ESSENTIAL JOB FUNCTIONS</h1><div><div><ul><li>
Provides educational information to patients and families regarding the hospice philosophy, eligibility requirements, admission procedures, and Angela Hospice Home Care Inc.
</li><li>Completes an initial assessment of patient and family to determine hospice needs</li><li>
Completes all the initial necessary admission paperwork with the patient and/or family member/caregiver to get the patient signed onto Angela Hospice services</li><li>
Provides a complete physical assessment and history of current and previous illness(es)
</li><li>Initiates the patient&rsquo;s plan of care and includes the patient and the family in the planning process</li><li>
Provides quality, professional nursing care to patients utilizing specialized nursing skills related to end-of-life care</li><li>
Performs comprehensive assessment of assigned patients in a timely manner upon admission and as required</li><li>
Assesses and evaluates patients and makes necessary revisions to the plan of care as patient status and needs change</li><li>
Initiates appropriate preventive and rehabilitative nursing procedures
</li><li>Administers and documents medications and treatments as prescribed by the physician</li><li>
Orders medications as needed from pharmacy</li><li>
Educates the patients and families regarding medications, treatments and procedures, disease process, and end of life care/process</li><li>
Uses established confidential communication channels to communicate pertinent information</li><li>
Communicates patient/family concerns to appropriate interdisciplinary team member and initiatives a corrective action if required</li><li>
Assists the patient with activities of daily living and facilitates the patient&rsquo;s efforts toward self-sufficiency and optional comfort care</li><li>
Lifts, moves, and transports patients using proper body mechanics or lifting devices
</li><li>Completes, maintains and submits accurate and relevant clinical notes regarding patient&rsquo;s condition and care given
</li><li>Records pain/symptom management changes/outcomes as appropriate</li><li>
Communicates with the physician regarding the patient&rsquo;s needs and reports changes in the patient&rsquo;s condition</li><li>
Communicates and collaborates with community health related persons to coordinate the care plan</li><li>
Teaches the patient and family/caregiver self-care techniques as appropriate</li><li>
Provides and maintains a safe environment for the patient</li><li>
Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver</li><li>
Ensures that arrangements for equipment and other necessary items and services

Maintains the comfort, privacy, and dignity of each patient and interacts with patients in a manner of compassion and empathy</li><li>
Reports an incident or evidence of abuse or violation of residents rights to the Director of Patient Access, or any administrative staff</li><li>

Ensures compliance with the Medicare conditions of participation and other state regulations that govern the provision of healthcare</li><li>
Complies with all Health Insurance Portability and Accountability Act (HIPAA) requirements in accordance with federal, state and organizational policies</li><li>
Participates in organizational monitoring of the quality of medical services and quality improvement initiatives</li><li>
Develops, maintains and upgrades professional knowledge and practice skills through attendance at seminars, conferences and participation in continuing education and in-service classes</li><li>
Fulfills the obligation of requested and/or accepted assignments</li><li>

SUPERVISORY RESPONSIBILITY</h1><div><p>
This position does not have any direct supervisory responsibilities, but provides clinical supervision to LPS, SNAs and hospice aides. This position is required to orient employees as needed and will act as a mentor to staff and volunteers.</p></div><br>

PHYSICAL REQUIREMENTS</h1><div><p>
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk and hear. The position requires sitting and standing for long periods. The position is also exposed to infectious diseases and requires lifting of a minimum of 50+ pounds regularly. It also requires bending, stooping, turning and twisting on a regular basis. They require the ability to operate a computer, phone and other office equipment as needed. Manual dexterity is also required to perform tasks including computer tasks.</p></div><br>

WORK ENVIRONMENT</h1><div><p>
This position will be required to operate in an infield home health care setting under all types of weather conditions as required. The role requires working directly with patients and other team members.

In this role, you will provide high-quality nursing services to patients in the comfort of their own homes, helping them manage their care and regain independence. You will oversee a supporting Licensed Vocational Nurse and work closely with physicians, therapists, and a supportive clinical team to coordinate care and support patients and their families.

 What You'll Do:

   Monitor patient care plans, ensuring compliance with medical regulations and home health standards.

   Coordinate patient admissions, discharges, and transfers, ensuring all necessary documentation and medical records are complete and up-to-date.

   Work closely with field nurses, aides, and other staff to assign cases, manage schedules, and oversee the timely completion of patient visits.

   Provide education to patients and families regarding home health services, treatment plans, and medication management.

   Assist in the development and implementation of quality control procedures, ensuring the highest level of patient care and compliance with regulatory requirements.

   Collaborate with the billing department to review patient records, ensure proper coding, and resolve any billing or insurance issues.

   Maintain accurate patient files, clinical documentation, and compliance reports in accordance with state and federal regulations.
