Mental Health Support Services (MHSS) is the provider of mental health, developmental disabilities, and substance use disorder services for children and adults in Chesterfield County. We are currently seeking a part time Case Manager (Part C Service Coordinator) with experience in service coordination activities and with young children with developmental disabilities to join our dynamic and supportive team! 
 
Minimum Qualifications: 
Bachelor's degree in a human services field, such as psychology, sociology, rehabilitation counseling, or a related field; two years of experience in a human services field; or an equivalent combination of training and experience. Employment experience in a Part C Local System and/or extensive knowledge of Part C regulations preferred; experience in service coordination activities with young children with developmental disabilities preferred. 
 
Required Knowledge, Skills, and Abilities: 
Working knowledge of case management methods, practices, and procedures. Ability to interview clients to identify deficiencies in personal, social, economic and health needs; to work in a multi-disciplinary team environment providing treatment and rehabilitation services; to communicate effectively orally and in writing; to operate a personal computer and related software and other standard office equipment; to develop and maintain effective working relationships with internal and external customers. Knowledge of Infant Mental Health preferred.

Position Overview
 
Under the supervision of the Family Preservation Clinical Coordinator, the Case Manager provides comprehensive family assessments and delivers in-home, strengths-based services to families enrolled in the Family Preservation Program. This role partners closely with families and a multidisciplinary team to develop and implement individualized service plans that promote child safety, family stability, and overall well-being. All services are delivered in alignment with best practices and contractual requirements set forth by the Department of Children and Family Services (DCFS).
 
Duties
 
<ul>
<li>Conducts in-home visits to assess family needs, build rapport, and support clients in achieving established case plan goals.</li>
<li>Collaborates with multidisciplinary team members and clinical supervisors to develop, implement, and monitor individualized service and treatment plans.</li>
<li>Co-facilitates case conferences to review client progress, address barriers, and adjust interventions as needed.</li>
<li>Connects families to community-based resources and supports that enhance family functioning and strengthen parenting capacity.</li>
<li>Establishes and maintains respectful, culturally responsive relationships with children and families from diverse backgrounds.</li>
<li>Maintains accurate, up-to-date client records in compliance with agency policies and regulatory standards.</li>
<li>Completes all required documentation in a timely manner, including Family Assessment Forms (FAFs), weekly progress notes, monthly reports, and discharge summaries.</li>
<li>Ensures strict adherence to confidentiality standards in accordance with state laws and agency guidelines.</li>

Purpose &amp; Impact:

The purpose of the Child Welfare Case Manager is to provide full wrap around case management services to LSF clients and families while establishing a relationship with them and living the agency&rsquo;s core values.

Essential Functions:
<ul>
<li>Accept all cases assigned by unit supervisor and meet with children in their primary residence within 2 working days of ITR staffing and with identified parents in sufficient time to complete a Family Assessment within 15 working days of ITR staffing.</li>
<li>Negotiate and develop a case plan based on identified strengths and needs of the family, the circumstances bringing the family into care, recommendations provided by the Comprehensive Behavioral Health Assessment and other relevant data. This plan is to be created in collaboration with the parents, Guardian Ad Litem, foster parents and other pertinent parties with the goal of reunification or other permanency for the child.</li>
<li>Visits the child in their primary residence according to required frequency, but no less than once every 25 days. At least one visit each quarter is to be unannounced.</li>
<li>Accurately document all case activities in the Florida Safe Families Network database within 48 hours. Maintain the physical case file in chronological order, by subject, in accordance with the standardized case file format.</li>
<li>Submit service requests to the lead agency Utilization Management Department and provide clients with timely referrals to services. Develop and maintain knowledge of community resources, program eligibility requirements, key contact persons, emergency procedures, and waiting lists of available resources. Maintain regular contact with service providers and document service progress in FSFN.</li>
<li>Complete and submit court documentation within in required time frames. Prepare for, attend, and participate in all court activities as necessary.</li>
<li>Arrange for, attend, and participate in individual case staffing as necessary. Complete all required staffing and application packets.</li>
<li>Conduct initial and/or ongoing child safety assessments as required. Prepare initial and on-going safety plans as necessary. 
Arrange for emergency placement, emergency medical treatment, and emergency services for children at risk.</li>
<li>Conduct diligent searches for parents and family members when deemed necessary and thoroughly document that the effort has been made to find the parents and family members.</li>
<li>Conduct home studies as required for prospective placements. Provide relevant medical, psychological, behavioral and educational background information about the child or children to prospective caregivers as needed. Plan and facilitate parental and sibling visits as needed and appropriate.</li>
<li>Personal vehicle utilized to transport children as needed.</li>
<li>Ensure that all Independent Living functions are completed as required.</li>
<li>Attend all appointments, staff meetings, trainings, seminars, workshops, etc., as necessary and as required by the supervisor.</li>
<li>Function as agency on-call Case Manager as scheduled.</li>
<li>Works cooperatively with Program Directors, with other Case Managers, with placement staff, with Protective Investigators, with Child Welfare Legal staff and with agency support staff.</li>
<li>Effectively manages time to ensure that all home visits are completed as required, all documentation is entered into FSFN within 48 hours, court documentation is prepared according to specified time frames and court appearances are attended as necessary.</li>
<li>Follows Florida Statutes, Administrative Code, written policies and orders of the Dependency Court in managing cases toward goals recorded in case plans.</li>
<li>Organize, prioritize and complete all work assignments by the established deadlines.

The Case Manager is responsible for providing social services to children, youth and families that are consistent with the MCH Model of Care and ensuring a safe, caring and therapeutic environment for those served. This position requires the intellectual and emotional maturity to work with families who are experiencing difficulty in providing appropriate care for the children in their home and children who have behavioral issues, developmental delays and mental health needs. This position also requires respect and compassionate understanding for these children and their families, and the ability to form effective working relationships with children, parents and other caregivers. Case Managers must have a thorough understanding of and demonstrated competency in the Trust-Based Relational Intervention (TBRI) and other evidence-based programs offered in the assigned area. The Case Manager must maintain a healthy value system, including moral and ethical behavior consistent with the agency's mission and core values.
<h4 class="jobSectionHeader">DUTIES AND RESPONSIBILITIES</h4>

<ul><li>Identify and assess eligible participants according to program criteria and obtain consent for services for those accepted into the program, and make referrals to other community resources for those not accepted into the program.
</li><li>Develop and implement written plans of service based on the assessed needs of the child or family in accordance with program guidelines. Revise and update the written plans of service as needed to address the changing needs of those served and to comply with program guidelines.</li><li>

This is a professional position that may require more hours than the normal 40-hour workweek. This position involves providing services in the client's home and extensive work in the community. Responsibilities include work in the evenings and on weekends. This position requires the ability to serve in an on-call capacity and respond to emergencies after office hours. Out of town and overnight travel are also required. An automobile for in town and out of town travel is required. Mileage reimbursement is provided for use of personal automobile for approved business purposes. A technology stipend is provided to support the communication requirements of this position.

The Care Coordinator partners with a Mental Health and Developmental Family Therapist to support families referred to Child First. The Care Coordinator works collaboratively with the family to connect the child and family with desired, community-based services and supports while maintaining Child First&rsquo;s reflective, relationship-based stance. The Care Coordinator also enhances caregiver executive functioning skills (e.g., planning, organizing, managing time, focusing attention, regulating emotions, reflecting on progress) and engages the caregiver-child dyad in activities to strengthen the relationship. Through their work with families, the Care Coordinator both decreases the &ldquo;toxic&rdquo; stress in the home environment and enhances opportunities for optimal child development, thereby promoting healthy brain development for infants and young children. Program Description Child First is an intensive, early childhood, two-generation, home visiting model that helps families build strong, nurturing relationships that heal and protect young children (prenatal through age five years) from the impact of trauma and chronic stress, with the goal of healing. Child First has been recognized as an evidence-based home visiting model by the U.S. Department of Health and Human Services (HHS).Summary of Position The Care Coordinator partners with a Mental Health Clinician to support families referred to Child First. The Care Coordinator works collaboratively with the family to connect the child and family with desired, community-based services and supports while maintaining Child First&rsquo;s reflective, relationship-based stance. This role enhances caregiver executive functioning skills and engages the caregiver-child unit in activities to strengthen the relationship. Through their work with families, the Care Coordinator/both decreases the &ldquo;toxic&rdquo; stress in the home environment and enhances opportunities for optimal child development, thereby promoting healthy brain development for infants and young children. The best candidate for this position is highly organized, self-motivated, reliable, and flexible with an openness to learning, capacity for self-reflection, eagerness to participate in reflective clinical supervision and desire to be part of a team.Key Job Responsibilities <ul><li>Delivery of home‑based services to families in the Alexandria and Lafayette regions, with up to twice‑weekly visits in each area, including the identification of and collaboration with community‑based service providers and supports.</li><li>Engage with the Child First family and the Clinician in the collaborative family assessment process (i.e., use data from interviews, observations, interactions, and standardized measures to identify family strengths, needs, and challenges).</li><li>Promote family stabilization by working collaboratively with family to identify and support needs integrating service of care, and addressing barriers to services as they arise.</li><li>Engage in weekly individual, team, and group reflective clinical supervision with Clinical Supervisor.</li><li>Engage actively in all aspects of the Child First Learning Collaborative, including in-person or live-remote training, distance learning curriculum, and specialty training.</li><li>Track completion of all assessments and enter in the appropriate database.</li><li>Keep all appropriate documentation for clinical accountability and reimbursement.</li><li>Maintain schedule and complete tasks to achieve home visiting Benchmarks and meet Accreditation standards.</li><li>Participate in other clinical and administrative activities as appropriate.</li></ul>Qualifications <ul><li>Applicants are required to possess a bachelor&rsquo;s degree.</li><li>Bachelor&rsquo;s degree in child development, psychology, nursing, human services, or related field preferred.</li><li>Experience working with culturally diverse families and young children under the age of six years preferred.</li><li>Knowledge of early childhood development, parent education, parent-child relationships, and individual, family, and community-level risk factors (e.g., poverty, homelessness, maternal depression, domestic violence, substance abuse, teen parenthood).</li><li>Knowledge of and experience with community-based services and support in service area</li><li>Able to communicate well verbally and in writing.</li><li>Comfortable with computers and experienced with Word and Excel.</li><li>Reliable vehicle and appropriate insurance for home visits are required</li></ul>Starting Salary: $45,500Job Type: Full-timePay: From $45,500.00 per yearBenefits:<ul><li>Paid time off</li></ul>Work Location: On the road
