<ul><li>Participates in daily Care Team Huddles to ensure collaborative care is given to each patient on every visit</li><li>
Performs review of patient's medical records for upcoming, scheduled clinic visit. Abstracts pertinent data related to activity within the healthcare system since last visit, quality indicators and the patient's personal clinical goals of therapy. Makes notes as necessary to serve as a reminder of issues that need to be addressed during the clinic visit.
</li><li>Greets and escorts patient to the exam room. Obtains and documents all pertinent vital signs and measurements. Assesses medication regimen in detail to evaluate each medication and dosage currently in the EMR and any newly reported medications or dosages. Notes any discrepancies to discuss with provider. Reconciles allergies.
</li><li>Engages patient in conversation regarding reason for visit and overall health. Assesses for social environment, gaps in care, quality measures and progression and/or barriers to reaching the goals of therapy.
</li><li>Prepares chart with any labs or other data for ease of access for provider, including reminders for quality indicators.
</li><li>Remains in the exam room, enters all findings, including physical exam and assessment provider notes. Enters provider orders and plan of care instructions, including prescriptions. Provides instructions to patient and educational materials based on the exam findings. Informs patient for timing of next visit and directs to check out for scheduling.
</li><li>Documents all activities/interventions, patient/family responses, medication dispensed/prescribed in the electronic medical record.

The Department of Pathology at Texas Children's Hospital (TCH) is seeking to expand its faculty in the Division of Anatomic Pathology with an M.D., D.O. or M.D./Ph.D. board-certified or board-eligible pediatric pathologist. Academic appointment will be at the level of Assistant Professor (tenure-track) in the Department of Pathology &amp; Immunology at Baylor College of Medicine. 
TCH is the nation's largest children's hospital and is ranked No. 3 in U.S. News and World Report's 2023 Honor Roll of Best Children's Hospitals, and in the top 5 in six of the ten U.S. News &amp; World Report-recognized pediatric subspecialties. The Texas Children's Hospital complex, located within the Texas Medical Center, the largest medical campus in the world, includes the original Abercrombie Building, West Tower, Legacy Tower, Mark A. Wallace Tower and the Feigin Tower. Within these buildings are a full range of primary to complex pediatric services, in addition to pediatric-focused research and administration. Adult services within the TCH complex include the Pavilion for Women, a stand-alone center for high-risk obstetrics, gynecology, fetal medicine and fetal surgery; and the newly opened Adult Congenital Heart program, located in Legacy Tower. The broader TCH facilities network is comprised of the West Campus, a community hospital in suburban West Houston; and The Woodlands, the first hospital devoted to children's care for communities north of Houston. Scheduled to open in 2024 is the Presidio Campus, located in Austin, which will extend TCH's reach beyond the greater Houston area. 
Expansion in the size and scope of Texas Children's Hospital necessitates additional support in pediatric surgical and autopsy pathology, placental/perinatal pathology, solid organ transplant pathology, and cytopathology services. Additional subspecialty services include neuropathology, hematopathology, electron microscopy and gynecologic pathology. The combined annual surgical case volume exceeds 16,000 specimens from pediatric and gynecologic surgeries and obstetric deliveries, with approximately 3,000 additional cytopathology cases and 120 pediatric and perinatal autopsies annually. Within the Department of Pathology at TCH are additional Divisions of Genomic Medicine, Laboratory Medicine, Informatics, Transfusion and Coagulation, Community Pathology, and Discovery. 
Pathologists at Texas Children's Hospital have primary full-time academic appointments in the Department of Pathology &amp; Immunology at Baylor College of Medicine and are expected to contribute to educational and scholarly activities in the Department of Pathology based at Texas Children's Hospital, as well as pathology resident and fellow education. <div>We are an Assisted Living Facility seeking a Med Tech to join our team! The Med Tech provides prescribed medical treatment and personal care services to residents and assists in providing a clean, safe, dignified, happy and healthy environment for residents by performing the following duties.

SCHEDULE: FULL TIME AND PART TIME WORKING FOR NOC SHIFT ONLY

Primary Med Tech Responsibilities:
<ul>
<li>Complies with the facility's attendance policy requiring regular, reliable, and punctual attendance</li>
<li>Assists with the self-administration of medication for residents as prescribed by the physician in accordance with established policies and procedures and applicable state/federal regulations</li>
<li>Observes documents and reports change of conditions to LVN (if applicable), Administrator, Physician, and responsible parties, including symptoms and conditions of residents that could also be related to medication interactions, adverse drug reactions or medication side effects.</li>
<li>Assessments are to be noted in resident file if there are any changes in condition</li>
<li>Creates resident files upon admission, verifying accuracy with physician report and medications</li>
<li>Stores medications in a safe and accurate manner in accordance with established policies and procedures and applicable state/federal regulations</li>
<li>Monitors medication to ensure adequate accountability measures are taken when medication are ordered, received into the facility, self-administered, and at change of shift required by policies and procedures and applicable state/federal regulations</li>
<li>Routinely inspects, clean and monitors equipment and supplies to ensure resident safety. Reports any equipment or supply issues to the LVN (if applicable), Administrator, or direct supervisor for correction or repair.</li>
<li>Maintains the overall med room operations for problems or discrepancies. Monitor supply levels, and creates and places orders to replenish department</li>
<li>Accompanies physicians who are visiting residents and accurately records data from that visit in a timely, accurate and factual manner</li>
<li>Communicates with doctors about prescriptions. Any medications orders given in writing are to be carried out and sent to pharmacy immediately. All changes in medication are also to be documented in M.A.R and &quot;Centrally Stored Medication&quot; binder upon receipt of medication. This includes medication handed in by family or SNF either by admission or routine family ordering</li>
<li>Meet and greet clients, family members and visitors. Handles all family and/or resident complaints regarding medication concerns</li>
<li>Completes incident reports within 24 hours of incident and assures reports are provided to LVN (if applicable) and administrator. Immediately notifies Administrator, physicians, and responsible parties</li>
<li>Reads 24-hour report book. Checks on any problems reported on previous shifts. If any resident is sent to the office, calls and checks on their progress</li>
<li>Maintains records neatly and orderly in designated filing areas</li>
<li>Other duties may be assigned</li>
</ul>
Med Tech Requirements:

PAY SCALE: $18 - $22 / hour
