ESSENTIAL FUNCTIONS OF THE SAFETY SERVICES COMMUNITY WORKER:<ul><li>
Interview and evaluate clients to complete the intake process.</li><li>
Evaluate youth and families for treatment needs and refer to appropriate resources.</li><li>
Analyze client needs and strengths to determine services required control safety threats.</li><li>
Create, implement, and review safety assessments and plans.</li><li>
Exercise judgment to intervene as an advocate for clients in emergency and crisis situations.</li><li>
Provide parenting education, support, and advocacy to program clientele through individual, family, and group work.</li><li>
Conduct face-to-face contact with families to ensure safety and cooperation with safety plan.</li><li>
Act as a resource to evaluate and determine necessary responses in de-escalating crisis situations with youth and families.</li><li>

LOCATION: Kenosha, WI. Local travel is required to meet with clients at the home or other community spaces, with mileage reimbursement provided.
SCHEDULE: Full-time 40 hours per week. Monday-Friday 8:00am-5:00pm with one evening shift per week (1:00-9:00pm) and 1-2 Saturdays per month.

Key Responsibilities 
 DCFS Caseworkers and Social Service Workers strive to keep children with their own families whenever safely possible by assessing their and their families' needs, getting them connected to available resources, and conducting in-home services of appropriate intensity and duration. They work to reunify children who have been removed from their homes whenever possible or find enduring safety and permanency for them through alternative options. To accomplish this, caseworkers work closely with the subject matter experts, are provided with regular professional supervision, and complete DCFS Practice Model Training while performing the following tasks:<ul><li>
Investigating reports of abuse, exploitation, and/or neglect; assessing safety, risk, strengths, and barriers and determining appropriate actions governing a family's situation.</li><li>
Working with families to create plans for children&rsquo;s safety and for ongoing services.</li><li>
Coordinating and referring services to children and families offered by community resources, placement providers, collateral agencies, the courts, and community partners.</li><li>
Supervising visits between parents and children to ensure safety.</li><li>
Conducting in-home visits to assess safety and track progress.</li><li>
Facilitating Child and Family Team Meetings with the family, their formal and informal supports.</li><li>
Testifying in court hearings regarding actions, findings, procedures, and protocols.</li><li>
Preparing detailed case documentation that includes: intake records, investigation summaries, assessments of safety, risk and family functioning; service plans, court reports, case history and activity logs.</li><li>
May include performing 24-hour-on-call services as needed under supervisor's direction. 24-hour on-call service assignments may be based on, and subject to, geographic employment location and/or agency demand.

Qualifications<ul><li> 
Previous or current employment with the DHHS; working with residential youth or adults participating in programs related to domestic violence, abuse, or homelessness; working in a domestic violence or homeless shelter, or with any agency providing mental or behavioral health services; any public or private school in an educational or social work-related role; foster or kinship provider, contracted DHHS service provider, or other fields closely related to child welfare), or a combination of experience and education to equal four years.</li><li>
If work experience was not full-time, the equivalent must be calculated in comparison to full-time. For example, two years of half-time work would be equivalent to one year of full-time work.</li></ul>

Program Overview:
In partnership with Tennessee's Department of Children's Services (DCS), Youth Villages provides family-centered, strength-based services to youth from birth to age 18 through the Family Service Worker (FSW) program. Focused on delivering case management services, custodial families are referred by other DCS program areas, juvenile courts, or self-referral. Staff provide on-call crisis support, identify services and resources tailored to each family&rsquo;s unique needs, and utilize formal and informal assessments for case planning. Goals include increasing stability, strengthening relationships, building self-sufficiency, and enhancing well-being and permanency. 
Essential Duties and Responsibilities:
The Family Service Worker:<ul><li>
Carries a caseload of 15 to 20 cases</li><li>
Serves as a youth's guardian and must be aware of DCS policies that guide care and services</li><li>
Creates and monitors permanency plans so that adequate services and assistance are provided to youth and families</li><li>
Delivers services in the family's home or wherever they need us (school, court, etc.)</li><li>
Provides on-call crisis support to the youth and family (schedules vary by location)</li></ul><ul><li>
Ensures all medical, dental, and educational needs are met and accurately documented</li><li>
Attends 3 weekly meetings (individual, team, and clinical consultation) for professional development to help enhance clinical skills</li><li>
Completes accurate and timely documentation in an electronic medical record system (EMR)</li><li>
Performs other duties as assigned</li></ul>

Key Responsibilities</h3><ul><li>
Conduct patient assessments to identify social service needs.</li><li>
Develop and implement personalized care plans in collaboration with an interdisciplinary team.</li><li>
Provide emotional support and counseling to patients and families.</li><li>
Advocate for patient needs within the facility and coordinate with external resources.</li><li>
Maintain accurate and confidential patient records.</li><li>
Oversee discharge planning and ensure continuity of care post-discharge.</li></ul><h3 class="jobSectionHeader"> 
Qualifications</h3><ul><li>
Minimum 1 year of experience in Social Work or a related field.</li><li>
Experience in a long-term care or healthcare setting, particularly in social services.</li><li>
Strong understanding of case management and care coordination.</li><li>
Exceptional communication and interpersonal skills.</li><li>
Ability to work compassionately with a diverse patient population.</li></ul>

<div><div><div>Social Services Workers are qualified by education and experience to provide direct care to both patients and family members.
<ul><li>Minimum of a Masters of Social Work (MSW) degree or Masters in counseling, psychology, sociology, or psychiatric nursing</li>
<li>Licensed Clinical Social worker (LCSW) preferred</li>
<li>Must have experience in hospital, mental health, oncology or home health environments</li>
<li>Must possess good communications skills, both oral and written</li>
<li>Must be willing to participate in a multi-disciplinary team process of treatment planning and patient care evaluation</li>
<li>Must be willing to be cross trained and function in all areas of the hospitals social service program</li>
