Position Title: Nurse Practitioner 
Reports To: Medical Director

Job Summary: The Nurse Practitioner (NP) will play a crucial role in delivering comprehensive healthcare services in both telehealth and in-person environments. You will manage chronic and acute conditions, oversee patient monitoring through RPM, and collaborate with interdisciplinary teams to provide patient-centered care. This role also supports CCM and CHI initiatives, ensuring ongoing care for chronic disease management and improving community health outcomes. 
Key Responsibilities:

<li>Patient Assessment: Conduct comprehensive assessments of patients' health conditions, including physical examinations, medical histories, and diagnostic test results. 
</li>
</ul><ul><li>Diagnosis and Treatment: Diagnose and treat common acute and chronic illnesses, injuries, infections, and other medical conditions. Develop treatment plans and prescribe medications as needed. 
</li>
</ul><ul><li>Preventive Care: Provide preventive care and health maintenance services, including vaccinations, health screenings, and lifestyle counseling. 
</li>
</ul><ul><li>Health Education: Educate patients and their families about health conditions, treatment options, medications, and self-care techniques. 
</li>
</ul><ul><li>Care Coordination: Collaborate with the Medical Director, Operations Manager, Care Navigators, and Medical Assistants to streamline patient care. 
</li>
</ul><ul><li>Telehealth: Offer seamless virtual healthcare consultations, referrals, and follow-up care through our telehealth platform. 
</li>
</ul><ul><li>Record Keeping: Maintain accurate and up-to-date patient records and documentation in compliance with healthcare regulations and standards. 
</li>
</ul><ul><li>Prescription Management: Manage and monitor patients' medication regimens, including adjusting dosages and renewing prescriptions as needed. 
</li>
</ul><ul><li>Compliance: Ensure all documentation and patient care follow federal and state regulations, including HIPAA and telehealth guidelines. 
</li>
</ul><ul><li>Community Health: Support CHI initiatives aimed at reducing healthcare disparities in underserved communities. 
</li>
<li>Follow-ups: Ensure that patients receive appropriate follow-up care, either personally or through coordinated care teams.</li></ul> 


Requirements: 
<ul>
<li>Education: Hold a Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) degree from an accredited nurse practitioner program. 
</li>
</ul><ul><li>Licensure: Possess a valid and unrestricted nurse practitioner license in the State of New York, 
</li>
</ul><ul><li>Certification: Be certified as a nurse practitioner by a recognized certifying body, such as the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP). 
</li>
<li>Clinical Skills: Demonstrate proficiency in conducting physical assessments, diagnosing medical conditions, and developing treatment plans. 
</li>
<li>Communication Skills: Have excellent interpersonal and communication skills to effectively interact with patients, families, and healthcare team members.</li>
<li>Bilingual in Spanish is a huge plus.</li></ul>
