 As a HEDIS nurse health coach, you will be responsible for engaging members, providing community resources, and referring to case management&mdash;all to achieve maximum health outcomes.  As an integral part of the HEDIS HOS Intervention Team within Clinical Quality and Compliance, you will coordinate and collaborate with teams and associates inside and outside of HMM, including Care Management, Member and Provider Service, and Performance Measurement and Improvement.
 
 
 
 Key Responsibilities

 Make outreach calls to members with gaps in care to assure adherence with HEDIS/STARs measures and contact provider offices to promote coordination of care as needed

 Identify member barriers to obtaining care and provide education, discuss clinical services/benefits, identify appropriate resources, and make necessary referrals both internally and externally, to mitigate barriers

 Ensure timely and clinically appropriate documentation in all required systems and meet daily productivity goals

 Participate in assistance with performance, measurement and improvement with activities such as medical record reviews and provider outreaches
 </li>

 Perform bone density screenings for appropriate Med Adv members, including encouraging the member to share results with their provider, documenting in BCBSMA system that screening took place, and following all requirement as outlined in policies/protocol

We are seeking a community-based Case Manager to join our team in Colmar, PA serving clients in Montgomery County.
The Case Manager is responsible for developing, coordinating, and implementing services for adult consumers as identified in the goal-oriented service plan. This role includes providing case management, service planning, and coordination of services with other agency providers to meet the consumer needs and increase community stability in consumer's life.

The Recovery Coach will:
<ul><li>Link consumers with a mental health or behavioral health diagnosis to resources and encourage, teach, and guide them to advocate for themselves.</li>
<li>Case Manager will complete comprehensive assessments, service plans, and environmental matrixes to determine level of care and frequency of contact with consumers on their caseload.</li>
<li>Complete other paperwork requirements from OMHSAS, Magellan Behavioral Health, Montgomery County, and Merakey and will be monitored by Case Manager and completed timely.</li>
<li>Have appointments with consumers in their homes, community, and hospitals and should be familiar with Montgomery County, including being able to access resources to meet client's needs.</li>
<li>Carry a caseload between 25-30 clients and Case Manager will meet with clients according to matrix score, either every 14 or 30 days while maintaining Merakey's productivity standard.</li>
<li>Attend supervision meeting with Manager monthly and team meetings weekly.</li>
<li>Work typically Monday-Friday: 9:00am - 5:00pm but shift may vary at times based on client's needs. This role includes optional on-call.</li>

This position is Community-based and requires travel to homes, and other community locations to provide services and support clients. Mileage reimbursement provided.
Earn $22.29 per hour.

An additional $2.00 per hour is available with the selection of our Enhanced Pay option!

Requirements:
The ideal candidate will possess the following qualifications:
<ul>
<li>Valid driver's license with clean motor vehicle report.
</li><li>Basic computer skills and experience with an EMR system preferred.</li>
<li>Case Management experience in mental health or behavioral health is preferred.
</li></ul>Educational Requirements:
<ul><li>Bachelor's degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education; or</li>
<li>Registered nurse; or</li>
<li>A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science and two years of experience in direct contact with mental health consumers; or</li>
<li>A high school diploma and five years of mental health direct care experience in public or private human services with employment as a case management staff person prior to April 1, 1989.</li></ul></div>
