ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.</div><ul><li>
Understands and practices safekeeping of client protected health information per Bridgeways&rsquo; HIPAA policies and procedures.</li><li>

Creates an individual treatment/recovery plan with the individual, in collaboration with the referring person/agency and others identified as important as natural supports, as requested by the individual.</li><li>
Provides services consistent with the practice and principles of IPS, Individual Placement Services.</li><li>
Works closely with Division of Vocational Rehabilitation (DVR) and other local employment service systems. Under DVR contract, Bridgeways Employment Specialist/Case Manager facilitates Community Based Assessments (CBAs) as well as Job Placement and Job Retention services for DVR customers.</li><li>
Assists the individual in job readiness, job development, and job search activities directed toward jobs that are consistent with their needs, interests, and preferences. Completes, in collaboration with the individual, a vocational assessment, resume&rsquo;, internet job searches, interview skills, mock interviews and if requested, support during interviews at potential employers.</li><li>
Develops job opportunities with community employers and provides consultation to employers, assisting with coordination, communication, and education about working with employees with disabilities.</li><li>
Provides education and support to employers regarding working with individuals with disabilities. This may include information about behavioral health, Americans with Disability Act, how to provide accommodation, and how to research potential financial incentives for participating in the program.</li><li>
May include supporting individuals through applying, interviewing and retaining employment at Bridgeways as a Mission Focused employee, or in another community based, competitive job.</li><li>
Provides ongoing support, education, triage, and consultation to Bridgeways staff regarding an individual&rsquo;s employment goals, social service needs, and support with benefit planning.</li><li>

 The Case Manager provides comprehensive, trauma informed, and culturally responsive supportive services to families residing in the Innovative Urban Village supportive housing program. Case Managers conduct assessments, develop individualized service plans, provide counseling and crisis intervention, and coordinate referrals to promote long term housing stability, safety, and independence.
 
 
 This role requires strong engagement skills, the ability to support families impacted by domestic violence, and a commitment to evidence informed practices.  This program is provided to adults living in permanent subsidized housing to promote housing stability, wellness, independence, recovery, and community integration. Housing stability is measured by ongoing housing, decreased number of hospitalizations/ER visits/incarcerations, decreased frequency and duration of crisis episodes, and by increased and/or stable participation in maintenance of personal housing stability and wellness. The Housing Support staff will serve as the first point of contact for landlords/property owners for any issues arising with an individual in supportive housing, and will provide linkage to community, general entitlements, psychiatric/substance use/medical services, crisis prevention, and intervention services.
 
 
 
 Job Summary
 
 
 
 The Housing Support Case Manager is an integral part of the Housing Support Program, which provides multiple supports including case management and psychosocial rehabilitation to assist individuals living in permanent subsidized housing to promote ongoing housing stability. Under general supervision, Housing Support Case Managers must provide the following types of support:

 The Housing Support Case Manager will:

 Carry a caseload (25-30 clients) and conduct a minimum of 1 (one) visit per week/per client within current residence or community setting for the first three months after obtaining housing. After three months, individuals are required to be seen twice per month

 Provide care coordination to ensure stability of housing and independent living skills and facilitate referrals and linkages to internal/external community services/providers to assure a continuum of services

 Provide on-going support, case management, and skills training in accordance with the strengths, needs, abilities, and preferences identified within the treatment plan to help the individual achieve stated goals and objectives

Founded in 1980, Urban Resource Institute (URI) is the nation&rsquo;s largest provider of domestic violence shelter and support services and a leading provider of housing and services for families experiencing homelessness. Across 24 shelters in New York City, URI provides safe housing for nearly 4,000 individuals and their pets every night and serves more than 40,000 people annually.

URI&rsquo;s work is grounded in survivor-centered, trauma-informed, and culturally responsive care. In addition to emergency and transitional housing, URI delivers a robust continuum of Justice Initiatives&mdash;including economic empowerment, legal education and advocacy, prevention and youth programming, and abusive partner intervention&mdash;designed to break cycles of violence, instability, and poverty. URI also develops permanent affordable and supportive housing, advancing long-term pathways to safety, healing, and stability for families and communities.

Innovative Urban Village, opening in Summer 2026, is URI&rsquo;s first supportive housing program, developed in partnership with Christian Cultural Center. Located in East New York, Brooklyn, Phase 1A includes 376 income based rental residences, with 94 units dedicated to families who have experienced homelessness due to domestic violence and who qualify for ESSHI services. The program provides permanent supportive housing and trauma informed, culturally responsive services designed to promote long term housing stability and independent living. As URI&rsquo;s inaugural supportive housing initiative, Innovative Urban Village requires the development of a comprehensive program model, policies, procedures, and service framework from inception through full implementation.

POSITION OVERVIEW

The Case Manager provides comprehensive, trauma informed, and culturally responsive supportive services to families residing in the Innovative Urban Village supportive housing program. Case Managers conduct assessments, develop individualized service plans, provide counseling and crisis intervention, and coordinate referrals to promote long term housing stability, safety, and independence.

This role requires strong engagement skills, the ability to support families impacted by domestic violence, and a commitment to evidence informed practices. Case Managers collaborate closely with Housing Specialists, the Economic Empowerment Specialist, and the Director of Clinical Services to ensure integrated, high quality service delivery aligned with ESSHI and OTDA standards.

Salary: $60,000 - $70,000

MAJOR DUTIES AND RESPONSIBILITIES

Direct Service &amp; Case Management
<ul>
<li>Conduct comprehensive intakes, biopsychosocial assessments, and individualized service plans for assigned residents.</li>
<li>Provide supportive counseling, crisis intervention, and safety planning using trauma‑informed, survivor‑centered approaches.</li>
<li>Facilitate groups and workshops focused on parenting, wellness, DV recovery, tenancy skills, and life skills.</li>
<li>Conduct required home visits to assess safety, tenancy conditions, and service needs.</li>
<li>Provide advocacy and accompaniment for residents navigating benefits, legal systems, education, childcare, and healthcare.</li>
</ul>
Housing Stability &amp; Tenancy Support
<ul>
<li>Support residents in maintaining long‑term housing stability through proactive engagement and early identification of risk factors.</li>
<li>Educate residents on lease compliance, communication with property management, conflict resolution, and household management.</li>
<li>Collaborate with Housing Specialists and property management to address tenancy concerns and prevent eviction.</li>
<li>Assist residents with recertifications, inspections, and housing‑related documentation.</li>
</ul>
ESSHI, OTDA &amp; Supportive Housing Compliance
<ul>
<li>Ensure all services align with ESSHI service categories, including case management, employment support, benefits access, crisis intervention, and tenancy support.</li>
<li>Maintain accurate and timely documentation in CAPS, HMIS, and other required data systems.</li>
<li>Complete service plans, progress notes, assessments, and incident reports in accordance with agency and funder standards.</li>
<li>Participate in audits, case reviews, and quality assurance activities.</li>
</ul>
Crisis Intervention &amp; Risk Management
<ul>
<li>Respond to crises using de‑escalation, safety planning, and trauma‑informed strategies.</li>
<li>Conduct risk assessments related to DV lethality, mental health, child safety, and tenancy stability.</li>
<li>Coordinate emergency responses with internal leadership and external partners, including ACS, NYPD, mobile crisis teams, and hospitals.</li>
</ul>
Resource Navigation &amp; Community Linkages
<ul>
<li>Connect residents to community‑based services including mental health treatment, substance use programs, childcare, legal services, and medical care.</li>
<li>Provide benefits navigation for SNAP, Medicaid, SSI/SSD, childcare subsidies, and other entitlements.</li>
<li>Support residents in accessing education, vocational training, and employment programs in collaboration with the Economic Empowerment Specialist.</li>
</ul>
Interdisciplinary Collaboration
<ul>
<li>Participate in interdisciplinary case conferences and service coordination meetings.</li>
<li>Collaborate with the Director of Clinical Services to ensure clinical needs are addressed and referrals are completed.</li>
<li>Work closely with Housing Specialists to support move‑ins, housing retention, and tenancy success.</li>
<li>Maintain strong communication with operations and property management teams.</li>

Training &amp; Professional Development
<ul>
<li>Participate in ongoing training on trauma‑informed care, motivational interviewing, DV best practices, crisis intervention, and supportive housing standards.</li>

SPECIFICATIONS FOR EDUCATION/CERTIFICATIONS/LICENSES

Permanent Supportive Housing and Other Housing Opportunities
Specific Duties:

<li>Conduct interviews and assessments leading to program qualification.</li>

<li>Prepare reports and other paperwork per established program standards.</li>
<li>Participate in regular staff, case staffing, in-service, and other meetings.</li>

 Caseload Management:

<li>Oversee as a team with Housing Services Manager of Permanent Supportive Housing the delivery and evaluation for PSH programming to ensure all deliverables are met.</li>

<li>Act as a liaison when required to ensure that clients are properly referred to agency programs or external services in order to remove barriers to treatment and care.</li>

Service Planning and Documentation:

<li>Ensure all documentation is timely, accurate, legible and clear.</li>
<li>Empower clients to participate in their treatment planning as needed.</li>
<li>Maintain progress notes and progress reviews in client records as specified in agency policy, program guidelines and performance standards.</li>
<li>Input client information using electronic data entry according to agency and departmental guidelines.</li>
<li>Maintain an accurate record on time sheets reflecting time spent on program worked.</li>
<li>Prepare necessary program reports and records as requested by the supervisor and/or manager.</li>

 Manage Resources:

ABOUT THE ROLE

The YouthBuild Supportive Services Counselor plays a key role in the YouthBuild North Shore Program. We are seeking an individual to lead case management and counseling services for program participants. This individual will be responsible for overseeing intensive case management and offering guidance to support participants in their personal, legal, and social service needs.
