Maintain complete, timely, and accurate documentation of all services within the electronic health record including but not limited to assessments, progress notes, treatment plans, screening tools, and discharge plans as required by SEARCH policy and procedure.</li><li>
Self-monitor and fulfill licensure requirements as required by clinician&rsquo;s licensing board.</li><li>
Adhere to patient confidentiality, sharing, and release of information per HIPAA and SEARHC policy.</li><li>
Coordinate care within the SEARCH health system and local community social agencies to assure appropriate referrals are offered for optimal individualized care.</li><li>
Attend, engage, and professionally participate in staff meetings, training, and multidisciplinary team meetings.</li><li>
Consistently meet productivity expectations to support ongoing patient access to timely care.</li><li>
Promptly respond to coding and audit requests and corrections in a respectful, professional, and timely fashion.</li><li>
Complete all required provider enrollment expectations and documentation.</li><li>

Education, Certifications, and Licenses Required<ul><li>
Master&rsquo;s Degree in Social Work, Psychology, Counseling, or related discipline from an accredited educational institution.</li><li>
Licensed as Clinical Social Worker (LCSW), Professional Counselor (LPC) or Marriage and Family Therapist (LMFT) preferred.<ul><li>
Once licensure is obtained it is a requirement to keep it current for the duration of employment.</li><li>

Grassroots Crisis Intervention

Position Summary:Grassroots has an opening for OMHC Clinician who will provide assessment and counseling to patients and their family members at the Grassroots Outpatient Mental Health Clinic/Substance Use Disorder Crisis Stabilization Center. The OMHC Clinician will conduct intakes and provide individual, family, and couples therapy; prepare treatment planning; and conduct group therapy. The OMHC Clinician will also complete SUD assessments and make recommendations for treatment. <div>The Behavioral Health Clinician is a Master&rsquo;s-level licensed or license-eligible therapist responsible for providing comprehensive mental health services, including assessment, diagnosis, treatment planning, and evidence-based therapeutic interventions. This role supports individuals experiencing a wide range of mental health conditions and may operate across multiple service lines, including outpatient therapy, crisis assessment and intervention, and specialized programming (e.g., Applied Behavior Analysis), depending on program assignment. The Clinician promotes recovery, symptom stabilization, and improved functioning through person-centered, trauma-informed, and culturally responsive care.

RESPONSIBILITIES
<ul>
<li>Provide individual, group, and family therapy utilizing evidence-based practices (e.g., CBT, DBT-informed approaches, trauma-informed care)</li>
<li>Conduct comprehensive biopsychosocial and psychiatric assessments, including suicide and risk evaluations</li>
<li>Formulate DSM-5 diagnoses and assess appropriate levels of care using standardized tools (e.g., LOCUS)</li>
<li>Develop, implement, and regularly update individualized, measurable treatment plans in collaboration with clients</li>
<li>Monitor client progress, adjust interventions, and support achievement of clinical goals and outcomes</li>
<li>Deliver crisis intervention, de-escalation, and stabilization services for individuals in acute distress</li>
<li>Coordinate care and facilitate referrals to appropriate levels of care, including outpatient, intensive outpatient, residential, or inpatient services</li>
<li>Provide psychoeducation to clients and families regarding mental health conditions, coping strategies, and recovery processes</li>
<li>Deliver Applied Behavior Analysis (ABA) services, including behavior assessments and intervention planning, when applicable to program assignment</li>
<li>Maintain timely, accurate, and compliant clinical documentation in accordance with Rule 132, Medicaid, and other regulatory standards</li>
<li>Ensure adherence to ethical standards, confidentiality requirements (HIPAA), and professional scope of practice</li>
<li>Collaborate with interdisciplinary teams, including psychiatrists, case managers, nurses, and external providers, to ensure coordinated and holistic care</li>

QUALIFICATIONS
