ESSENTIAL DUTIES:</div>

<li>Assist individual to access and use available entitlement benefits.</li>
<li>Assist individual with completion of all reporting requirements for benefits.</li>
<li>Monitor individual's health and medical needs.</li>
<li>Identify home or community health and safety hazards and resources.</li>
<li>Advocate for individual whenever necessary.</li>
<li>Assist individual with development of personal goals and decision making.</li>
<li>Provide problem solving support in social and interpersonal situations.</li>
<li>Intervene in crisis situations, locate necessary resources, and coordinate services.</li>
<li>Document all services and make recommendations for additional support.</li>

Direct Support Professional (DSP) Part-Time

The salary range for this position is $48,000-$55,000 annually

Responsibilities

Assessment &amp; Observation
<ul>
<li>Assess individual&rsquo;s to determine the habilitation needs</li>
<li>Develop and supervise the implementation of the Staff Action Plan (SAP)/Residential Habilitation Plan (RHP) goals and the Individualized Protective Oversight Plan (IPOP)</li>
<li>Monitor the integration of services and make changes in the objectives of the SAP (RHP/IPOP)</li>
<li>Analyze the data based on the execution of the RHP to monitor the progress of individuals and write the monthly progress notes</li>
<li>Perform observations and spot checks to monitor the implementation of goals. Retrain staff and revise goals as needed based on observations and clinical recommendations</li>
</ul>
Training &amp; Communication
<ul><li>Attend Life Plan (Individualized Service Plan) meetings (Annual review and 6-month review), monthly staff meetings, and Interdisciplinary Team (IDT) meetings</li>
<li>Ensure the quality of staff in delivering and providing the necessary supports and supervision to each individual served by providing supervision, coaching, training, etc.</li>
<li>Maintain contact with the individual&rsquo;s family, Care Manager, Behavior Intervention Specialists, Day Program, treatment centers, and other QSAC programs to ensure continuity of services</li>
<li>Participate in the screening of potential residents</li>
</ul>
Qualifications &amp; Work Experience
<ul><li>Bachelor&rsquo;s Degree in one of the following professional categories (including, but not limited to) human services, sociology, special education, rehabilitation, counseling, psychology or related field required</li>
<li>One (1) year experience working directly with people with intellectual disability or individuals with physical and/or developmental disabilities and co-morbid psychiatric diagnoses
</li><li>Knowledge about the &ldquo;Level of Care Eligibility Determination&rdquo; (LCED)</li>
<li>Proficiency in Microsoft Office</li>
<li>Able to work a flexible schedule, including weekends</li>
<li>Possess a valid driver&rsquo;s license, to visit multiple work locations</li>
<li>General knowledge of theories and methods of applied behavior analysis (ABA) process, teaching strategies, assessments, and goal development</li>
<li>Ability to safely assist lifting individuals of various weights &amp; 20 lb item</li>
<li>Ability to run, when needed</li>
