Position Overview
<br><br>
Under the supervision of the Family Preservation Clinical Coordinator, the Case Manager provides comprehensive family assessments and delivers in-home, strengths-based services to families enrolled in the Family Preservation Program. This role partners closely with families and a multidisciplinary team to develop and implement individualized service plans that promote child safety, family stability, and overall well-being. All services are delivered in alignment with best practices and contractual requirements set forth by the Department of Children and Family Services (DCFS).

Duties
<br><br>
<ul>
<li>Conducts in-home visits to assess family needs, build rapport, and support clients in achieving established case plan goals.</li>
<li>Collaborates with multidisciplinary team members and clinical supervisors to develop, implement, and monitor individualized service and treatment plans.</li>
<li>Co-facilitates case conferences to review client progress, address barriers, and adjust interventions as needed.</li>
<li>Connects families to community-based resources and supports that enhance family functioning and strengthen parenting capacity.</li>
<li>Establishes and maintains respectful, culturally responsive relationships with children and families from diverse backgrounds.</li>
<li>Maintains accurate, up-to-date client records in compliance with agency policies and regulatory standards.</li>
<li>Completes all required documentation in a timely manner, including Family Assessment Forms (FAFs), weekly progress notes, monthly reports, and discharge summaries.</li>
<li>Ensures strict adherence to confidentiality standards in accordance with state laws and agency guidelines.</li>
<li>Participates in required staff meetings, trainings, supervision sessions, and professional development opportunities.</li>
<li>Engages in weekly individual and group supervision to support professional growth and service quality.</li>
<li>Fosters strong, collaborative relationships with internal staff and external partners.</li>
<li>Contributes to Quality Assurance (QA) and Continuous Quality Improvement (CQI) efforts to enhance program effectiveness and outcomes.</li>
</ul>
Knowledge, Skills, Abilities, and Other Characteristics

<li>Bachelor's degree in in social work, psychology, sociology, counseling or related field from an accredited college or university.</li>
<li>One year of case management experience, foundational knowledge of human development and family dynamics desired. Academic training may be substituted for a portion of the experience.</li>
<li>Knowledge of child abuse/crisis intervention techniques.</li>
<li>Strong written and oral communication skills in English and Spanish are required, and we celebrate linguistic diversity, valuing the unique perspectives and strengths that multilingual team members bring to our organization.</li>

Duties include assessment, evaluation, service planning, and provision, as well as coordination of all other identified services.</p><p>1. Participate in the assessment and evaluation of all program referrals for CLTS and gather necessary documentation.</p><p>2. Complete CLTS functional screens and re-evaluate the functional level of care and eligibility at regular 6-month reviews and as needed with a client needs change.</p><p>3. Work with children and their families to develop an Individualized Service Plan (ISP) to address identified needs and ensure that services address person-centered outcomes.</p><p>4. Attend school IEP meetings and participate in the discussion of service needs; provide advocacy as needed.</p><p>27. Support and assist with transitional service coordination, including transition to high school and adult services.</p><p>28. Assist in coordinating work-related and vocational services for children when appropriate.</p><p>29.
