POSITION SUMMARY

Considers the physical, cultural, psychosocial, spiritual, age specific and educational needs of the patient in the plan of are. Plans care in collaboration with members of the multidisciplinary team. Reviews and revises the plan of care to reflect changing patient needs based on evaluation of the patient&rsquo;s status. 
 
3. Implements the plan of care through direct patient care, coordination, delegation and supervision of the activities of the health care team. Provides care based on physician orders and the nursing plan of care, in compliance with policies and procedures, standards of care, and regulatory agency requirements. Delegates appropriately, and provides nursing supervision in the provision of care to patients by other licensed nurses and other personnel. Registered Nurse (RN) &ndash; Home Care Clinical Oversight (Part-Time / On-Call) ameriCARE Twin Cities NorthWe are a growing non-medical home care agency serving the north Twin Cities metro, currently expanding our license from Basic to Comprehensive under Minnesota Statutes 144A.As part of this transition, we are hiring a Registered Nurse to provide the clinical oversight required under a Comprehensive Home Care license.This is not a traditional full-time clinical role. It is a low-hour, high-responsibility position designed for an experienced RN who values flexibility, autonomy, and clear expectations.The RoleThis position exists to fulfill the clinical and supervisory requirements of a Minnesota Comprehensive Home Care license.You will serve as the agency&rsquo;s RN of record, responsible for ensuring that care plans, assessments, and oversight meet state requirements. The day-to-day caregiving and scheduling functions are handled by our internal team.This role is primarily:<ul><li>On-call / as-needed</li><li>Low time commitment (estimated 5&ndash;10 hours/week to start)</li><li>Flexible scheduling</li></ul>Core Responsibilities<ul><li>Complete initial and ongoing client assessments as required by MN 144A</li><li>Develop and oversee client care plans</li><li>Provide clinical supervision of unlicensed personnel (UP)</li><li>Be available for consultation on client condition changes or care questions</li><li>Participate in required documentation and policy compliance</li><li>Support readiness for state survey and ongoing regulatory compliance</li></ul>What This Role Is NotTo be clear&mdash;this is not:<ul><li>A shift-based nursing role</li><li>A high-volume visit model</li><li>A micromanaged environment</li></ul>We are not looking to &ldquo;fill hours.&rdquo; We are looking for the right clinical partner to help us operate correctly and responsibly under a Comprehensive license.What We Are Looking For<ul><li>Active Minnesota RN license (required)</li><li>Experience in home care, case management, or community-based care (strongly preferred)</li><li>Familiarity with MN 144A home care regulations (or willingness to learn quickly)</li><li>Comfort working independently without constant oversight</li><li>Strong judgment and ability to make practical, real-world decisions</li><li>Clear, direct communicator</li></ul>Compensation &amp; Structure<ul><li>Hourly or monthly retainer structure (based on experience and availability)</li><li>Flexible hours with significant autonomy</li><li>Designed to fit alongside another role, semi-retirement, or independent consulting work</li></ul>About UsameriCARE Twin Cities North is built around a simple idea: care should support independence, not quietly take it away.We focus heavily on care models that reduce the burdens of living so our clients can lead fuller, happier lives.
