Case Managers will work with clients in an office, community, school setting, and in the foster home. This position will add value to the company by equipping the child and foster parent (s) with the support and resources needed to achieve permanency.

The candidate selected will be responsible for the following:

Placement/Intake/Assessment
<ul>
<li>Participate in the referral matching process for children and families, while advocating for best possible home match.</li>

<li>Establish relationships of foster parent and foster child through training and support</li>
<li>Meet and/or exceed all state and licensure regulations regarding contact with the foster children on your caseload</li>

<li>Complete home visits as required by rules</li>

Crisis Prevention/Management

<li>Participate in weekly rotating &ldquo;on-call&rdquo; schedule to provide on-call advisory and support to children and foster parents,</li>
<li>Document and report any on-call activities and follow-up with team leaders</li>
<li>Report to supervisor or to designated authorities any instances of non-compliance with Necco, regulatory rules/policies, reports of abuse or suspected abuse, neglect, or suicidal ideations - immediately</li>
<li>Ensure all critical incidents are staffed with team leaders and entered into the Electronic Health Record within established timeframes.</li>

Data Entry

<li>Complete and Submit Medicaid documentation as appropriate</li>

Principal Duties and Responsibilities
 Case Management<ul><li>
Prepare reports and reviews in a timely, clear, and concise manner per Service Manual guidelines, including individual treatment plans or person-centered plans, contact logs, quarterly reviews, supervision notes, discharge summaries, Child and Family, and other assigned reports, utilizing computer-based clinical management software and other network software</li><li>
Review medical administration records (MARs) and grids at twice a month to ensure timeliness and accuracy.</li><li>

Participate in the planning and delivery of services regarding admission, treatment, and discharge. This includes coordination and facilitation of Child and Family Team Meetings as well as other services as needed.</li><li>
