ESSENTIAL DUTIES AND RESPONSIBILITIES:
Director of Social Services provides Social Services for chronically mental ill homeless single women. Residence operates under contract to the New York City Department of Homeless Services (DHS) and as a contract agency, has reporting responsibilities to DHS and is subject to inspections and audits by DHS, the New York State Office of Temporary and Disability Assistance and the Coalition for the Homeless of New York City.
Essential Duties and Responsibilities:<ul><li>Provide ongoing supervision to the Social Services Staff.</li><li>Supervise the Case Management Services for all clients.</li><li>Provide coverage for cases if requested by the Director.</li><li>Respond to DHS inquires.</li><li>Provide overall coverage in the absence of the Director.</li><li>Meet all performance expectations established for the shelter by Care for the Homeless.</li><li>Ensure that the program meets the contractual obligations and reporting requirements of or the Department of Homeless Services. This family shelter is funded by New York City Department of Homeless Services and offers families a supportive, structured therapeutic, safe, and drug-free facility. It provides comprehensive case management services to 169 families who are in a transitional residence program for homeless families. The three story facility has rooms of various sizes, common areas and provides on-site security and social services.

What The Director of Social Services Does: 
<ul><li>Ensure that newly admitted clients receive social service intake with 48 hours of assignment to case manager.</li><li>Monitor and track client intakes and move-outs utilizing DHS rosters to update the Landing master roster daily.</li><li>Oversee all ACS cases and ensure that monthly tracking forms are completed and submitted to DHS.</li><li>Review and update ACS client rosters.</li><li>Meet with community agencies to assist with high risk case conferences as needed.</li><li>Maintain professional relationships with clients and client confidentiality.</li><li>Communicate with peers in other programs on issues related to client progress.</li><li>Understand all aspects of contract requirements and communicates this understanding to staff.</li><li>Supervise a minimum of four direct reporting staff and their subordinates to ensure that clients complete tasks (such as gaining entitlements, employment and housing options).</li><li>Work with staff to set and achieve realistic monthly performance targets in accordance with contract requirements (such as completing ILPs, inspections, monitoring families with children age 0-24 months, ACS, LTS families and housing placements).</li><li>Teach, coach and work with direct staff regarding how to interact with and/or engage clients to maintain entitlements to become eligible for housing subsidies.</li><li>Conduct weekly client progress reviews with staff.</li><li>Conduct periodic internal chart reviews and implement Quality Assurance measures as needed to ensure quality service delivery to clients.</li><li>Respond to critical incidences that require immediate attention.</li><li>Access client data using various tracking and program databases (i.e. &ndash; Client Tracking System (CTS), New York City Way, 4002 System).</li><li>Prepare contract reports and statistical information for both CAMBA management and funder use.</li><li>Assist in the analysis of program and demographic data to make programmatic improvements.</li><li>Organize onsite and offsite training opportunities for professional development of staff.</li><li>Assist the Vice-President in providing oversight for all aspects of the program in his/her absence.</li><li>Confer with the Vice-President, and/or other management staff as needed regarding programmatic and personnel issues.</li></ul>

JOB SUMMARY
Full-time Social Services Director needed for our skilled nursing facility in northern Illinois. A four year college degree in a Human Services field of study, social work, psychology, sociology, counseling, etc preferred. The Social Service Director must be able to present themselves well in group settings and individual 1:1 sessions. An excellent command of the English language for both verbal and written skills is required.
The ability to multi-task is very important. The Social Service Director will work a full-time schedule 8 hours daily with some weekends and some evening hours. The Social Service Director will participate in the Manager on Duty rotation. ESSENTIAL FUNCTIONS<ul><li> 
Be available to welcome new admission residents, provide support and initiate a trusting/therapeutic relationship.</li><li>
Must be able to clearly assess and document residents&rsquo; identified feelings about LTC placement (short/long-term), emotional, mental, environmental changes or physical limitations verbalized by resident (or family), which includes any observed non-verbal communications.</li><li>
Become knowledgeable of each individual resident&rsquo;s background, cultural, life history, disease, and medical care needs in detail to ensure an appropriate person-centered social service plan.</li><li>
Document interaction with and in-between resident and/or family in the assessment, care plan and/or social services notes in support of resident advocacy as required by the State standards of practice.</li><li>
Prepare a plan of care for treatment with the Interdisciplinary Team (IDT) based on the Comprehensive Assessment for each resident. Contribute as an integral</li><li>
member of the IDT on a continual basis and at the Resident Care Plan Conferences. This also involves documenting the social/emotional/mental needs related to the resident&rsquo;s illness/disability, adjustment to placement, cognitive, emotional/mental (mood), psychosocial functioning and the absence/presence of any behaviors (verbal/nonverbal) within the supportive network, and his/her response to the treatment/rehabilitation/need for placement according to each individual residents&rsquo; case. Based upon these, the SSD will make specific recommendations to assist in the resident&rsquo;s overall care and genuine well-being within the care plan for the best IDT approach.</li><li>
Initiate, facilitate, and/or participate in the written discharge plan which states the resident&rsquo;s specific need to be in the facility or if the resident is expected to be able to function in a more independent setting. The discharge plan should include consultation with other disciplines, the family and of course the resident.</li><li>
Make supportive visits to residents, writing progress notes at least quarterly, earlier when there is a notable change in condition.</li><li>
Provide good rapport through active listening with residents who express the need to talk and must be available to provide supportive counseling and behavioral intervention/programming as identified through oneself and/or community programming. Must assist the resident to participate in individual and/or group programs and to utilize these programs to full advantage when offered in order to empower residents for better quality of life.</li><li>
SSD must act as resident advocate, as well as a liaison between the resident and his/her family, the facility and community agencies.</li><li>
Maintain adequate record system for obtaining, recording, and filing of social service information. Participate in the development, maintenance, and implementation of the facility&rsquo;s Quality Improvement Programs.</li><li>
Participate in the facility safety committee, as assigned.</li><li>
Supervise, educate, counsel and discipline social service staff, as needed.</li><li>
Conduct, oversee, and complete initial and all on-going assessments and MDS, CAAs and care planning initiatives, including but not limited to social, medical, cognitive, physical, neuro-psychological, behavioral, communication abilities and spiritual needs (in conjunction with/at the lead of the Activity Department), amongst others as it relates to individual social service needs.</li><li>
Entering an initial assessment note in the social service section of the chart within 48 &ndash; 72 hours of admission, completing assigned sections of the MDS and CAAs, and completing the comprehensive set of social service assessments to be completed within 14 days of admission, re-evaluation of the resident on a quarterly basis (sooner when a significant change/behavioral incident occurs), and a comprehensive re-evaluation annually.</li><li>
Assessment, intervene, document and follow-up with all behavioral incidents, as necessary.</li><li>
Complete all required documentation.</li><li>

The qualified applicant for this position should possess:

<li>Degree in Social Work from a school or university accredited by the council on Social Work education - preferable MSW, but BSW encouraged to apply</li>
<li>Knowledge of the medical case management process, needs of geriatric patients, health care settings, working knowledge of funding resources such as Medicare Part A, B, C, ad D, Medicaid, Managed Care and other private insurances</li>
<li>Imagination and creative ability, skill in working cooperatively with other professionals, ability to organize and carry out responsibilities efficiently and effectively</li>
<li>Ability to communicate effectively and appropriately both verbally and in writing in a wide variety of circumstances</li>
<li>Long Term Care experienced preferred

<div>Good Shepherd Services seeks a results-driven, energetic, and compassionate leader to serve as the Director of Case Management/Social Services for its 17th Street Residential Programs. This role provides strategic and hands-on leadership within a youth-centered, family-focused framework, promoting safety, permanency, and well-being for female adolescents residing in a 35-bed, 24/7 hard-to-place group residence. 
 
The Director oversees all case management services within the child welfare system, continuously assesses program strengths and areas for growth, and ensures high-quality, compliant service delivery. This position collaborates closely with internal leadership, multidisciplinary teams, and external partners including ACS, OCFS, and the Family Court system, and partners closely with the Director of Behavioral Health and Director of Residential Life to advance program goals and Good Shepherd Services&rsquo; mission.
Reports To: Vice President, Youth &amp; Family Well-Being 
Location: 337 East 17th Street, New York, NY 10003 
Schedule: 35 hours per week / Exempt 
Salary: $110,000 - $125,000
<h1 class="jobSectionHeader">Major Duties &amp; Responsibilities:</h1>
<ul><li>Provide leadership to integrate, sustain, and continuously strengthen high-quality case management practices across the group residence.</li>
<li>Promote permanent connections for youth, including reunification, foster boarding home placement, adoption, and alternative permanent living arrangements.</li>
<li>Embed a Family Systems approach through supervision, treatment team structures, and program practices.</li>
<li>Ensure all city and state mandates and regulations are communicated, understood, and adhered to by the case planning team.</li>
<li>Oversee compliance with all mandated documentation and records related to ACS, OCFS, Family Court, permanency hearings, and Good Shepherd Services&rsquo; Impact &amp; Evaluation requirements.</li>
<li>Collaborate closely with Residential Life leadership and Behavioral Health teams to address systemic and individual casework needs.</li>
<li>Participate in internal and external committees, meetings, workgroups, and task forces related to youth advocacy and child welfare.</li>
<li>Provide direct supervision to the Assistant Director of Social Services and support the Case Planning team, including assessment of training needs.</li>
<li>Partner with the FTC facilitator to center youth and family voice and choice while maintaining focus on safety, permanency, and well-being goals.</li>
<li>Work with the Impact &amp; Evaluation Department on data integrity, documentation, outcome monitoring in Connections, and required reporting.</li>
<li>Collaborate with Good Shepherd Services support departments including IT, Training, Human Resources, and Government Contracts.</li>
<li>Assist with stabilization, crisis management, respite services, and provide on-call support as needed.</li>
<li>Provide emergency coverage when required, including home visits and court appearances.</li>
<li>Perform other duties as assigned.</li>
</ul>
Qualifications:
<ul><li>Education, Licensure, and Experience:</li>
<li>LMSW or equivalent required; LCSW preferred.</li>
<li>Minimum of five (5) years of supervisory experience with demonstrated leadership and managerial skills.</li>
<li>Seminar in Field Instruction (SIFI) preferred.</li>
<li>Experience working with adolescents and older youth in residential settings and with families.</li>
</ul>
Required Physical Demands and Work Environment:
<ul><li>Ability to work flexible hours, including possible on-call responsibilities.</li>
<li>Ability to travel throughout NYC using public transportation.</li>
</ul>
Knowledge, Skills, and Abilities:
<ul><li>Strong working knowledge of ACS child welfare systems, Family Court processes, and OCFS standards of practice.</li>
<li>Experience with Connections (CNNX), Legal Tracking System (LTS), Permanency Hearing Reports (PHR), Family Assessment and Service Plans (FASP), and Family Team Conferences (FTC).</li>
<li>Proficiency in Microsoft Office applications.</li>
<li>Excellent interpersonal, written, and verbal communication skills.</li>
<li>Strong organizational skills with the ability to manage multiple priorities and deadlines.</li>
<li>Collaborative leadership style and strong team orientation.</li>
<li>Commitment to trauma-informed, strength-based, youth and family development principles.</li></ul></div>
