The organization began in 1998 with three board members meeting over a cup of coffee. Over time Annie&rsquo;s Hope expanded to serve more than 3,600 kids and adults annually with 10 distinct programs. It now has 300-plus volunteers, 12 staff persons, and a budget of more than $1,000,000. Summary

The Healthy Families - Family Support Specialist (HF-FSS) optimizes the family home environment for the physical and mental well-being of children who are at risk for abuse/neglect. The HF-FSS works in the families' homes to improve the quality of parenting by providing support, connecting parents to available resources to strengthen the parent-child relationship by improving parenting and independent living skills. 


Essential Duties and Responsibilities include the following. Other duties may be assigned. 


CASE 
MANAGEMENT

Adheres to the Healthy Families America Critical Elements and Healthy Families Indiana, and Healthy Families LaPorte County policies and procedures; 


Establishes a Family Goal Plan (FGP) to use as a guide for service for each family that includes goals, objectives, and activities; assist family to meet goals; 


Teaches parenting skills and model nurturing behaviors during home visits using program approved curricula; 


Inputs computer data and generates reports in a consistent, accurate and timely manner. 


SUPPORT 
SERVICES

Establishes a trusting relationship with at-risk families by initiating and maintaining regular contact; 
Assists parents in scheduling and planning transportation to appointments for doctors, social service agencies, etc., act as a liaison between families and other community agencies as needed; 


Identifies community resources to reinforce independent living and parenting skills; educate the parents about the resources and facilitate the referral process; 


Maintains confidentiality of program participants. 


GROWTH &amp; DEVELOPMENT

Meets with supervisor on a weekly basis to evaluate client status and service to families; 


Attends regular Staff (Dunebrook Staff and Healthy Families Team) meetings, supervisions, in-service trainings, and other meetings/trainings as required; 


Participates in community education fairs and work-related group responsibilities. 


Education and/or Experience

Bachelor's degree in human services (social work, psychology, sociology, family counseling, early childhood, or nursing) from four-year college or university preferred; 


High school or equivalent diploma required; 


Experience in working with or providing services to children ages 0-5 and families. 


Infant mental health endorsement preferred; 


Knowledge, Skills, and Abilities

Outstanding interpersonal skills, resilient, patient, non-judgmental; 


Working knowledge of parent-child interaction and knowledge of infant and child development; 


Understand the dynamics of child abuse and neglect and can demonstrate the foundational skills necessary to apply this knowledge;

Physical Demands and Working Conditions

Viewing computer terminal, perceiving, and transcribing data with accuracy and keyboarding fill much of the day in an office environment; 


Communication with telephone callers and associates requires an ability to express oneself as well as perceive and exchange ideas; 


The employee is exposed to environmental conditions including noise levels that cause the worker to shout in order to be heard; home conditions may not be clean resulting in odors and insects; home conditions may be close quarters with inadequate seating requiring the employee to kneel, crouch or stand for long periods of time;- 


The employee may be required to ascend and descend stairs or ladders, balance on unstable surfaces while working in client homes. Work may include walking long distances, and exerting force up to 25 pounds to lift and move objects;

<div></div><div>Child Mental Health Professional (CMHP) - Individual with specialized training and one year of experience in the examination, evaluation, and treatment of minors and their families and who is a physician, psychologist, licensed or limited-licensed master&rsquo;s social worker, licensed or limited-licensed professional counselor, or registered nurse; or an individual with at least a bachelor&rsquo;s degree in a mental health-related field from an accredited school who is trained and has three years supervised experience in the examination, evaluation, and treatment of minors and their families; or an individual with at least a master&rsquo;s degree in a mental health-related field from an accredited school who is trained and has one year of experience in the examination, evaluation and treatment of minors and their families. For the BHT/ABA services individuals must be a BCBA or BCaBA or Psychologist working within their scope of practice with extensive knowledge and training on behavior analysis and BCBA certified by 9/30/2020.</div>
<div></div><div>Experience: One year working with the SED or IDD/DD children population and meet state requirements as a Qualified Mental Health Professional or Qualified Intellectual Disability Professional. Proficient computer skills including Office 365.</div>
<div></div><div>Other: Ability to interact with physicians and other healthcare providers in a professional manner. Working knowledge of physical and mental health, chronic health conditions, and behavioral health medications.</div>

<div></div><div>SUMMARY OF RESPONSIBILITIES:
</div><div>The Family Support Specialist (FSS) Assesses, plans, links, advocates, coordinates, and monitors to assist children or young adults still in school in gaining access to needed health and dental services, financial assistance and benefits, housing, education, social services, and other natural supports using a family driven, youth guided planning process.</div>
<div></div><div>ESSENTIAL JOB FUNCTIONS:
</div><ul><li>Assures that the person-centered planning process takes place and that it results in the individual plan of service, and meets direct and indirect service activity expectations</li>
<li>Assures that the plan of service identifies what services and supports will be provided, based upon medical necessity, who will provide them, and how the FSS will monitor (i.e., interval of face-to-face contacts) the services and supports identified under each goal and objective.</li>
<li>Oversees the implementation of the individual plan of service, including supporting the child and family&rsquo;s dreams, goals, and desires for optimizing independence, promoting recovery; and assisting in the development and maintenance of natural supports.</li>
<li>Assures the participation of the child and family on an ongoing basis in discussions of her plans, goals, and status.</li>
<li>Identifies and addresses gaps in service provision.</li>
<li>Coordinates the child&rsquo;s services and supports with all providers, making referrals, and advocating for the child and family.</li>
<li>Assisting the child and family to access programs that provide financial, medical, and other assistance such as Home Help and Transportation services and Medicaid Enrollment and Spend downs.</li>
<li>Assures coordination with the child&rsquo;s primary and other health care providers to assure continuity of care.</li>
<li>Coordinates and assists the child in crisis intervention and discharge planning, and community supports after hospitalization.</li>
<li>Facilitates the transition process (e.g., from inpatient to community services, school to work, dependent to independent living), including arrangements for follow up services.</li>
<li>Assists the child and family with crisis planning. Participates as member of Children&rsquo;s Crisis Mobile Team.</li>
<li>Identifies the process for afterhours contact.</li>
<li>Monitors and ensures that applicable laws, rules, policies, and procedures are followed.</li>
<li>Reports cases of alleged abuse or neglect to agency Recipient rights Officer and/or with Michigan Department of Health and Human Services Protective Services Workers or Licensing Consultant as prescribed by law.</li>
<li>Provides accurate and timely documentation for the clinical records and data systems, in accordance with Medicaid and agency requirements. Refer to attached documentation grid for NCCMH standards.</li>
<li>May assist in transporting and accompanying individuals and parents to medical appointments, evaluations, etc.</li>
<li>Participates in meetings and other related duties as directed by the supervisor.</li>
<li>Complies with agency requirements for continuing education, peer review, credentialing/privileging, and QIDP and/or QMHP requirements.</li>
<li>May assist, arrange, or provide in-service training programs for consumers, direct support workers, foster care providers, parent, guardians, and interested community members or civic organizations.</li>
<li>Performs other duties as assigned by supervisor.</li>
</ul><div>PHYSICAL REQUIREMENTS:</div>
<div>Ability to walk, bend, stand, sit, lift to 25 pounds with or without assistance, stretch/reach, hear, see, hand/finger dexterity and drive a vehicle. Position requires physical ability to complete and maintain certification in de-escalation techniques, which will be a training class provided to the job incumbent. <div></div><div>OTHER REQUIREMENTS:</div>
<ul><li>Must have own phone and internet for communication, reliable access to messages, and multi-factoring capabilities.</li>
<li>Must be able to complete and remain current in initial and ongoing training requirements.</li>
<li>Have reliable transportation and be able to drive a company vehicle. Must also possess a valid Michigan driver&rsquo;s license and have a satisfactory driving record.</li>
<li>Ability to work within guidelines of the code of conduct, regulatory compliance plan and personnel policies.</li>
<li>Supports a philosophy of service delivery that is recovery-based, person centered and culturally competent.</li>
<li>Skilled in planning, organizing, and prioritizing.</li>

<li>Excellent interpersonal, oral and written communication skills. Ability to facilitate a group.</li>
<li>Able to effectively present clinical information to psychiatry or to supervision.</li>
<li>Ability to negotiate conflict and maintain constructive working relationships with people at all levels of the organization and with community stakeholders, families, and clients.</li>
<li>Has knowledge of standards, policies and procedures regarding rights, confidentiality, Mental health code, HIPPA, code of ethics etc.</li>
<li>Demonstrates basic computer skills required for clinical documentation, communication and data entry.
</li><li>Ability to visit clients in their residence, educational or employment setting.</li>
<li>Irregular hours will be necessary periodically.</li>
