<div>Who We Are: 
Mental Health Support Services (MHSS) is the provider of mental health, developmental disabilities, and substance use disorder services for children and adults in Chesterfield County. We are currently seeking a part time Case Manager (Part C Service Coordinator) with experience in service coordination activities and with young children with developmental disabilities to join our dynamic and supportive team! 
 
Minimum Qualifications: 
Bachelor's degree in a human services field, such as psychology, sociology, rehabilitation counseling, or a related field; two years of experience in a human services field; or an equivalent combination of training and experience. Employment experience in a Part C Local System and/or extensive knowledge of Part C regulations preferred; experience in service coordination activities with young children with developmental disabilities preferred. 
 
Required Knowledge, Skills, and Abilities: 
Working knowledge of case management methods, practices, and procedures. Ability to interview clients to identify deficiencies in personal, social, economic and health needs; to work in a multi-disciplinary team environment providing treatment and rehabilitation services; to communicate effectively orally and in writing; to operate a personal computer and related software and other standard office equipment; to develop and maintain effective working relationships with internal and external customers. Knowledge of Infant Mental Health preferred.

Current valid driver&rsquo;s license and good driving record required. JOB RESPONSIBILITIES:

<li>Act as a liaison and advocate between the community and the Child Development program by initiating and maintaining strong community partnerships. Keep current with local community resources and keep updated resource lists and materials. Attend appropriate community meetings as assigned.</li>

<li>Implement the health content to ensure that every enrolled child is up to date in their schedule of well child care according to NH EPSDT including all necessary follow up.</li>

<li>Develop a case plan with the participant based on the primary goals of housing stability, increased income/financial resources and skills, and self-determination within one week of intake which includes short-term and long-term goals with objectives.

Minimum Qualifications:
Education: Bachelor's degree in human service area: prefer BSW from an accredited college or university. Associates degree with extensive experience in strength-based case management may substitute for bachelor's degree as determined on a case-by-case basis.

Experience: Minimum of two years of case management experience in a comparable social service program that offer multi-faceted case management interventions designed to address the needs of those who are vulnerable utilizing a strength-based approach.

CHILDREN'S TRUST

POSITION SUMMARYThe Family Resource Specialist plays a key role in promoting the Healthy Families program and connecting families to vital community resources. This position focuses on community outreach, engagement, and recruitment of eligible families, while also supporting the Family Support Specialists and program operations.The Family Resource Specialist will build and maintain relationships with referral sources, community partners, and local agencies to strengthen the program&rsquo;s presence and ensure families receive comprehensive support.ESSENTIAL DUTIES AND RESPONSIBILITIESThe essential functions include, but are not limited to the following:<ul><li>Conduct ongoing outreach in the community to promote Healthy Families services and recruit new participants.</li><li>Establish and maintain partnerships with hospitals, health departments, schools, social service agencies, and community organizations.</li><li>Represent Healthy Families at community events, fairs, and resource meetings to increase program visibility.</li><li>Distribute educational and promotional materials in high-traffic community locations and through digital platforms.</li><li>Coordinate community presentations and information sessions to raise awareness about family support and child abuse prevention.</li><li>Receive referrals from hospitals, community partners, and self-referrals; complete eligibility screenings for new families.</li><li>Conduct initial home visits and assessments as needed to connect families to appropriate services.</li><li>Ensure smooth transitions of families to Family Support Specialists for ongoing home visitation.</li><li>Maintain accurate and timely documentation in the program database.</li><li>Assist with data entry, report preparation, and maintenance of outreach and referral logs.</li><li>Support the coordination of community baby showers, resource fairs, and other special events.</li><li>Collaborate with team members to identify gaps in services and develop outreach strategies to address community needs.</li><li>Familiarize oneself with the dynamics of child abuse and neglect. Immediately within the same working day, bring concerns related to potential child abuse and neglect to the Program Supervisor as required by the role of a mandated reporter.</li><li>Meets with Program Supervisor weekly for 1.5 - 2 hours to review family status. Develop skills in sharing interactions, perceptions and needs with Program Supervisor.</li><li>Documents plans made in conjunction with Program Supervisor. Develops strategies to follow through with those plans with active guidance from Program Supervisor.</li><li>Participates in weekly staff meetings and regular trainings, case conferences, in-service training and other meetings as required and in a professional manner.</li><li>Meet all training requirements (as listed in the PIMS Database system).</li><li>Record pertinent information related to family observations, actions, and health behaviors.
