As an Elara Caring Clinical Team Manager, you&rsquo;ll contribute to our success in the following ways:<ul><li>
Provides oversight of all patient care services and personnel including: making patient and personnel assignments, coordinating patient care, coordinating referrals, assuring patient needs are continually assessed, and assuring the development, implementation, and updates to the individualized patient plan of care.</li><li>
Plans, directs, and participates in delivery of home health services to provide diagnosis-specific patient care, including coordination of services with providers, vendors, or facilities.</li><li>
Ensures the timely start of patient care within recommended regulatory guidelines.</li><li>
Processes and coordinates physician orders, staff assignments, and communications between direct care team members, the patient and their family, and the physician and primary care physician (as appropriate).</li><li>
Reviews and approves plan of care and evaluates proposed changes to the plan of care for clinical appropriateness.</li><li>
Integrates orders from all relevant physicians involved in the plan of care and ensures the orders are approved by the responsible physician.</li><li>
Conducts regular OASIS reviews for all patients.</li><li>
Meets with all direct care team members on a regular basis to provide guidance, coaching and communicate information related to specific issues or organizational changes Initiates hiring requests, interviews candidates and recommends hiring of direct care staff.</li><li>
Communicates frequently with the Branch Director or Administrator on key performance metrics.</li><li>
Remains available for field work, as needed.</li><li>

What is Required?<ul><li>
Associate degree in a Nursing related field required</li><li>

Job Description
Essential Functions:<ul><li>
Optimize care coordination with hospital, ER, Nursing homes, Assisted living facilities, physicians and community resources as necessary. Collaborates with physicians, providers and interdisciplinary team to indentify appropriate patients for home care and hospice.</li><li>
Acts as a resource to interdisciplinary teams, patients and families throughout episode of care. Facilitates efficient, appropriate, seamless, coordinated care to help ensure positive patient outcomes.</li><li>
Quality review of data, care plans, coding, documentation, oasis, HIS is analyzed to ensure the achievement of consistent clinical/financial outcomes. Data outcomes is used to identify indicators where process improvements projects are developed.
</li><li>Works with caregivers, schedulers, physicians and administration to coordinate care based on patient needs, geographic area, caregiver skill set to meet regulatory standards. Demonstrates effective fiscal management through cost containment measures while ensuring high quality, cost effective care is being provided to patients within the home care program.</li><li>
Assists leadership in the development of program goals and objectives. Assist with the development , implemnatation and maintenance of quality programs that include best practice patient care outcomes.</li><li>
Directly or indirectly ensures 24 hour care provided 7 days per week across our service area.</li><li>
Coaches staff and assists in performing supervisory visits to ensure regulatory compliance is met. Takes a leadership role in the participation of any survey across the division. Assists leadership in action plans/audits.</li><li>

Additional Requirements
Required:
Education - Completion of a nursing education program that is approved by a board of nursing
Certification - Registered Nurse (RN) - South Dakota Board of Nursing; Basic Life Support (BLS) Certification - American Heart Association (AHA) - Within 60 days of hire or transfer.
Regarding BLS certification: The department you are applying to may require BLS certification sooner than 60 days post transfer/hire. The hiring manager will discuss this with you if it is applicable. 
 
Preferred: 
Education - Bachelors in Nursing
Experience - 3+ years of Registered Nurse Experience; 3+ years of Home Medical Equipment Experience
Scheduling Requirements: Requires the ability to work assigned schedules and hours as determined by the department.
 
 
Physical Requirements: Heavy work - exerting up to 25 pounds of force constantly (67-100% of the time), and/or up to 50 pounds frequently (34-66% of the time), and/or up to 100 pounds of force occassionally (up to 33% of the time), and or up to 100 pounds of force seldomly to move objects. <ul><li>Exceed physician, provider and member expectations by providing professional and personalized service at all times.</li><li>Is aware of and continually supports strategic plans that ensure company objectives and goals are obtained.</li><li>Attends collaborative meetings with both internally and externally healthcare team members to support management services for clients served.</li><li>Builds and leverages cross-functional collaborative relationships to facilitate a team-work oriented atmosphere.</li><li>Assess clients for care management services after a referral has been made based on the admission screening and referrals for potential patient issues.</li><li>Develop and manage a coordinated plan of care to meet client and/or family needs</li><li>Assure that clients are placed on appropriate self-management plan and protocols, individualizes client care needs through consultation with the multidisciplinary team.</li><li>Coordinates services to support the appropriate care needs to promote best client outcomes and efficient use of healthcare resources.</li><li>Works with clients and providers to enhance the quality of client management and satisfaction.</li><li>Performs appropriate assessments and develops client-directed care plans within health plan required timeframes and per NCQA standards.</li><li>Assess need to involve medical director or primary care team when appropriate to assist with development of client-directed plan of care.</li><li>Regularly reviews and updates care plans for continuity of care and facilitates plan modifications including barriers to goals.</li><li>Coordinates with Non-Licensed Care Navigators to ensure appropriate follow-up with high risk clients to address identified goals and barriers.</li><li>Documents all interventions and telephone encounters with providers, members and vendors in accordance with established documentation standards.</li><li>Provide comprehensive training for new employees</li><li>Continues own education by keeping his/her knowledge current and conducts independent research of commercial plan guidelines to strengthen general understanding of state and federal resources to support position responsibilities.</li><li>Ensures compliance with departmental and PSW policies and procedures, with special emphasis on compliance with HIPAA privacy and security requirements and all state, federal and plan regulatory mandates.</li><li>Provides Transitional Care Management services post discharge from emergency room, acute inpatient or post-acute inpatient stays.</li><li>Follows Eric Coleman&rsquo;s Pillars of Transitional Care to support reduced risk of adverse events following discharge.</li><li>Provides education on seeking medical attentional early or using urgent care services when appropriate versus an emergency room.</li><li>Follows the standards of care management services as outlined in the guideline manual updated no less than annually.</li><li>Provides telephonic engagements to clients on worklist to meet standards of engagement to include both timing, frequency, and duration of engagements.</li></ul></div>

Required Education and/or Work Experience: 
<ul><li>Three (3) years of full proficiency clinical experience.</li><li>Verifiable experience or knowledge of a variety of clinical areas of medical treatment as related to general population and pediatric needs.</li><li>Knowledge and experience in providing care management and/or transitional care management services</li></ul></div>

Preferred Education and/or Work Experience: 
<ul><li>Bachelor&rsquo;s degree from accredited school of nursing.</li><li>Case Manager Certification.</li><li>Experience with commercial health plans.</li></ul></div>

Required Certificates, Licenses and Registrations: 
<ul><li>Nursing degree (ie: RN)</li><li>Active unrestricted state license in State of Washington.</li></ul></div>
