The Health Unit Coordinator, under the direction of the RN, demonstrates knowledge and understanding of the HUC role in the team delivery of patient care. Uses current technology to transcribe orders, maintain the integrity of the patient medical record, and gathers/enters and credits patient charges. Keeps the unit stocked.

The Patient Services Coordinator plays a crucial role in the delivery of healthcare services.
The incumbent serves as liaison for patient care activities in the practice and ensures quality patient care during patient visits.
The Patient Services Coordinator helps educate patients on their care plans and coordinate services with Primary Care Providers, and out-patient services diagnostic testing facilities. This is for the Suffern clinic location. 
Responsibilities<ul><li>
Works cohesively with the Primary Care Provider to facilitate patients care plan by participating in daily huddles/monthly case discussions, facilitating appointments and coordinating information and continuing care requirements.</li><li>
Collaborates with other clinical staff, to resolve issues impacting patients care progression.</li><li>
Monitors and updates dashboard with reminders and alerts, to organize daily tasks such as: visit status reminders, follow-up appointments, authorizations for medical review, coordination of specialty appointments and resolving billing errors (when applicable).</li><li>
Contact insurance carriers for patients pre-certification; consults with Department/Practice Manager with respect to patients needs or addressing escalated issues.</li><li>
Provides front-end coverage as needed; records telephone encounters for routing to appropriate Providers.</li><li>
Works as liaison with Physicians, patients and clinical staff to connect patients with relevant internal/external resources.</li><li>

Compensation

The First Steps SPOE Service Coordinator works with children ages birth to three with developmental disabilities and their families. This role ensures timely completion, coordination, and implementation of all service planning activities within the Missouri First Steps Early Intervention Program. Responsibilities include managing assigned caseloads, maintaining compliance within the Missouri WebSPOE system, and serving as a key liaison between families and service providers.

ESSENTIAL FUNCTIONS AND PERFORMANCE AREAS 
Service Coordination &amp; Case Management
<ul><li>
Maintain an assigned caseload of children and families.</li><li>
Complete and document the entire Intake process&mdash;scheduling, intake visit, eligibility determination, evaluations/assessments, Family Assessment (as applicable), and Initial IFSP&mdash;within the 45 day timeline.</li><li>
Ensure all new First Steps services begin within 30 days of parental consent and that services are delivered as written in the IFSP.</li><li>
Maintain consistent communication with families and document all contacts in the child&rsquo;s electronic record.</li><li>
Hold and finalize IFSP meetings within DESE timelines.</li><li>
Complete authorizations for evaluations, assessments, meetings, and services accurately and timely.</li><li>
Monitor corresponding authorizations and conduct consistent follow up to prevent billing errors</li><li>
Complete all required EXIT activities per DESE guidance, including exit summaries per CARF requirements, at least 30 days prior to inactivation</li></ul>

JOB SUMMARY
 HealthRIGHT 360, a nonprofit organization and a family of programs, is committed to providing accessible
 and comprehensive healthcare services to vulnerable populations. Our mission is to tackle systemic

Prototypes CHS-SUD Substance Treatment and Reentry Transition (START) provides direct in-custody SUD
 counseling, group therapy, and case management services to up to 150 women located at the Century
 Regional Detention Center (Lynwood County Jail). START services are evidence-based and culturally
 appropriate that utilize cognitive behavioral therapies and motivational interviewing approaches to treat
 addiction.
 
The Care Coordinator II is an integral part of an interdisciplinary team that supports our clients in their
 treatment and recovery by coordinating services, providing clinical assistance, and ensuring accurate
 documentation. The Care Coordinator II will develop individualized treatment plans and will help our
 clients meet treatment goals, manage crisis, assist with public benefits, and facilitate access to educational
 and community resources. Additionally, this role will advocate for our clients and ensure that they have
 the resources needed for successful reentry and long-term recovery.
