Responsible for conducting state approved assessments, identifying goals for recovery planning with adults, 18 and older with a mental health diagnosis</li><li>
Teaches/uses evidence-based practices/curriculum with individuals to help improve daily functioning, using a person-centered approach</li><li>
Services can include referral/linkage to community resources, psychosocial rehabilitation, skills training, substance abuse treatment, housing and/or employment skills training</li><li>
Organize, provide culturally competent care activities and collaboration among all participants involved with the individual&rsquo;s care</li><li>
Provide services via phone &amp; face-to-face, in both home and community-based settings</li><li>

The Case Manager position is perfect for a determined, experienced behavioral health technician (BHT), social worker or behavioral health professional looking for an exciting and rewarding career opportunity working with young adults and their families to be the primary point of contact in the coordination of comprehensive healthcare. This is not a supervisory position. Majority of time is spent in homes and the community rather than in an office setting. 
Case Manager Job Responsibilities:<ul><li>

The Case Manager II/Trainee- COPES provides clinical case management services to adults receiving long-term term services and supports through an Aging and Long-Term Care of Eastern Washington (ALTCEW) program. Evaluate client strengths and needs, complete a comprehensive assessment, determine level of functioning, develop and implement service plans that provide a reliable and safe plan of care that enables clients to achieve a maximum level of health and independence and targets case stabilization, ensuring independence. Complete administrative duties and meet required program deadlines. Coordinate with and perform advocacy with relevant parties and community agencies.
ESSENTIAL DUTIES AND RESPONSIBILITIES includes but is not limited to the following:<ul><li>
Adhere to program standards and guidelines and follow case management program protocols.</li><li>
Provide long-term care services responsive to the needs of elderly and adults with disabilities who are eligible for Title XIX Program services.
</li><li>Conduct face-to-face comprehensive assessments at the client's residence to determine level of care, including determining nursing facility level of care needs.</li><li>
Develop ongoing care plans which provide the support necessary to allow clients to live independently.</li><li>
Facilitate the coordination of in-home caregivers (Individual Providers and Agency Providers) for client care needs.</li><li>
Coordinate with ALTCEW and provide advocacy, coordination, consultation and support to clients, providers and stakeholders across multiple disciplines and systems.</li><li>
Perform program charting requirements, correspondence, and other paperwork accurately and in accordance to program deadlines.</li><li>
Authorize service payment through Provider One/Individual Provider One.</li><li>
Conduct interdisciplinary case reviews and consultations.</li><li>
Establish and maintain a good working relationship with FBH staff and community partners. Perform other related duties as required.</li><li>
Comply with all State WAC requirements and FBH policies and procedures.</li><li>
Requires travel to see clients in the community and travel between work sites.</li><li>

QUALIFIED CANDIDATES WILL HAVE:
REQUIRED:<ul><li>

Key Responsibilities:<ul><li>
Collaborate with the clinical team to develop individualized treatment plans that address the unique strengths, needs, abilities, and preferences of each client.</li><li>
Connect clients with essential community resources, including housing, social services, and substance abuse treatment, to support their overall well-being.</li><li>
Provide direct support to adults, children, and families, empowering them to achieve their treatment goals and improve their quality of life.</li><li>
Advocate for clients' rights and ensure they have access to the services and support they need to thrive.</li><li>
Monitor and assess clients' progress, providing ongoing feedback to the treatment team and making adjustments to the plan as needed.</li><li>
Assist clients in developing essential life skills, such as personal health management, self-advocacy, and independent living skills.</li><li>
Develop supports for clients who are employed or involved in employment programs and teach consumers how to maintain employment.</li></ul>
Education, Experience, and/or Credential Qualifications:<ul><li>
Bachelor's degree in social service, education, behavioral health, or other health-related field from an accredited college is required.</li><li>
Documented experience in the area of case management services is preferred.</li><li>
Must demonstrate current certification as a Certified Behavioral Health Case Manager (BHCM) II with the Oklahoma Department of Mental Health and Substance Abuse Services.</li></ul>
Additional Qualifications:<ul><li>
Current driver&rsquo;s license, acceptable driving record, and current auto insurance are required.</li></ul>
