Position Summary:
The DV Client Advocate is responsible for individual services, including case management, advocacy, and crisis-counseling as well as presenting educational programs to the community. The DV Client Advocate reports directly to the Outreach Program Supervisor.

Duties and Responsibilities:
<ul><li>
Provide crisis-intervention and counseling to individuals and family members and significant others affected by sexual violence.
</li><li>Accompany individuals through procedures at area hospitals, police departments, prosecutor's office and court. Provide victims with case-related medical and legal information.</li><li>
Present educational programs in schools, community and church groups, and other professional organizations. Assist with additional outreach efforts and attend scheduled events.</li><li>
Provide case management assisting with resources, provide appropriate referrals, safety planning and completing monthly surveys.
</li><li>Coordinate and facilitate support group services for individuals affected by domestic violence.</li><li>
Respond to 24-hour hotline, accompany individuals through procedures at area hospitals, police departments, prosecutor's office and court. Provide individuals with case-related medical and legal information.</li><li>
Maintain individual case files, following state and agency guidelines.</li><li>
Complete monthly and quarterly statistics and reports.</li><li>
Participate in community collaboration meetings, SERV Prevention Coalition and monthly NJCDEV meetings.
</li><li>Meet the goals/outcome of grant requirements.</li><li>
Assist with office procedures pertinent to the operations of the program.</li></ul>

Requirements:
<ul><li>
The DV Client Advocate must successfully complete 60-hour New Jersey State Confidential Sexual Violence Advocate and Domestic Violence Response Team training provided by Services Empowering Rights of Victims (SERV).
</li><li>Advocates have experience working with people, especially on a one-to-one basis, and demonstrated sensitivity toward the needs of individuals affected by domestic violence. Effective verbal and written communication skills and public speaking is required.
</li><li>A Bachelor's Degree in Social Work, Psychology, Criminal Justice or Human Services is preferred. Work experience may substitute for education on a year-to-year basis.</li><li>
Current driver's license with a driving record of at least three years</li><li>
Vehicle with current insurance and registration</li></ul>

 RESPONSIBILITIES
The Case Manager I will perform duties including, but not limited to:

</li><li>Ensure case management assessments are completed within ORR&rsquo;s allotted timeframes.
</li><li>Conduct on-site admission and initial intake interviews of youth to include gathering familial, possible sponsorship information and to establish age of the youth.
</li><li>Conduct interviews of sponsors/family members, friends of family to vet sponsor&rsquo;s ability to care for the minor(s).
</li><li>Act as UC case POC for assigned Federal Field Specialist, Case Coordinator and Contract Field Specialist.
</li><li>Prepare case for, conduct and lead pre staffing process with multidisciplinary team of professionals including case managers, clinical counselors, medical coordinators and other designated stakeholders.
</li><li>Attend staffing(s) with Case Coordinators, Federal Field Specialists and other ORR stakeholders as needed.
</li><li>Coordinate with local pro bono attorneys for the timely provision of &ldquo;Know Your Rights&rdquo; presentations and legal screenings to children in care.
</li><li>Work with program administration, clinical, medical and educational staff in identifying best case management practices while maintaining a collaborative multi-disciplinary environment.
</li><li>Work to ensure children in care are provided a safe environment and safe and timely release from ORR care pursuant to ORR MAP Section 2; this may include completing online address searches, obtaining birth certificates to prove relationship, income verifications, background checks, and other actions to ensure proper vetting of the Sponsor, household members and adult caregivers.
</li><li>Document all actions taken and contacts with youth, sponsor, and stakeholders in the form of progress notes as required by NYAP.
</li><li>Complete and submit reunification packets and Release Requests for initial review to Lead Case Manager or Program designee.
</li><li>Submit completed reunification packet with appropriate referral made by Case Manager for the timely release of youth to designated sponsor, including referrals for home studies and post release services (PRS).
</li><li>Provide weekly face to face updates to youth and telephonic updates to family members/sponsor.
</li><li>Ensure the provision of two weekly telephonic contacts with family in the US or COO, primary caregiver and/or sponsor.
</li><li>Facilitate incoming calls to minors with the appropriate family members and other approved caregivers.
</li><li>Facilitate attorney to client contact as requested by youth.
</li><li>Coordinate weekly treatment team meetings with representatives from all departments at the Program.
</li><li>Establish and maintain a strong relationship with assigned foster parents, attend meetings with foster parents and act as program liaison with foster caregivers.
</li><li>Drive children to facilitate program services, may include transportation to court appointments, attorney visits, other appointments as needed per contractual duties; this may also include transporting youth within the United States for reunification purposes.
</li><li>Coordinate case management and family reunification services for children, including active involvement in discharge planning.
</li><li>Actively participate in documenting safety plans and Post 18 age out plans.

Firefly Children &amp; Family Alliance is looking for a full-time Home-based Case Manager in Indianapolis, Indiana. You will serve Marion county. Case Managers provide services to families in the families' homes on a routine basis to assist them in obtaining their goals. You will need to provide education and support to these families and transportation. You will support your clients by attending court, responding to crisis and stabilizing family functioning. This position is field based and will work out in the community. Hours are typically Monday through Friday, with evening hours required. Occasional weekend availability may be needed as well.

You Will:</h3>
<ul>
<li>(Family Progress - Monitoring and Coaching (50%)
<ul>
<li>Coach collaboratively &ndash; maintaining a balance between teaching and serving
</li>
<li>Visit each youth/family on a routine and 'pop in' schedule
</li>
<li>Assist youth/families in achieving their goals
</li>
<li>Provide education and support to youth/families
</li>
<li>Provide transportation to youth/families
</li>
<li>Attend court with clients and render recommendations
</li>
<li>Communicate with schools and multiple institutions related to youth/family matters
</li>
<li>Respond to all crisis calls and manage those situations
</li>
</ul>Service Plans / Goal Development (20%)
<ul>
<li>Create plans, including goals, for each youth/family
</li>
<li>Update plans and goals
</li>
<li>Develop relationships with agencies in the community
</li>
<li>Work with other social welfare agencies
</li>
</ul>Intake and Needs Assessment (15%)
<ul>
<li>Determine 'crisis' situations
</li>
<li>Visit each youth/family to gather necessary information to complete the assessment
</li>
<li>Follow Firefly Children &amp; Family Alliance professional standards
</li>
</ul>Report (15%)
<ul>
<li>Maintain case and agency paperwork according to standards
</li>
<li>Prepare written reports for the Department of Child Services / Probation office within identified timeframes
</li>
</ul></li></ul><h3 class="jobSectionHeader">You Have:</h3>
<ul><li>Bachelor's degree in social work or related field is required.</li>
<li>Must be at least 21 years of age</li>
<li>A valid driver's license and insurance with no more than 6 points on driving record</li>
<li>Travel between appointments to service clients within their community</li>
<li>Knowledgeable regarding social service systems within the state</li>
<li>Knowledge of and adherence to current confidentiality practices</li>
<li>Appreciation for and ability to respond to cultural differences</li>
<li>Develop relevant networks of contacts</li>
