<div>The Transporter/Dialysis Assistant provides safe, timely and courteous transport to and from the nursing home to the dialysis center. Assist Dialysis staff where necessary in the overall running of the unit and to maintain a clean and sanitary environment.

The Hours are 5:30 AM- 3:00 M/W/F (27 hours per week)

Performs in accordance with the established rules, regulations and operating standards of the facility. Abides by all Federal, State and local regulations and laws relating to the operation of an ESRD facility as well as ensuring patient confidentiality.

Specific Duties:

Transporting

1. Transports patients from the nursing home to the dialysis center

2. Assures the chair being used for transport is clean and good working order

3. Verifies patient scheduled treatment time and picks up patients timely

4. Brings along patients inter-facility communication book and assures entry from floor

5. Places patient in available station as directed by clinical staff

6. When notified by clinical staff of completion of treatment, return patient to floor nursing home

7. Assist dialysis staff to transfer patients safely to chairs.

8. Returns patient to floor safely with their communications book after entry made by dialysis staff

STOCKING
<ul><li>Daily stocking the unit, checking inventory, ordering, deliveries, store incoming deliveries, stocking items in an organized first in first out form</li></ul>
<ul><li>EOM inventory to be completed by the end of the business day of the month</li></ul>

Housekeeping -Provides specific cleaning duties as required by Facility using equipment and disinfectant selected by facility

1. Clean Floors by dry/wet mopping/ vacuuming and polishing

2. Clean /Disinfect bathroom, toilets, sinks, mirrors, walls

3. Refill soap, paper towels and toilet paper

4. Clean offices (Dust sweep mop) and waiting areas

5. Clean Dialysis machines, chair and surrounding Area

6. Periodically wash chairs, and biohazard wire racks

7. Perform High dusting including vents and louvres

8. Spot clean walls, garbage bins, fire extinguishers

9. Collecting and disposing of garbage and replacing clean liners

10. Transport garbage to compactor and biohazard materials and sharps container to biohazard storage room

11.Keep the unit&rsquo;s Janitors closet clean and organize at all times</div>

Summary: Responsible for the safe, timely and accurate transport of patients, medical equipment and patient-related items to their destinations.
Essential Duties and Responsibilities:<ul><li>
Properly and safely transports patients, secures IVs, drainage tubes, etc., and secures patient.</li><li>
In accordance with the transportation of patients, uses wheelchairs and other approved rolling stock to carry out the essential duties of the position.</li><li>
Demonstrates competency in transporting all specific age groups.</li><li>
Ensures the safe, timely and accurate transport of patients to their destination using the proper requested equipment.</li><li>
Communicates effectively with nursing personnel, clinicians, technicians or reception and dispatch.</li><li>
Ensures the comfort of the patient by being attentive. Offers blankets, checks modesty and only engages in appropriate conversation involving patients. Never transports patients in severe pain.</li><li>
Alerts nursing staff immediately to any signs of patient distress or equipment malfunction.</li><li>
Maintains equipment and reports equipment needing repairs.</li><li>
Complies with regulatory agency standards, including federal, state and JCAHO.</li><li>

Responsibilities:

A day in the life of a Patient Transport Aide at Hackensack Meridian Health includes:</div><ul><li>
Transports patients by the use of wheelchairs and with stretcher/bed transports, ensuring safe handling of patients to prevent falls.</li><li>
Transports patients to and from appropriate units or Imaging Modalities within expected time frames, and also ensuring the patient's chart is brought along.</li><li>
Maintains regular and open communication with Dispatch/Modality/Unit to notify of receipt of task, changes or delays in transport, and completion of tasks. Open communication is important to avoid non-productive time.</li><li>
Ensure that patient support apparatus such as IV pumps and oxygen tanks are handled safely during transport. Responsible for removing equipment from service that is unsafe or unclean. Responsible for oxygen tank exchange.</li><li>
Where applicable, obtains supplies from central supply as requested.</li><li>
Transporter follows established isolation protocol in use of personal protective equipment and hand washing techniques.</li><li>
Transporter uses two patient identifiers to correctly identify patient.</li><li>
Proper protocol followed for patient expirations.</li><li>
Where applicable, transports specimens to the laboratory, and delivers blood and blood products in a timely manner. Proper protocol followed for specimens/blood work.
</li><li>Where applicable, picks up slides for physicians to review from laboratory as requested.
</li><li>Participates/uses the automated systems in a timely and accurate manner to assist with patient throughput.</li><li>
Lifts a minimum of 10 lbs., pushes and pulls a minimum of 50 lbs. and stands a minimum of length of shift.
</li><li>Identifies the needs of the patient population served and modifies and delivers care that is specific to those needs (i.e., age, culture, language, hearing and/or visually impaired, etc.). This process includes communicating with the patient, parent, and/or primary caregiver(s) at their level (developmental/age, educational, literacy, etc.).
</li><li>Assists with environmental duties in departments or units including pick up and disposal of recycling/trash receptacles; also linen removal from laundry bins to proper disposal area such as chutes for collection as requested. Adhere to cleaning requirements and procedures as assigned
