consult with and provide clinical guidance to case managers on complex cases involving mental health, DV, trauma, or child welfare, within applicable confidentiality requirements.

<ul><li>Education: Master's degree in Social Work (MSW) required.

An LCSW in this role would satisfy an additional LL35 mental health professional unit for the shelter.

Neighborhood Services Organization

<div>SUMMARY: 

Provide services to formerly chronically homeless persons with a severe mental disability who are living at Palo Duro I &amp; II to increase participant stability and self-sufficiency, a high housing retention rate, and reducing participant use of shelters, hospital, and jail. Establish cooperative relationships with local shelters, hospitals, and law enforcement to identify and recruit perspective participants. Monitor the resident&rsquo;s progress and needs while living at PDI and II, link residents to and coordinate resources, conduct discharge planning, and track participants who have exited the program. Support the appropriate and timely documentation necessary for housing eligibility, maintain accurate progress notes and alerts. 
ESSENTIAL FUNCTIONS:

Case Management:
<div>
<ul><li>Serve as case manager to approximately 36 clients. Case management services include, but are not limited to screening, intake, assessment, service plan development, monitoring, linkage to appropriate community resources, follow up, appropriate discharge, crisis coordination, and tracking.</li></ul>
<ul><li>Meet with clients to assess their physical and mental wellness, barriers to living independently, and develop case plans upon establishment of their tenancy and revise them as needed.</li></ul>
<ul><li>Collect supplementary information needed to assist clients, such as employment records, medical reports, and other reports as deemed necessary to comply, and to determine the eligibility for resources available in the community.</li></ul>
<ul><li>Conduct wellness checks and schedule regular meetings with clients to assess the progress toward goals, address concerns, identify any new or recurring barriers to independent living and adjust case plans, as appropriate.</li></ul>
<ul><li>Complete appropriate documentation, log entries, case notes, and other written information regarding client progress and concerns in an accurate and timely manner.</li></ul>
<ul><li>Enter participant&rsquo;s information into the HMIS database in a timely manner and update as needed.</li></ul>
<ul><li>Maintain food/supply pantry utilizing the Weekly Market.</li></ul>
<ul><li>Comply with all safety policies, practices, and procedures. Monitor non-resident personnel in the building. Report all unsafe activities to supervisor.</li></ul>

<ul><li>Ensure compliance with OCHA polices, fair housing policies, the Landlord Tenant Act, and NSO requirements at all times.</li></ul>
<ul><li>Explain and enforce appropriate rules, policies, and regulations for all residents. Report any ongoing noncompliance issues to the supervisor, as well as document for the file.</li></ul>
<ul><li>Maintain the confidentiality and privacy of all clients with other clients and the external community. Consult with Director of Housing concerning program and individual issues as deemed necessary.</li></ul>

Property Management Duties:

Document Management/Certification Assistance:

Other Duties:

JOB REQUIREMENTS: 
Minimum of associates&rsquo; degree in social work, human services or related field Band two years&rsquo; experience in the field required. Additional experience in the field may be used as a substitute for educational requirements. Bachelors/Masters degree in relevant field may be considered in lieu of field specific experience.

Must have a valid driver&rsquo;s license and auto insurance within the limits defined in the employee handbook. 
Clearance of background check and drug screen. This position is considered safety-sensitive and therefore is not subject to the protections of the Oklahoma Medical Marijuana and Patient Protection Act. 
Must have reliable transportation for participation in rotating on-call schedule for after-hours issues in housing program.
