Position Summary
The Homeless Outreach program is a dynamic and unique team. Our staff work autonomously out in the community to assist clients with basic needs, case management and referrals to higher levels of care or community partners. Objectives will be to create supportive relationships with homeless individuals through street outreach and referrals to community partners to prioritize meeting clients where they are at.
This position provides outreach services to Salt Lake City&rsquo;s unsheltered individuals and/or families in order to connect clients with supportive services with the goal to decrease Salt Lake City&rsquo;s unsheltered population and connect individuals with safe and stable housing. Our city care coordination team is made up of three individuals. This team will be provided a work vehicle and will be expected to provide services to unsheltered clients across the city limit. Daily tasks may include providing clients with essential supplies, driving clients to community partner appointments, and assisting clients with obtaining vital documents that will later be essential in the housing process. Care coordinators will have the ability to build rapport with clients to assess housing motivation and add clients to HOP&rsquo;s internal case management referral list. This team is expected to respond and assess community referrals. In this position you will also respond to community concerns, assist with community education, and problem solve around homelessness issues.
Essential Duties?<ul><li>
Proactively engages and initiates contact with clients in accessing basic needs items and community-based resources, to help them overcome homelessness.</li><li>
Prioritizes client engagement in Salt Lake City, identifying priority areas, coordinate needs, and address barriers.
</li><li>Identify priority needs, (i.e. pregnant, IV/IM drug user, dual diagnosis, and other medical conditions) and refer/coordinate services as needed.</li><li>
Provide direct, Trauma Informed Care to clients experiencing unsheltered homelessness, including crisis intervention, outreach, ongoing assessments (i.e., SPDAT, Lethality Assessment , Crises Response Plans, etc.), creative problem solving, agency and community resource utilization, and network building.</li><li>
Assess medical emergencies, and provides immediate and professional response to any emergency following appropriate policies and procedures. (contacting emergency medical, utilize Naloxone kits, Etc.)</li><li>
Participate in regularly scheduled staff meetings to ensure appropriate case planning and referral of clients to the most appropriate service provider. (i.e. CTG, CE enrollment, clinical staffing, etc.)</li><li>
 Ensuring that all paperwork related to client and program activities is properly completed, managed confidentially and stored in an appropriate location and submitted in a timely manner.</li><li>
Assist in resolving conflicts utilizing de-escalation skills and trauma informed practices.</li><li>
Evaluate own performance to increase effectiveness as a Care Coordinator and maintains willingness to accept supervision and direction.</li><li>
Maintain positive, professional interactions co-workers, and all other community providers including; Homeless Resource Center staff, emergency services, fire, police and sheriff, criminal justice, staff from other community providers, neighborhood residents and businesses, client family members, agency donors, community support groups and referral sources.</li><li>
Manage purchase receipts and submit them in a timely manner.</li><li>
Assist with community education, response, and problem solving around homelessness issues.</li><li>
Works to Identify and refer clients to Case Management when appropriate. Ability to utilize Trauma Informed practices to act and make sound decisions within the scope of position responsibilities as well as reach out for supervision for major situations.</li><li>
Maintain an active client caseload as assigned by supervisor, assist them in finding housing and self-sufficient resources.</li><li>
Assist with client moves.</li><li>

 Position Summary
 
 
 
 Under the leadership of the Center Manager, the Home Care Coordinator is responsible for the planning, monitoring, and scheduling of in-home services. The Home Care Coordinator will work closely with the Personal Care Supervisor (PCS) ensuring all personal care needs of the participants are met.
 
 
 
 Essential Functions

 Ensures that home care schedules are maintained, accurate, and participants are allocated enough time/services.

 Coordinates care through staffing with contracted home care providers as well as Personal Care Attendants (internal staff).

This role requires strong outreach skills, independent work in community settings, accurate documentation, and the ability to collaborate effectively across systems of care.This is a hybrid remote employment opportunity where much of the work is done via a combination of home-based work and field work reaching clients in their homes and other appropriate locations. This position is responsible for providing all aspects of Care Coordination in Chelan and Douglas counties under multiple care coordination programs.The ideal candidate will be trustworthy, organized, and have excellent interpersonal skills to engage and carry a case load of clients. The position requires effective outreach and recruitment of beneficiaries in a large and mostly rural geographic area, ability to work effectively with minimal supervision, and the use of Health IS platforms for documentation.Primary Duties:Outreach<ul><li>Conduct proactive outreach to identify, engage, and enroll eligible individuals into care coordination services in alignment with program requirements.</li><li>Perform outreach through multiple methods including phone, mail, electronic communication, in-person visits, and community-based settings.</li><li>Explain program purpose, services, and participant expectations clearly and accurately to potential participants and their families or caregivers.</li><li>Build and maintain effective working relationships with referral sources, community partners, healthcare providers, and social service organizations to support ongoing referrals and engagement.</li><li>Make repeated and timely attempts to contact hard-to-reach or disengaged participants using approved outreach strategies.</li><li>Document all outreach efforts, contacts, and outcomes accurately and timely in the Electronic Health Record (EHR) or required tracking systems.</li><li>Adjust outreach approaches based on participant needs, cultural considerations, geographic barriers, and program guidelines.</li><li>Meet program-defined outreach, engagement, and enrollment benchmarks as applicable.</li></ul>Client Engagement and Support<ul><li>Engage assigned participants through phone, mail, electronic communication, and in-person visits.</li><li>Build trusting relationships with participants and, when appropriate, their families or caregivers.</li><li>Complete screenings, assessments, and program-required tools to identify needs, strengths, and priorities.</li><li>Support participants in developing and following individualized care or action plans that reflect participant-identified goals.</li><li>Provide services in a respectful, trauma-informed, and culturally responsive manner, with attention to social determinants of health.</li></ul>Care Coordination and Collaboration<ul><li>Client Care Manager&rsquo;s complete program-required care plans, screenings, and consents in accordance with applicable care coordination models and contracts.</li><li>Coordinate care across healthcare providers, behavioral health agencies, social service organizations, and community resources.</li><li>Facilitate referrals and follow-up to ensure continuity and timeliness of services.</li><li>Participate in care team communication, case conferencing, and interdisciplinary collaboration as required by program standards.</li><li>Advocate for participants to reduce barriers to care and improve access to needed services.</li></ul>Documentation and Compliance<ul><li>Maintain accurate, timely, and thorough documentation of all participant contacts and care coordination activities in the Electronic Health Record (EHR) or other required systems.</li><li>Ensure confidentiality and security of participant information in compliance with HIPAA and agency policies.</li><li>Adhere to all relevant federal, state, contractual, and program-specific requirements.</li></ul>Administrative and Organizational Support<ul><li>Manage assigned caseloads effectively and meet productivity and documentation expectations.</li><li>Respond to participant inquiries and concerns in accordance with agency policies and procedures.</li><li>Represent the agency professionally in the community and with partner organizations.</li><li>Assist with program-related projects or duties as assigned by leadership.</li></ul>Requirements/Qualifications<ul><li>Bachelor&rsquo;s degree Nursing, Social Work, Psychology, Social Services, or a closely related field required;
