Responsible for the development of plans for family services and community engagement and facilitating the implementation of these plans.</li><li>
Responsible for the preparation of regular internal and periodic external reports on the progress being made toward meeting the requirements of the Performance Standards, goals and objectives established by Enrichment Services Program, Inc. and the submission of these reports as required to the Director.</li><li>
Responsible for participating in the writing of proposals for funds to support family services and community engagement.</li><li>
Responsible for providing support to service delivery teams as directed by the Head Start Director.</li><li>

As a Case Manager, you will be responsible for providing in-home support services for foster parents and children. This is for our Therapeutic Foster Care program. This position is in-office 5 days a week with travel for in-home services.

Work independently and create your own schedule based on your caseload and availability of your assigned families.</li><li>
Provide in-home, direct services to families and children in your caseload. Generally, caseloads consist of 15-18 clients that you&rsquo;ll see weekly.</li><li>
Create documentation for all services you provide in our Electronic Record System.</li><li>
Participate in rotating on-call schedule with the other team members. Generally, we're on call 1 week every 2 months.</li><li>

Travel to families&rsquo; homes in and around Somerset &ndash; mileage reimbursement is included. Overall hours being worked outside of the office is estimated to be around 60%, which includes traveling to and from visits and time spent at the home conducting a home visit.</li><li>
Set your schedule in collaboration with the team. Generally, we work standard hours of around 9 am &ndash; 6pm, however there are times when you may need to work until 8pm or later, based on the needs of families on your caseload.</li></ul>

The Adult Family Care Case Manager provides case management for AFC Participants and Caregivers including development and evaluation of AFC Plan of Care, goal setting, training, advocacy, and referrals. The Family Strengthening Case Manager is primarily responsible for assisting the program with providing intensive in-home case management. Family Strengthening&rsquo;s purpose is to maintain and strengthen family units, by providing in-home services designed to prevent children from entering the foster care system. This program works with family strengths, monitors the family dynamics, provides linkages to community resources, assists with housing or school issues, and provides information regarding child development and age-appropriate behavior and discipline as well as general case management services. ESSENTIAL JOB RESPONSIBILITIES<ul><li>
Conduct house visits to meet with families at various hours of the day as scheduled in some neighborhoods categorized as potentially high-risk;
</li><li>Serve as a resource of information between all parties involved in the case and identified stakeholders regarding medical, educational and mental health providers;

 General Functions
 
 
 
 Performs responsible social work in providing services to clients. Complies with the Head Start Performance Standards, the Head Start Act and other applicable regulations. Builds partnerships with families to assist them in identifying their own goals, strengths, needed services and support systems and in developing strategies and timetables for achieving their own goals.
 
 
 Work is performed under the supervision of an assigned Sr. Program Manager and is reviewed through observations, conferences, reports and results achieved.
 
 
 
 Representative Duties/Assignments

 Provides information and referral services for general assistance, counseling, medical assistance, and/or mental health services to Head Start children and families, according to Head Start guidelines.

 Implements and adheres to eligibility, recruitment, selection, enrollment and attendance (ERSEA) standard operating procedures and Orange County's policies and procedures.
 </li>
 <li>
 Conducts recruitment activities for program participants to include door to door recruitment, agency and community outreach, and distribution of materials as assigned.
 </li>
 <li>
 Completes eligibility and enrollment application to maintain a 100% full enrollment.
 </li>
 <li>
 Completes family needs assessment and develops a parent, family and community engagement Partnership Agreements with each Head Start family to accomplish identified goals. Monitors progress toward goals and recommends continuation or termination of services.
 </li>
 <li>
 Maintains regular contact with families by, making home visits, phone calls, center contacts and other investigations to ascertain the exact nature of home environment and/or progress in meeting goals and to supplement information gained by the personal interview.
 </li>
 <li>
 Provides information to parents for dental and physical screenings and follow-up appointments as indicated for Head Start children. Ensures required follow-up is completed in consultation with the Childhood Health and Development Services.
 </li>
 <li>
 Develops and coordinates prevention and community outreach services. Reviews and analyzes data regarding families on caseload to include Program Information Report, average daily attendance, goal achievement, and immunizations, medical and dental examinations.
