As an Elara Caring Clinical Team Manager, you&rsquo;ll contribute to our success in the following ways:<ul><li>
Provides oversight of all patient care services and personnel including: making patient and personnel assignments, coordinating patient care, coordinating referrals, assuring patient needs are continually assessed, and assuring the development, implementation, and updates to the individualized patient plan of care.</li><li>
Plans, directs, and participates in delivery of home health services to provide diagnosis-specific patient care, including coordination of services with providers, vendors, or facilities.</li><li>
Ensures the timely start of patient care within recommended regulatory guidelines.</li><li>
Processes and coordinates physician orders, staff assignments, and communications between direct care team members, the patient and their family, and the physician and primary care physician (as appropriate).</li><li>
Reviews and approves plan of care and evaluates proposed changes to the plan of care for clinical appropriateness.</li><li>
Integrates orders from all relevant physicians involved in the plan of care and ensures the orders are approved by the responsible physician.</li><li>
Conducts regular OASIS reviews for all patients.</li><li>
Meets with all direct care team members on a regular basis to provide guidance, coaching and communicate information related to specific issues or organizational changes Initiates hiring requests, interviews candidates and recommends hiring of direct care staff.</li><li>
Communicates frequently with the Branch Director or Administrator on key performance metrics.</li><li>
Remains available for field work, as needed.</li><li>

What is Required?<ul><li>
Associate degree in a Nursing related field required</li><li>

Job Description
Essential Functions:<ul><li>
Optimize care coordination with hospital, ER, Nursing homes, Assisted living facilities, physicians and community resources as necessary. Collaborates with physicians, providers and interdisciplinary team to indentify appropriate patients for home care and hospice.</li><li>
Acts as a resource to interdisciplinary teams, patients and families throughout episode of care. Facilitates efficient, appropriate, seamless, coordinated care to help ensure positive patient outcomes.</li><li>
Quality review of data, care plans, coding, documentation, oasis, HIS is analyzed to ensure the achievement of consistent clinical/financial outcomes. Data outcomes is used to identify indicators where process improvements projects are developed.
</li><li>Works with caregivers, schedulers, physicians and administration to coordinate care based on patient needs, geographic area, caregiver skill set to meet regulatory standards. Demonstrates effective fiscal management through cost containment measures while ensuring high quality, cost effective care is being provided to patients within the home care program.</li><li>
Assists leadership in the development of program goals and objectives. Assist with the development , implemnatation and maintenance of quality programs that include best practice patient care outcomes.</li><li>
Directly or indirectly ensures 24 hour care provided 7 days per week across our service area.</li><li>
Coaches staff and assists in performing supervisory visits to ensure regulatory compliance is met. Takes a leadership role in the participation of any survey across the division. Assists leadership in action plans/audits.</li><li>

Additional Requirements
Required:
Education - Completion of a nursing education program that is approved by a board of nursing
Certification - Registered Nurse (RN) - South Dakota Board of Nursing; Basic Life Support (BLS) Certification - American Heart Association (AHA) - Within 60 days of hire or transfer.
Regarding BLS certification: The department you are applying to may require BLS certification sooner than 60 days post transfer/hire. The hiring manager will discuss this with you if it is applicable. 
 
Preferred: 
Education - Bachelors in Nursing
Experience - 3+ years of Registered Nurse Experience; 3+ years of Home Medical Equipment Experience
Scheduling Requirements: Requires the ability to work assigned schedules and hours as determined by the department.
 
 
Physical Requirements: Heavy work - exerting up to 25 pounds of force constantly (67-100% of the time), and/or up to 50 pounds frequently (34-66% of the time), and/or up to 100 pounds of force occassionally (up to 33% of the time), and or up to 100 pounds of force seldomly to move objects. As a Home Health Clinical Manager, You will:</div><ul><li>
Conducts/participates in quarterly quality improvement meetings, reviews pertinent available data, identifies trends, and oversees the implementation of improvement plans</li><li>
