The Case Manager provides intensive, evidence-based home visiting services to pregnant and postpartum parents and their families to improve maternal and child health, strengthen parent-child relationships, and promote family self-sufficiency. The Case Manager delivers trauma-informed, culturally responsive, and strengths-based services, including assessments, care coordination, counseling, referrals, and advocacy.
Key Responsibilities
Direct Client Services<ul><li>
Provide regular home visits to program participants, delivering education and coaching on prenatal care, infant/child health, breastfeeding, parenting, nutrition, and safe sleep.</li><li>
Conduct comprehensive assessments (health, psychosocial, safety, developmental) for mothers, infants, and families.</li><li>
Develop and monitor individualized service plans tailored to client needs and goals.</li><li>
Offer counseling and support around perinatal mood disorders, stress management, and family strengthening.</li><li>
Connect families with medical homes, mental health providers, social services, housing, employment, and educational resources.</li></ul>
Documentation &amp; Compliance<ul><li>
Maintain accurate, timely case notes and data in the electronic record system in compliance with federal, state, and local program requirements.</li><li>
Complete required program forms (assessments, care plans, progress notes, consents) within designated timelines.</li><li>
Track and report outcomes for maternal and child health indicators (e.g., prenatal care, immunizations, developmental screenings).</li></ul>
Collaboration &amp; Advocacy<ul><li>
Coordinate services with health providers, schools, early childhood programs, and social service agencies.</li><li>
Advocate on behalf of clients to access benefits, rights, and resources.</li><li>
Participate in case conferences, multidisciplinary team meetings, and community partnerships.</li></ul>
Professional Development &amp; Supervision<ul><li>
Participate in reflective supervision, peer case review, and ongoing professional development.</li><li>
Maintain compliance with continuing education requirements for licensure.</li></ul>
Uphold program fidelity to evidence-based home visiting models.
Minimum Qualifications<ul><li>
Master&rsquo;s degree required in Social Work (MSW), Counseling, Psychology, Maternal/Child Health, Nursing, or a related human services field.</li><li>
At least 2 years of experience in case management, counseling, or direct services with pregnant women, infants, or young children.</li><li>
Knowledge of maternal and child health, infant development, trauma-informed care, and family systems.</li><li>
Strong interpersonal, communication, and crisis intervention skills.</li><li>
Ability to conduct home visits and work a flexible schedule, including occasional evenings/weekends.</li><li>
Proficiency with Microsoft Office and electronic case management systems.</li></ul>
Preferred Qualifications<ul><li>
Licensed or license-eligible in DC (e.g., LICSW, LPC, LMFT, RN).</li><li>
Bilingual skills (Spanish, Amharic, Arabic, Dari, or other languages common in DC).</li><li>
Experience with evidence-based home visiting models (NFP, PAT, HFA, EHS-HV).</li><li>

<div>RN Case Manager is responsible in handling case management of home health patients and the assigned staff team to ensure effective and quality delivery of home health care services in accordance with the State and Federal Regulations and the Agency's Policies and Procedures. He/She coordinates plan of care, patient care and physician's orders with assigned team members of Registered Nurses, Licensed Vocational Nurses, Medical Social Workers, Certified Home Health Aides, and Rehab Services.

Essential Functions:</h4>
1. Acts as an effective liaison to the home health team to ensure continuity and congruity of home health services in accordance with the patient's plan of care.
2. Oversees a team of Registered Nurses, Licensed Vocational Nurses and Certified Home Health Aides to ensure effective and quality home health services.
3. Provides professional home health care case management under the direction of the Director of Patient Care Services or Assistant Director of Patient Care. Communicates appropriate status/condition changes following Agency's Policies and Procedures.
4. Works collaboratively with others to achieve goals, solve problems, and meet established organizational objectives and management of patients.
5. Responsible for the overall management of assigned patients including the provision of direct care following a visit productivity standard.
6. Coordinates with the home health team and ensures implementation of home home health plan of care, effective visit utilization management and control.
7. Plans, organizes, and directs all patient care services in the assigned caseload ensuring achievement of optimum quality home health care.
8. Reviews patient referral information and responds to requests and inquiries as needed.
9. Coordinates with physicians, other health care professionals - rehab therapists, social services in developing and coordinating plan of care.
10. Coordinates all information on patient referrals, plan of care, patient care and physician orders.
11. Assists in establishing the initial Plan of Care and provides supplies and equipment arrangements for the client.
12. Ensures implementation of a correct, timely and complete clinical documentation in accordance with the Agency's Policies and Procedures and State and Federal regulatory requirements.
13. Communicates with clinicians and provides training and guidance following compliance with the Agency Policies and Procedures.
14. Participates in home health case conferences and other Agency meetings.
15. Provides guidance to Agency home health staff, other health care professionals, patients, and families related to appropriate and available health care resources.
16. Adheres and implements compliance to Health Insurance Portability and Accountability Act of 1996 (HIPAA) for protection of confidentiality of all written and computer files.
17. Promotes and participates in the development and execution of the Quality Assurance Performance Improvement (QAPI) Program and activities.
18. Other related duties and responsibilities that may become necessary or appropriate to meet the clinical care and compliance needs of agency.<h4 class="jobSectionHeader">
Knowledge, Skill and Experience:</h4>
1. Registered Nurse with current license to practice in the State of Arizona
2. Minimum Experience required: Minimum of 1 year of home health case management experience preferred;
3. Knowledge of home health regulatory and reimbursement requirements preferred;
4. Strong coordination, multitasking, telephone skills, organizational and interpersonal skills with sensitivity toward Medicare population required;
5. Computer skills required both clinical and clerical programs; With Computer and Microsoft Office Skills; Must be able to type a minimum of 45 words per minute and have a working knowledge on medical terminology;
6. Excellent verbal and written communication skills;
7. Ability to work independently on assigned tasks as well as accept direction on given assignments;
8. Ability to supervise and direct members of home health care team;
9. With organizational and time management skills.
Typical Working Conditions: Typical office environment - no unusual exposures.

Essential Physical Tasks: Must occasionally carry boxes of patient information documents weighing approximately 30 lbs. Work at computer monitors for prolonged periods must guard against eyestrain and muscle pain.

The Physical Therapy (PT) Case Manager provides and oversees home health services for clients within the community. Collaborates with the client, their family, and the health care team to develop and implement goal-centered care plans taking into consideration quality measures and financial aspects of treatment. Provides skilled care to clients and clinical oversight to professional and unlicensed personnel. Builds and maintains respectful relationships with others. The position reports to the Clinical Supervisor.
 
SCHEDULE

<li>This is a full time position, Monday - Friday. As PT Case Manager, you will manage your schedule during office hours.</li>
<li>This position will require you to travel to client's homes to provide 1:1 client care that may take 1-3 hours per client.</li>

<li>This position reports to the St. Cloud, MN office and provides services in Cold Spring, St. Joseph, Paynesville, St. Augusta and surrounding cities.</li>

DUTIES AND RESPONSIBILITIES
 <ul>
<li>Reflects the company’s mission and values in developing and maintaining professional working relationship with others.</li>
<li>Performs initial and periodic assessment of clients to determine care needs, implements interventions, and evaluates the effectiveness of care to achieve positive outcomes.</li>
<li>Collaborates with physicians/non-physician practitioners and the home health team to develop and update the client’s home health plan of care at least every 60 days or more often as changes occur.</li>
<li>Serves as a client advocate ensuring safety needs are addressed by assessing the home environment, developing and implementing individualized abuse prevention plans, and reporting any suspected vulnerability such as abuse, neglect or financial exploitation.</li>
<li>Meets documentation standards ensuring accuracy, timeliness, and compliance with state and payer/billing requirements.</li>
<li>Provides education to the client, family, caregivers, licensed clinicians (as indicated), and unlicensed personnel.</li>
<li>Provides direct care to clients of varying ages, clinical acuity levels, and diagnoses in accordance with the company’s policies, procedures, processes and the Minnesota Physical Therapy Practice Act.</li>
<li>Coordinates client care with family, caregivers, community resources, and other health care</li>

members.

<li>Provides clinical oversight, delegation, and leadership to licensed clinicians and unlicensed personnel.</li>
<li>Ensures effective and appropriate financial management of the client’s home health services are met.</li>
<li>Manages work schedule effectively, adapts to and remains flexible with unexpected changes</li>
</ul>
PHYSICAL/ENVIRONMENTAL DEMANDS
 Client care provided in individual client homes and may require transfers and lifting with adaptive equipment. Must be adaptable to a variety of environments and community settings. Must be able to drive or use public transportation in all types of weather.
 
Heavy. Amount of weight moved; 50 to 100 pounds occasionally; 25 to 50 pounds frequently; 10 to 20 pounds constantly. Physical Demand requirements are in excess of those for Medium Work.
 
QUALIFICATIONS
 <ul>
<li>Currently licensed as a Physical Therapist (PT) in the State of Minnesota. The license must not have been revoked, suspended and without limitations or restrictions.</li>
<li>Proof of negative TB testing: Mantoux skin test, single TB blood test or chest x-ray.</li>
<li>Access to a dependable automobile in order to travel to multiple company business stops a day.</li>
<li>A valid driver’s license and proof of current car insurance when using a personal vehicle for company business.</li>
<li>Have U.S. Citizenship or evidence of valid Alien Work Permit.</li>

SKILLS AND EXPERIENCE
 <ul>
<li>Have at least two (2) years’ experience in a health care facility or home care field.</li>
<li>Demonstrates critical thinking skills, the ability to follow standards/procedures, and make appropriate clinical judgments to provide safe and effective client care.</li>
<li>Ability to work independently with minimal supervision, adapt to change, and manage a variety of assignments while establishing priorities and ensuring the necessary duties are completed.</li>
<li>Ability to build and maintain professional relationships.</li>
<li>Excellent written and oral communication skills; interpersonal communication and teaching skills.</li>
<li>Basic computer skills and willing to learn new software programs; experience with Electronic Medical Records and Microsoft Outlook preferred.</li>
<li>Current CPR certification highly recommended.</li>
