Position: Case Manager
 
The Case Manager is a key provider of services within Volunteers of America’s Case Management Model. The Case Manager works with the client to assess their strengths and needs, plan a course of action, link the client to services, counsel the client and monitor progress. S/he must assure that clients receive appropriate, competent, and professional information and advice, referrals, and advocacy services to achieve successful outcomes. May include supervision of direct care staff.

Minimum Qualifications: 
<ul>
<li>Associate's degree or 60 college credits courses in Human Services field, or High School Diploma or GED with 2 years of relevant shelter experience.</li>
<li>The position also requires a demonstrated ability in verbal and written communication, including basic computer skills.</li>

Candidates with related experience with domestic violence preferred.
 
Case Manager Principal Responsibilities: 
1. Manage the client intake and assessment process to orient the client to the program and produce a comprehensive assessment of client service needs.
 2. Develop with the client a mutually agreed-upon individual service plan reflecting assessment findings goals. Update the plan with the client as required.
 3. Meet regularly with the client to assist him/her in obtaining needed services/benefits to achieve their individual service plan goals. Provide supportive counseling to strengthen his/her ability to make appropriate life decisions.
 4. Document case management activities in accordance with agency and funder guidelines and procedures. Collect service data and participate in evaluation and quality assurance activities.

The Housing Services Navigator/Case Manager is part of InterAct&rsquo;s Personal and Economic Empowerment team, providing case management, advocacy, and support services to adult survivors of domestic violence and/or sexual assault. Key Responsibilities 


Client Support Services

Conduct housing and service intakes for new participants, verifying eligibility and ensuring complete, accurate documentation.</li><li>

Documentation and Compliance

The Case Manager supports an average of 6-10 households fleeing domestic violence, sexual violence, stalking, or human trafficking. Victims entering Artemis House are in imminent danger and in need of crisis intervention and intensive safety planning assistance. Case Managers are responsible for providing direct services to clients in shelter by assessing client needs upon entry, connecting clients to appropriate services, and advocating to ensure the provision of comprehensive services meet the identified needs. Case Management services at Artemis House are provided in a client-centered manner driven by the victim&rsquo;s needs and goals and respecting client self-determination.

 Team-oriented with problem-solving and conflict resolution abilities.</li><li>
 Proficiency in Microsoft Office (Word, Excel, PowerPoint, Outlook).</li><li>
 Excellent written and verbal communication skills.</li></ul> 

<ul><li>
 Bachelor&rsquo;s or master&rsquo;s degree in social work, psychology, or a related human services field.</li><li>
 At least three (2) years of experience in supervision, direct services, crisis intervention, or advocacy, or five (5) years of equivalent experience.</li><li>
 Experience working with survivors of domestic and/or sexual violence preferred.</li><li>
 Successful background check and compliance with InterAct&rsquo;s Communicable Diseases Policy.</li><li>
 Valid driver&rsquo;s license, reliable transportation, and a clean driving record.</li></ul>

<div><div>Being a on-call Crisis Response Advocate provides crisis intervention, emotional support, danger assessment, safety planning, shelter placement, and information and referral to domestic and sexual violence survivors through hospital accompaniment, 24-hour hotlines, and chat.

On-call Schedule: One week per month</li><li>
On-call hours from 6 p.m. to 6 a.m., Monday through Friday, and 24-hour availability on Saturday and Sunday.</li><li>

The Requirements We are Seeking:
<ul><li>Bachelor&rsquo;s Degree in Social Science, Psychology, Counseling, Sociology, Social Work, Criminal Justice, or Public Health</li><li>
A minimum of one year of experience working with survivors of domestic and sexual violence, or social services setting working with vulnerable populations with clients from diverse backgrounds.
</li><li>Intermediate Proficiency within Microsoft Office, specifically Teams, Outlook, Excel, and Word.</li><li>
Bilingual in both English and Spanish, preferred.</li><li>
You have a valid Texas driver&rsquo;s license with a good driving record; reliable transportation; ability to travel to off-site locations utilizing personal transportation</li></ul>
What you Deliver in this role:
<ul><li>You will provide hospital accompaniment, crisis intervention, emotional support, case management, information &amp; referral, education, support, and advocacy to survivors of domestic and sexual violence at local hospitals and via nurse line as needed.</li><li>
You will provide crisis intervention, emotional support, case management, information &amp; referral, education, support, and advocacy to survivors of domestic and sexual violence via 24 crisis hotline and chat as needed while on standby for hospital visits.</li><li>
You will conduct danger assessment and creates safety plans with survivors in high-risk cases.</li><li>
You will communicate with on-call staff including Supervisors, Crisis Hotline/Chat Counselors, SANE nurses, law enforcement, and community partners.</li><li>
You will coordinate shelter placement and emergency transportation of clients as needed with Hotline and Shelter team.</li><li>
You will attend mandatory monthly meetings and training with Manager of Hospital Accompaniment &amp; Crisis Intervention Services, All Department Staff Meetings and/or meet as assigned.</li><li>
You will do mandatory Reporting: report suspected and prior abuse/neglect to either Child Protective services or Adult Protective Service when appropriate.</li><li>
You will maintain sufficient inventory at all times while on-shift of information packets and clothing for clients. Supplies provided by agency.</li><li>
Other duties/projects may be assigned. Should such duties become routine, the job description will be reviewed and revised.</li></ul>
