About the Role</b><br>
The Afterhours Care Manager RN is responsible for assessing and coordinating patient and family needs during after-hours shifts and maintaining primary clinical oversight for assigned patients. This includes routine visits, admissions, death visits, urgent after-hours calls, and any visit assigned per the patient&rsquo;s plan of care. This role serves as an active member of the Interdisciplinary Group (IDG) to ensure coordination between physicians, families, and the hospice team. Clinical administrative responsibilities, workflow auditing, and survey readiness support are also part of this role.</p><p></p><p><b><br>
Essential Functions</b></p><ul><li>
Ensure appropriate delivery of hospice services to eligible patients and families.</li><li>
Provide direct in-person patient care during on-call, weekend, and after-hours shifts, including assessments, admissions, routine visits, death visits, and live discharges.</li><li>
Conduct initial and ongoing assessments of physical, emotional, psychosocial, and spiritual needs.</li><li>
Support the IDG process through chart reviews, plan of care updates, and interdisciplinary communication.</li><li>
Develop and revise the Hospice Plan of Care in collaboration with physicians and IDG members.</li><li>
Educate patients and families on disease progression, symptom management, medications, and hospice philosophy.</li><li>
Communicate significant changes in patient condition promptly to hospice and attending physicians.</li><li>
Maintain accurate, timely clinical documentation including assessments, care plans, and notes.</li><li>
Assist with rescheduled, missed, or reassigned visit requests based on triage direction.</li><li>
Participate in calendar and scheduling modifications, including service codes and clinician assignments.</li><li>
Support administrative and compliance activities including workflow audits, survey readiness tasks, and documentation collection.</li><li>
Assist with TPE reviews, QAPI efforts, and regulatory compliance.</li><li>
Ensure infection control standards and universal precautions are consistently followed.</li><li>
Provide leadership and guidance to field staff; conduct supervisory visits when appropriate.</li><li>
Participate in required meetings, after-hours updates, and trainings.</li><li>
Maintain proficiency in pain and symptom management for hospice and end-of-life care.</li><li>

Responsibilities:

Recommends employment decisions for staff to the director of operations, including decisions to hire, fire, advance, promote or change status.</div></li><li>
Responsible for overall quality of patient care and clinical performance of nurses and aides.</li><li>
Organizes clinical operations for the care center.</li><li>
Reviews requests for services and determines patient eligibility/suitability for home care services. Works with intake in coordinating patient referrals and admissions.</li><li>
Oversees scheduling.</li><li>
Oversees payor verification and precertification requirements.</li><li>
Reviews documentation of other staff members and ensures missing, incomplete and/or untimely documentation issues are resolved.</li><li>
Assists clinicians in establishing immediate and long-term patient goals, setting priorities and developing Plan of Care (POC).</li><li>
Regularly evaluates the needs of the patient and makes necessary revisions to the plan of care in collaboration with clinicians.</li><li>
May provide direct patient care on an infrequent basis in times of emergency.</li><li>
Participates in the on-call process.</li><li>
Assists with training of staff to ensure quality care, compliance and fiscal responsibility.</li><li>
Participates in the investigation and resolution of patient/family/physician concerns.</li></ul></div></div><p></p>
Qualifications:

Responsibilities:

Collaborates with patient care teams to develop and implement care plans.</li><li>
Educates families on patient condition, needs and care.</li><li>
Performs head to toe physical assessments.</li><li>
Checks vital signs and manages medications.</li><li>
Gives injections, IV therapy or tube feedings.</li><li>
Administers and maintains catheters.</li><li>

Supervises LPNs and HHAs.</li><li>

Qualifications:
<div><div><div><ul><li>One year of clinical experience as a registered nurse (RN). If less than 1 year clinical experience as a RN, candidate must be approved by VP clinical.*</li><li>
Current RN license, specific to the state(s) you are assigned to work.</li><li>
Minimum six months home health experience.</li><li>
Current CPR certification.</li><li>
Valid driver's license, reliable transportation and liability insurance.</li><li>
Able to assess patient status and identify requirements relative to age specific needs.</li><li>
Must be familiar with general use and functions of the computer, such as, user names and password concepts; internet; e-mail; navigation of computer desktop, including starting programs, using files and windows, effectively use navigation buttons and tool bars; ability to self-manage online HR services and online training programs.</li><li>Note - If less than 6 months clinical experience as a RN, candidate must participate in RN Intern program.</li></ul></div></div></div><div></div><div><div><i>
