Program Overview:
In partnership with Tennessee's Department of Children's Services (DCS), Youth Villages provides family-centered, strength-based services to youth from birth to age 18 through the Family Service Worker (FSW) program. Focused on delivering case management services, custodial families are referred by other DCS program areas, juvenile courts, or self-referral. Staff provide on-call crisis support, identify services and resources tailored to each family&rsquo;s unique needs, and utilize formal and informal assessments for case planning. Goals include increasing stability, strengthening relationships, building self-sufficiency, and enhancing well-being and permanency. 
Essential Duties and Responsibilities:
The Family Service Worker:<ul><li>
Carries a caseload of 15 to 20 cases</li><li>
Serves as a youth's guardian and must be aware of DCS policies that guide care and services</li><li>
Creates and monitors permanency plans so that adequate services and assistance are provided to youth and families</li><li>
Delivers services in the family's home or wherever they need us (school, court, etc.)</li><li>
Provides on-call crisis support to the youth and family (schedules vary by location)</li></ul><ul><li>
Ensures all medical, dental, and educational needs are met and accurately documented</li><li>
Attends 3 weekly meetings (individual, team, and clinical consultation) for professional development to help enhance clinical skills</li><li>
Completes accurate and timely documentation in an electronic medical record system (EMR)</li><li>
Performs other duties as assigned</li></ul>
Additional Information:
<ul><li>Schedule is flexible and non-traditional as it is based around the availability of youth and families served.</li><li>

JOB RESPONSIBILITIES:

<li>Maintain organization of family file. Keep family records up to date in accordance with CAPHR. Child Development procedures. Responsible for data entry and tracking of health, nutrition, and assessment data utilizing the Child Plus record keeping system; compile data for the Program Information Report (PIR).</li>
<li>Implement the health content to ensure that every enrolled child is up to date in their schedule of well child care according to NH EPSDT including all necessary follow up.</li>

Program Description Child First is an intensive, early childhood, two-generation, home visiting model that helps families build strong, nurturing relationships that heal and protect young children (prenatal through age five years) from the impact of trauma and chronic stress, with the goal of healing. Child First has been recognized as an evidence-based home visiting model by the U.S. Department of Health and Human Services (HHS).Summary of Position The Care Coordinator partners with a Mental Health Clinician to support families referred to Child First. The Care Coordinator works collaboratively with the family to connect the child and family with desired, community-based services and supports while maintaining Child First&rsquo;s reflective, relationship-based stance. This role enhances caregiver executive functioning skills and engages the caregiver-child unit in activities to strengthen the relationship. Through their work with families, the Care Coordinator/both decreases the &ldquo;toxic&rdquo; stress in the home environment and enhances opportunities for optimal child development, thereby promoting healthy brain development for infants and young children. The best candidate for this position is highly organized, self-motivated, reliable, and flexible with an openness to learning, capacity for self-reflection, eagerness to participate in reflective clinical supervision and desire to be part of a team.Key Job Responsibilities <ul><li>Delivery of home‑based services to families in the Alexandria and Lafayette regions, with up to twice‑weekly visits in each area, including the identification of and collaboration with community‑based service providers and supports.</li><li>Engage with the Child First family and the Clinician in the collaborative family assessment process (i.e., use data from interviews, observations, interactions, and standardized measures to identify family strengths, needs, and challenges).</li><li>Promote family stabilization by working collaboratively with family to identify and support needs integrating service of care, and addressing barriers to services as they arise.</li><li>Engage in weekly individual, team, and group reflective clinical supervision with Clinical Supervisor.</li><li>Engage actively in all aspects of the Child First Learning Collaborative, including in-person or live-remote training, distance learning curriculum, and specialty training.</li><li>Track completion of all assessments and enter in the appropriate database.</li><li>Keep all appropriate documentation for clinical accountability and reimbursement.</li><li>Maintain schedule and complete tasks to achieve home visiting Benchmarks and meet Accreditation standards.</li><li>Participate in other clinical and administrative activities as appropriate.</li></ul>Qualifications <ul><li>Applicants are required to possess a bachelor&rsquo;s degree.</li><li>Bachelor&rsquo;s degree in child development, psychology, nursing, human services, or related field preferred.</li><li>Experience working with culturally diverse families and young children under the age of six years preferred.</li><li>Knowledge of early childhood development, parent education, parent-child relationships, and individual, family, and community-level risk factors (e.g., poverty, homelessness, maternal depression, domestic violence, substance abuse, teen parenthood).</li><li>Knowledge of and experience with community-based services and support in service area</li><li>Able to communicate well verbally and in writing.</li><li>Comfortable with computers and experienced with Word and Excel.</li><li>Reliable vehicle and appropriate insurance for home visits are required</li></ul>Starting Salary: $45,500Job Type: Full-timePay: From $45,500.00 per yearBenefits:<ul><li>Paid time off</li></ul>Work Location: On the road
