About this Role: Responsible for the health, safety, family system's impact of permanency, and well-being of the children on caseload. Provide clinical and technical support to adoptive parents, and children through trauma-focused case management services, crisis management, home visits, weekly contact, monitoring of service compliance, documentation and ongoing training and advocacy. The main objective for this position is to effectively work towards maintaining permanency for children while actively assessing the needs of the child and their adoptive parent(s), and coordinating interventions with the therapist, and other members to the service team, as necessary
Responsibilities:<ul><li>
Provide Case Management for all assigned children and families. Maintains caseload of 10-12 children
 
and families.</li><li>
Conduct Face to Face interviews with each child and family individually at least one time per month.</li><li>
Maintain a working relationship with child, parents, teachers, therapist, and community providers to assist in collaborative efforts to preserve the placement of each child.</li><li>
Documentation of meeting contacts and case progress by completing paperwork, progress notes, incident reports and writing individual service plans in tandem with the therapist.</li><li>
Respect the dignity and worth of individuals.</li><li>
Maintain confidentiality per agency standards, HIPAA, state regulations, and NASW Code of Ethics.</li><li>
Consider the environmental, cultural, safety, diversity issues and needs for the child and family.</li><li>
Identify and maximize the strength, ability and competency of children and families.</li><li>
Participate in Program development as needed.</li><li>
Participate in supervision meetings with immediate supervisor.</li><li>
Participate in all FamilyCore assigned meetings.</li><li>
Maintains regular communication with clients through phone calls, emails, home visits</li><li>
Travel within seventeen county area using personal vehicle to provide services. Provide transportation for client appointments if needed.</li><li>
Track Case Management Outcomes and Outputs based on goals established in the Adoption Preservation(AP) Program Plan.</li><li>
Accept and implement direction, instruction, and correction from immediate supervisor.</li><li>
Working knowledge of de-escalation techniques and methods.</li><li>
Adhere to standards adopted through Council on Accreditation.</li><li>
Ability to organize and maintain data and confidential information.</li><li>

Requirements: 
Education and Experience 
<ul><li>
Bachelor's degree from an accredited school in the social services field.</li><li>
1-3 years of experience.</li></ul>
Competency: 
<ul><li>
Must pass required background check with Illinois Department of Children and Family Services (DCFS).</li><li>
Must pass background checks with Illinois Department of Public Health (IDPH) Health Care Worker Registry, and Illinois Department of Healthcare and Family Services OIG Provider Sanctions.</li><li>
Ability to set and maintain professional boundaries with clients.</li><li>

Must possess valid Illinois Driver's License in good standing and proof of valid vehicle insurance.

 Conducts program specific screening and evaluation, and formulates diagnosis in accordance with the Diagnostic Statistical Manual (DSM). Develops and documents, clear clinical formulations and treatment plans that are individualized and measurable, developed in collaboration with the client/family and others as relevant.
 Demonstrates knowledge of psychopathology and other behavioral health concerns and primary modalities of psychotherapeutic intervention. Triages clinical emergencies and implements program specific safety and consultation protocols. Utilizes best practices and evidenced based practice models where appropriate and adheres to model and program expectations.
 Demonstrates knowledge of internal and external levels of care, referral processes and resources. Makes clinically appropriate referrals for indentified service needs in a timely manner.
 Communicates in a positive, effective manner with client, family, and all internal and external providers. Attends any required meetings.

Qualifications<ul><li> 
Previous or current employment with the DHHS; working with residential youth or adults participating in programs related to domestic violence, abuse, or homelessness; working in a domestic violence or homeless shelter, or with any agency providing mental or behavioral health services; any public or private school in an educational or social work-related role; foster or kinship provider, contracted DHHS service provider, or other fields closely related to child welfare), or a combination of experience and education to equal four years.</li><li>
If work experience was not full-time, the equivalent must be calculated in comparison to full-time. For example, two years of half-time work would be equivalent to one year of full-time work.</li></ul>
Caseworker I: Pay $23.60:<ul><li>
Understands human development across the lifespan, and resources in the community, and has the ability to provide support and interventions in a culturally responsive way.</li><li>
Is adept at interacting professionally with all levels of management while managing a diversified workload and challenging situations.</li><li>
Deals with people in a manner which shows sensitivity, tact, and professionalism.</li><li>
Communicates effectively, clearly, and concisely verbally and in writing.</li><li>
Has four years of full-time education OR professional work experience related to working with children, parents, and/or families, OR a combination of education and professional work experience.</li><li>
Possesses a current valid driver's license.</li></ul>
Social Service Worker: Pay $26.21:<ul><li>
Holds an SSW or higher or higher licensure in good standing (required by statute UCA58-60-103).</li><li>
Understands human development across the lifespan, and resources in the community, and has the ability to provide support and interventions in a culturally responsive way.</li><li>
Is adept at interacting professionally with all levels of management while managing a diversified workload and challenging situations.</li><li>
Deals with people in a manner which shows sensitivity, tact, and professionalism.</li><li>
Communicates effectively, clearly, and concisely verbally and in writing.</li><li>
Has four years of professional work experience related to working with children, parents, and/or families or a combination of education and experience to equal four years.</li><li>
Possesses a current valid driver's license.</li></ul>

High Risk - Risks found in potentially dangerous or unusual environmental stress situations, e.g. working at great heights, working in extreme outdoor weather conditions, being subject to possible physical attack, etc.</li><li>
Some special risks which may require the use of special safety precautions e.g., entering clients homes, maybe working around contagious diseases, i.e. parasitic infections caused by head lice, bed bugs, scabies, etc. or chemicals, i.e. illegal substances, etc.</li></ul>
