Job Summary:

Core Responsibilities

Facilitate advance care planning, medical decision-making and educate patients on the Death with Dignity Law.</li><li>
Provide information to patients and families related to symptom management and psychosocial response to the diagnosis of cancer and related treatment.</li></ul>
Weekend: Participation in weekend urgent care clinic in Westbrook 3-4 times per year
