As a Physical Therapist Assistant, you&rsquo;ll contribute to our success in the following ways:<ul><li>
Promotes Elara Caring&rsquo;s philosophy, mission statement and administrative policies to ensure quality of care</li><li>
Performs direct physical therapy services in accordance with accepted standards of practice and certified by the patient&rsquo;s physician to ensure quality and continuity of care.</li><li>
Follows Plan of Care as established by Physical Therapist/being supervised by the Physical Therapist and immediate reporting of any changes in status back to Physical Therapist.</li><li>
Adheres to the State Practice Act Guidelines.</li><li>
Verifies the Plan of Care prior to each visit and provides care according to physician&rsquo;s orders, assessment data, and established standards and guidelines.</li><li>
Incorporates patient care goals established in the Plan of Care, into therapy care.</li><li>
Instructs patients and/or their families in the appropriate and safe use of prosthetic, orthotic and assistive devices (canes, walkers, wheelchairs, etc.).</li><li>
Instructs the patient and/or caregiver in the therapy home program</li><li>
Physically demanding, high stress environment</li><li>
Full range of body motion including handling, lifting and transfer of patients</li><li>
Potentially work irregular hours including call hours if applicable</li></ul>
What is Required?<ul><li>
Graduate of an accredited physical therapy assistant program approved by the American Therapy Association</li><li>
Current, unrestricted PTA license valid for the state of work.</li><li>
6+ months experience as a Physical Therapist Assistant is preferred</li><li>
Able and willing to travel within branch/office coverage area.</li><li>
Must have a dependable vehicle, valid driver&rsquo;s license, and current auto insurance in accordance with state laws.</li><li>
Must be able and willing to travel 50%</li><li>
Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment</li></ul>
You will report to the Branch Director, Clinical Manager, Clinical Supervisor or designee.

Responsibilities for a PT - Physical Therapist include:</div>

<li><div>Make home visits to clients in designated geographic territories.</div>
</li><li>Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.</li>
<li>Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents.</li>
<li>Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.</li>
<li>Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.</li>
<li>Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.</li>
<li>Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.</li></ul>

Qualifications for a PT - Physical Therapist include:</div>

<li>A current Maryland Physical Therapist - PT license.</li>
<li>A minimum of one year of recent work experience as a Physical Therapist - PT.</li>
<li>Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.</li>
<li>Ability to work independently and manage time effectively.</li>
<li>Strong interpersonal, organizational, and problem solving skills.</li>
<li>Solid computer skills; prior experience with electronic medical records (EMR) preferred.</li>

A Physical Therapist provides professional services within the scope of physical therapy practice standards in collaboration with primary care physicians and other members of the health care team and teaches and educates patients and their families. 


Qualifications: 


Successful completion &amp; graduate of a PT Program approved by the Commission on Accreditation in PT education (CAPTE) or successor organizations of the CAPTE or an education program outside of the United States determined to be substantially equivalent to physical therapist entry level education in the US by a credentials eval organization approved by the American PT Association or an organization identified in 8 CFR 212.15(e) as it relates to physical therapists.

A current, active license to practice Physical Therapy in .

A minimum of one (1) years experience as a Physical Therapist.

Criminal background check &amp; Current CPR certification. 


If trained outside of the United States before January 1, 2008, meets the following requirements:

<li>Was graduated since 1928 from a PT curriculum approved in the country in which the curriculum was located and in which there is a member organization of the World Conference for Physical Therapy.

<li>Meets the requirements for membership in a membership organization of the World Confederation for Physical Therapy.</li></ul> 

Responsibilities/Essential Functions: 


Receives the intake referral information and prepares PT paperwork/tools necessary for the visit.

Obtains all pertinent medical history from patient, family or significant others.

Performs PT assessment visit and documents accordingly.

Performs home safety check and environmental assessment of the patient's home environment.

Assists the physician in evaluating the level of function.

Helps develop an appropriate and effective PT Plan of Care (POC) to be submitted to the physician for approval and implementation, revising as necessary and at least every 30 days.

Functionally reassess the patient for on-going therapy.

Determines medical necessity for other services.

Evaluates the patient's ADL and in ADL abilities.

Develops and implements the HHA plan of care when HHA services are ordered and PT is the only skilled discipline.

Supervises the HHA in accordance with state/federal requirements and documents the supervision without having to be directed to do so, when PT is the only skilled discipline.

Effectively communicates ongoing with patient and family regarding the POC and progression of such.

Effectively instructs patients and families in the safe use of prosthetic, orthotic, and assistive devices (canes, walkers, wheelchairs, mechanical lifts etc.).

Effectively communicates with other disciplines in the case.

Communication with the patient's physician (verbally and/or in writing) to obtain effective treatment modalities and/or rehabilitative therapy modalities.

Assists the physician in evaluating patients by applying diagnostic and prognostic muscle, nerve, joint and functional ability tests.

Submits ALL OASIS assessments/documentation within 48 hours of the OASIS visit.

Performs clinical record reviews (CRR) in collaboration with the Nursing Supervisor (Peer Review).

Submits the CRR timely each month.

Participates in the Performance Improvement Committee process as requested.

Participates in staff meetings and in-services.

Participates in the growth of the office by being a willing preceptor for newly hired, same discipline employees.

Participates in the planning, operation and evaluation of the therapy services of the organization.

Maintains professional licensure and actively seeks out educational experiences to enhance the practice of home care physical therapy and the benefit of the patients serviced.

Notifies the Agency of emergencies, sickness, and other imminent occurrences that may affect the patient caseload as quickly as possible relative to the event's occurrence.

Submits written time requests 2 weeks or more in advance of planned time off.

Other duties as assigned.

Vehicle to do routine patient visits, possible transfer/lifting devices, medical supplies, PT assistive devices 


Work Environment: 


Indoors, performing patient assessment/tasks in the patient's residence Travel to patient residences/meetings; will require transportation to get to various work sites Possible exposure to communicable diseases 


Functional Abilities: 


Able to access patient's homes that may not be handicap accessible.

Able to hear, speak, understand and communicate effectively in English.
