Pay Range: $91,651.00 - $137,477.00

This hybrid role provides care through in-home visits and telehealth, allowing providers to build meaningful relationships with patients while delivering personalized care in the environments where patients feel most comfortable. Key Responsibilities<ul><li>
Perform comprehensive health assessments and physical examinations during in-home visits and telehealth appointments.</li><li>
Evaluate, diagnose, and treat acute and chronic conditions with an emphasis on preventive care.</li><li>
Develop and implement individualized treatment plans tailored to each patient&rsquo;s needs.</li><li>
Educate patients and families on health management, wellness, and disease prevention.</li><li>
Collaborate with physicians, specialists, and interdisciplinary care teams to ensure coordinated care.</li><li>
Perform minor clinical procedures such as wound care as appropriate during home visits.</li><li>
Maintain accurate and timely documentation in electronic medical records (eClinicalWorks or similar platforms).</li><li>
Advocate for patients by providing respectful, inclusive, and equitable care, including individuals with disabilities.</li></ul>
Qualifications<ul><li>
Active, unrestricted Nurse Practitioner license (required).</li><li>
Experience in geriatrics, internal medicine, or primary care preferred.</li><li>
Previous home health or home-based care experience strongly preferred.</li><li>
Experience caring for patients with disabilities is highly desirable.</li><li>
Comfortable delivering care in both in-home and telehealth settings.</li><li>
Excellent communication skills and a collaborative, team-oriented mindset.</li></ul>
