Summary
This position is part of a comprehensive career services program for Pre-Health students and alumni. The Pre-Health Career Coach provides career and academic advising, in conjunction with the Academic Advising office, to both degree-seeking and non-degree-seeking students. This includes support related to experiential education coaching, academic requirements and prerequisites for health programs, and preparation for pre-health graduate programs through individual coaching, classroom visits, and outreach. The role also includes employer relations and outreach to organizations in the healthcare field. Pre-Health coaching facilitates the university&rsquo;s academic mission by providing essential services in the health professions and maintaining relationships with campus academic and career advisors and faculty who serve the Pre-Health student population.
Essential Functions
The duties and responsibilities of the position include, but are not limited to:
Career Coaching to Pre-Health Students<ul><li>
Provide individual and small group career advising and coaching for students and alumni who express interest in pursuing health careers which include but are not limited to pre-nursing, pre-med, pre-physician assistant, pre-dental, pre-physical therapy, pre-occupational therapy, pre-pharmacy, pre-veterinary students.</li><li>
Advise current and prospective non-degree students interested in health professions.</li><li>
Work with students to understand academic requirements/pre-requisites for health programs at UCCS and for graduate degrees.</li><li>
Help students prepare for the application process, including practice interviews, and stay current with admission criteria, policies and procedures for graduate and professional school in health-related fields.</li><li>
Oversee and present a set of workshops on how best to prepare and submit applications to health professional fields. These may include topics such as an introduction to specific health fields, how to find clinical positions and internships,application preparation, interview strategies, MCAT preparation, etc.</li><li>
Plan, facilitate, and evaluate topically relevant programming and workshops (e.g., guest speakers, alumni panels, career-focused symposia) in collaboration with the Career Center, Academic Advising, Health Science Student Club, Student Nurses Association, and faculty in health-related fields.</li><li>
Distribute messaging/marketing about Pre-Health advising and programming opportunities.</li></ul>
Employer and Alumni Relations<ul><li>
Establish relationships with and engage alumni and friends of the University of Colorado Colorado Springs who work in the health professions; collaborate with Alumni Relations and Career Center on these efforts.</li><li>
Promote opportunities to engage with recruiters from health-related professions and professional schools.</li><li>
Collaborate across campus on cultivating opportunities for students to explore avenues for internships and volunteer experience in health-related fields.</li></ul>
Campus Partnerships<ul><li>
Establish relationships with and engage faculty and other campus community stakeholders in development of programming, resources, and advising support for Pre-Health students.</li><li>
Maintains relationships with campus advisors and faculty who serve the Pre-Health student population.</li><li>
Refer students to appropriate student support services, such as career advising, financial aid, disability services, tutoring, psychological counseling, the Office of Admissions, etc.</li></ul>
Misc.<ul><li>
Contribute to career center website and social media presence.</li><li>
Collaborate with career department on joint initiatives and provide support as needed</li><li>
Perform similar or related duties as assigned or required</li></ul>

Responsibilities

Summary:

The Care Ally, Case Manager is a key member of the interdisciplinary care team (ICT). They use a collaborative process of assessment, planning, implementing, coordinating, monitoring, and evaluating options and services required to meet the members health and social needs. They act as a liaison between our Members, their Responsible Parties and/or Power of Attorneys (RP/POAs), Advance Plan Provider/PCP, and key Align Senior Care stakeholders. The Care Ally, Case Manager reports to the Supervisor of Case Management.
Essential Duties &amp; Responsibilities:

Responsibilities<ul><li>
Executes on strategies and goals set by the Align Senior Care Board of Directors, the Senior Leadership Team, and Executive Director for managing and improving overall Member experience.</li><li>
Contacts Plan members to conduct a comprehensive health assessment of the individual, develop a plan of care, and participate in the facilities interdisciplinary care team meeting.</li><li>
Serves as health coach to educate the member, the family and/or caregiver, about disease status and treatment, plan benefits, community resources, and resource options</li><li>
Collaborates with members of the interdisciplinary care team and medical director(s) to facilitate appropriate treatment for members</li><li>
Routinely follows up with member as scheduled to assess progress towards goals</li><li>
Communicates with the member and/or caregiver to assist with the development of health goals and identify interventions to achieve these goals</li><li>

Provision BP monitors, assess proper measurement technique, and reinforce accurate self-monitoring practices</li><li>
Conduct comprehensive hypertension assessments, including review of medical history, medications, adherence patterns, and lifestyle habits</li><li>
Deliver intensive lifestyle and nutrition counseling focused on sodium and alcohol reduction, weight management through heart-healthy eating and portion control, increased physical activity, and smoking cessation support</li><li>
Identify barriers to healthy behaviors and coach members toward realistic, sustainable health goals</li><li>
Utilize motivational interviewing skills with the goal to influence positive sustained behavioral changes</li><li>
Maintain accurate and timely documentation of all member interactions</li><li>
Schedule follow-up appointments with yourself, program providers, or the member&rsquo;s primary care provider as needed</li><li>
Connect members with the right resources within Devoted to troubleshoot clinical or care-coordination issues</li><li>
