PURPOSE STATEMENT: Under the direction of the Assigned Administrator, the Health Services Manager - ECE manages, supervises and coordinates the nutrition and health services, including environmental health and safety checks for the Head Start/Early Head Start and State programs; provides training and technical assistance to internal and community partners; and implements, monitors, and maintains services within established guidelines and standards.  Job Summary: The Health Services Member Care Manager is responsible for coordinating and managing the care of members enrolled in SECUR Health Plan. This role involves developing individualized care plans, conducting follow-up member visits, and ensuring members receive appropriate and timely care. The Member Care Manager will work closely with an interdisciplinary team, including the member&rsquo;s Primary Care Provider, to provide comprehensive care and support to members of SECUR Health Plan. The role is member-facing, and the candidate will spend 90-95% of their work time in the field, directly interacting with members to provide care and support. In this role, the Health Services Member Care Manager must assess and make appropriate recommendations regarding the types of care and services that the member requires to improve their health and quality of life.  Health Risk Assessment (HRA): Completes initial and annual Health Risk Assessment (HRA) meeting all regulatory timeline requirements. Conducts comprehensive evaluation of potential health risks identifying exposure factors, analyzing data, and developing mitigation strategies to minimize health hazards in conjunction with the interdisciplinary team. This requires the RN to utilize their clinical expertise to identify subtle health risks and make informed decisions about the necessary interventions.

 Care Plan Development: Develops and updates Individualized Care Plans (ICP) based on HRA results and changes in members' medical needs. Incorporates the member's stated goals and desires to ensure that the care plan is personalized and aligns with their preferences and aspirations. The RN must use critical thinking skills to assess the member's condition, anticipate potential complications, and adjust the care plan accordingly.

 Member Follow-Up Visits: Conducts follow-up visits for members who have recently experienced post-acute discharges, falls, or have been assigned a risk score based on their most recent HRA. Conducts comprehensive assessments that include evaluating the member's clinical history, current medications, physical examination findings, psychosocial needs, functional status, and social determinants of health. The RN must make complex decisions regarding the member's care, considering multiple factors and potential outcomes.

 Care Coordination: Coordinates follow-up appointments, specialist visits, and other outpatient services for members. Partners with other members of the Health Services Department to stay knowledgeable of the latest member information. The RN must use their clinical judgment to prioritize care needs and ensure timely access to necessary services.

 Member Engagement: Conducts face-to-face visits with members to discuss care plans, assess progress towards goals, provide education, and ensure adherence to treatment. The RN must use their communication skills to effectively educate members and motivate them to adhere to their care plans.

 Audits and Inspections: Assists in periodic audits and inspections to identify potential member non-compliance issues in facilities and promptly reports to leadership and members of the Interdisciplinary Care Team (ICT) for intervention. The RN must use their attention to detail to identify compliance issues and take appropriate action.

 Care Management and Chronic Condition Management Programs: Identifies, suggests, and collaborates with the interdisciplinary care team (ICT) on appropriate care management and chronic condition management programs that might benefit the member offered through SECUR Health Plan. The RN must use critical thinking and assessment skills to piece together data from various sources, aligning recommendations with the member's needs and ensuring they are enrolled in the most suitable care management program. This involves analyzing the member's health status, identifying potential gaps in care, and recommending programs that will enhance their overall health outcomes.

 Collaboration with Primary Care Physician: Collaborates with the Primary Care Physician on HCC gap closures, essential charting components, and the overall clinical picture of the member. The RN must use high-level problem-solving skills to understand and address HCC codes, ensuring that all gaps in care are closed and the member's clinical picture is accurately documented.

This position is primarily an independent, complex administrative, consultative, and technical position, providing managerial direction and operations coordination to Area 1 HIV/AIDS Program (Escambia, Okaloosa, Santa Rosa, and Fort Walton Counties). The position performs a wide range of duties under the supervision of the Public Health Services Manager &ndash; SES. This position spends the majority of the time engaged in supervisory responsibilities for assigned program activities in a state and local Department of Health concerning Human Immunodeficiency Virus (HIV) / Acquired Immune Deficiency Syndrome (AIDS) Surveillance, HIV/AIDS Contract Management, HIV/AIDS Prevention, Early Intervention, and AIDS Drug Assistance (ADAP) programs. General Program Development and Management:

Act as the liaison with the Florida Department of Health (FDOH) HIV/AIDS Section, the local regional Departments of Health, and the community on HIV/AIDS issues. Educate the community on FDOH and local Departments of Health goals, objectives, and available programs and services. Participate in statewide, regional, and county HIV/AIDS program planning, development, and evaluation, as needed. Initiates and directs the development of policy material. Responsible for providing technical assistance and monitoring of regional program services. Plans workloads, workflows, deadlines, work objectives and time utilization with employees. Maintain positive communications with the Administration and Directors of Nursing in the regional counties.

Maintain knowledge of HIV/AIDS policies, guidelines, statutes, rules, and protocols. Coordinate with staff and community partners for outreach events. Ensure reports are submitted to the HIV Section of the DOH Central Office on time. Participate in monthly HAPC conference calls. State abreast of current legislation, activities trends and funding sources relative to HIV care. Ensure that all relevant HIV/AIDS, contracts, MOAs, and MOUs (responsibility of EIC) are appropriately managed and monitored by working with the EIC and contract manager to assure that they are meeting their HIV contract management responsibilities. Plan, develop, and coordinate an Area 1 HIV/AIDS education program for professionals, the community, and for providers. Educates the community on available programs and services (i.e., presentations and material).

Prevention:

Utilize HIV/AIDS staff to identify and develop a plan for service gaps in the region. Coordinate special events, i.e., World AIDS Day activities, National HIV/AIDS Testing Day. Supervise the Early Intervention Consultant (EIC). Assist the EIC in assuring HIV trainees are meeting Technical Assistance Guideline requirements. Ensure that the local counseling and testing programs are managed and evaluated correctly (direct responsibility of the EIC). Promotes or assists in promoting the development of community resources to address program needs. Ensure local outreach activities are occurring in the target populations identified in the local prevention plan. Ensure timely, complete, and accurate reporting by coordinating with surveillance staff.

Surveillance: Ensure the surveillance staff are providing education on reporting requirements and the importance of reporting to providers. Ensure that reporting is done in a timely manner and reports are shared with Area 1 CHD Administrators and other programs within the Health Department.

Patient Care: Ensure the planning and coordination of HIV/AIDS patient care,i.e patient's linkage to care and ADAP activites. Oversee the duites and tasks of all linkage activites and staff.Coordinates with public and private agencies to promote integrated service delivery. Provide technical assistance to community patient care HIV/AIDS programs throughout the region. Participate in area consortium meetings and planning activities. Ensure ADAP database is properly maintained. Ensure that ADAP staff review client files for the required documentation needed for determining client eligibility.

Position Summary/Mission
Responsible for the oversight of healthcare management staff including the organization and development of high performing teams. Works closely with functional area managers to ensure consistency in health related and psycho-social interventions supporting our members and is accountable for meeting the financial, operational and quality objectives of the unit.
Fundamental Components &amp; Physical Requirements
Oversee the implementation of healthcare management services for assigned functional area. Accountable for maintaining policies &amp; procedures in accordance with applicable regulatory and accreditation standards and implementing them at the employee level (e.g. NCQA, URAC, state and federal standards and mandates as applicable). Ensure implementation and monitoring of best practice approaches and innovations to better address the member's needs across the continuum of care. May act as a liaison with other key business areas. Manage resources responsible for identification of members, development and implementation of care plans, enhancement of medical appropriateness and quality of care and monitoring, evaluating and documenting of care. May have responsibility for their own case load work. May act as a single point of contact for the customer and the Account Team including: participation in customer meetings, implementation and oversight of customer cultural requirements, and support implementation of new customers. Assess developmental needs and collaborates with others to identify and implement action plans that support the development of high performing teams. Establish an environment and work style that promotes the concept of teamwork, cross product integration, and continuum of care thinking that results in strong performance. Consistently demonstrate the ability to serve as a model change agent and lead change efforts. Collaborate with licensed staff from other areas that assist with oversight of clinical staff and activities of licensed personnel. Serve as a content model expert and mentor to the team regarding practice standards, quality of interventions, problem resolution and critical thinking.
Requirements:
3 or more years of experience in a physical or behavioral healthcare setting 
1 or more years management experience 
3 or more years of experience in managed care 
Knowledge of the regulations, standards, and policies which relate to medical management
Preferred:
Certified Case Manager (CCM)
Bilingual - English and Spanish
Education:
Bachelor&rsquo;s degree in health or human services (e.g. Social Work, 
Psychology, Nursing, Mental Health, Counseling, Behavioral Sciences, Sociology, Gerontology or related field required.

The typical pay range for this role is:

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.

We are an agency committed to quality gender-responsive, trauma-informed care to individuals experiencing serious mental illness, development disabilities, and co-occurring disorders. Metrocare programs focus on the issues that matter most in the lives of the children, families and adults we serve. The QMHP-Adult Intensive position provides psychosocial/rehabilitative service to adults with serious mental illness. Services may include, but are not limited to, assessment, development of measurable treatment goals and objectives, engagement into treatment, referral, linkage, advocacy, monitoring, crisis intervention and continuity of care. The overall goal of this position is to maximize the individuals potential level of functioning, reduce hospitalization and aid in the successful reintegration of individuals into the community. 
 
 
 
ESSENTIAL DUTIES AND RESPONSIBILITIES:

</li><li>Provide overall service coordination, psychosocial rehabilitation, emergency services/assessment, referral, transportation, linkage, and advocacy to individuals with varying needs. Perform duties in concert with other members of an interdisciplinary treatment team.

</li><li>Formulate individual recovery plan based on assessment findings on all new admissions.
</li><li>Develop measurable objectives and goals agreed upon by the individual.
</li><li>Actively assist in obtaining and modifying goals as needed.
</li><li>Document all attempts to involve individual, service providers, and caregivers (unless opposed by the individual) in service plan process.
</li><li>Review service plans every 180 days or as clinically indicated.
</li><li>Participate in interdisciplinary team staffing. Present psychosocial assessment findings. Update team on progress or lack of progress in reaching agreed upon goals. Provide relevant information that might affect course of treatment.
</li><li>Follow-up with assigned individuals who miss a scheduled clinic appointment. Document attempts to contact individual. Report repeated unsuccessful efforts to contact the treatment team.
</li><li>Oversees and assists assigned individuals medication adherence. Provides med training, arranges transportation to scheduled office based appointments, and prompts needed refills.
</li><li>Document clinical services by close of next business day.
</li><li>Perform follow up hospital assessments the same day as requested by the hospital and report assessment outcome to the Clinical Manager
</li><li>Provides crisis intervention/on-call services during the work week and by weekend rotation. Make home and hospital visits, initiates mental illness warrants, work with police and other public servants as a needed to address crisis for assigned clients
</li><li>Facilitate inpatient admission upon request of the treatment team and/or hospital. Provide continuity of services throughout inpatient stay. Participate actively in discharge planning with hospital. Meet with the individual within 2 days of discharge in the community.
</li><li>Evaluate progress of clinical session, solicit feedback from individual(s), and consult with colleagues and team leaders when dealing with unfamiliar/uncomfortable issues.
</li><li>Co-facilitate family education workshops.
</li><li>Identify and assist individuals in obtaining entitlements by providing referrals, advocacy and negotiation, as needed.
