The Registered Nurse is responsible for the delivery of patient care through nursing process of assessment, planning, 
implementation, and evaluation. The registered nurse plans, organizes and directs home care services and is experienced in 
nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the 
community to plan and direct services to meet the needs of individuals and families within their homes and communities.

Job duties of this rewarding position include:

<li>Directly supervises Home Health Aides and LPN&rsquo;s. Carries out supervisory responsibilities in accordance 
with the organization's policies and applicable laws. Responsibilities include, but are not limited to, 
scheduling, planning, assigning, and directing work; appraising performance; addressing complaints and 
resolving problems. Performs supervisory visits with Home Health clients and conducts and authorizes inservices.</li>
<li>Conducts client assessments, suggesting level of needed service packages to client and their family.</li>
<li>Develops plans of care with the participation and consent of the client and/or caregiver/family member or legal representative.</li>
<li>Sets up medications for clients and monitors side effects, drug interactions, dosages, and implements changes per physician instructions. Informs client and family of changes immediately. Orders medication for clients from pharmacy.</li>
<li>Listens and documents significant changes in condition of clients, including incidents, unexplained injuries, medication errors, loss of client property, or evidence of client or family dissatisfaction and communicates with supervisor.</li>
<li>Analyzes data gathered from assessment and records reviews, and acts promptly to resolve problems.</li>
<li>Participates in quality assessment and assurance activities including implementation of recommendations and monitoring of results.</li>
<li>Develops plans of care with measurable objectives and timetables and accommodates clients&rsquo; needs and preferences including choices about preferred intensity of medical treatment.</li>
<li>Maintains ongoing communication with other disciplines/departments to evaluate client responses to interventions, suggest alternative approaches when indicated and to revise and update plans of care to reflect progress toward goals as directed by supervisor.</li>
<li>Interacts with clients, caregivers/family members, legal representatives, and resolve concerns.</li>
<li>Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.</li>
<li>Communicates with the physician regarding the patient&rsquo;s needs and reports any changes in the patient&rsquo;s condition; obtains/receives physician&rsquo;s orders as required.</li>
<li>Instructs, supervises and evaluates home health aide care provided every two (2) weeks or per regulatory guidelines.</li>
<li>Demonstrates C.A.R.E. values to our clients, family members, customers and staff.</li>
<li>Complies with and adheres to the appropriate use of Personal Protective Equipment (PPE) required by the Bloodborne Pathogens Standards. Protective Personal Equipment (PPE), including personal protective equipment for eyes, face, extremities, protective clothing, and protective shield and barriers, will be provided, used, and maintained.</li>

Job Requirements:

<li>Graduate of an accredited school of nursing with RN licensure in the state in which services will be provided.</li>
<li>One (1) year of home health care experience preferred.</li>
<li>One-year supervisory/management experience with direct care staff preferred.</li>
<li>Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; 
nursing skills per competency checklist.</li>

Home Health RN Case Manager

My Home Health

<div>RN Case Manager is responsible in handling case management of home health patients and the assigned staff team to ensure effective and quality delivery of home health care services in accordance with the State and Federal Regulations and the Agency's Policies and Procedures. He/She coordinates plan of care, patient care and physician's orders with assigned team members of Registered Nurses, Licensed Vocational Nurses, Medical Social Workers, Certified Home Health Aides, and Rehab Services.
<h4 class="jobSectionHeader">
Essential Functions:</h4>
1. Acts as an effective liaison to the home health team to ensure continuity and congruity of home health services in accordance with the patient's plan of care.
2. Oversees a team of Registered Nurses, Licensed Vocational Nurses and Certified Home Health Aides to ensure effective and quality home health services.
3. Provides professional home health care case management under the direction of the Director of Patient Care Services or Assistant Director of Patient Care. Communicates appropriate status/condition changes following Agency's Policies and Procedures.
4. Works collaboratively with others to achieve goals, solve problems, and meet established organizational objectives and management of patients.
5. Responsible for the overall management of assigned patients including the provision of direct care following a visit productivity standard.
6. Coordinates with the home health team and ensures implementation of home home health plan of care, effective visit utilization management and control.
7. Plans, organizes, and directs all patient care services in the assigned caseload ensuring achievement of optimum quality home health care.
8. Reviews patient referral information and responds to requests and inquiries as needed.
9. Coordinates with physicians, other health care professionals - rehab therapists, social services in developing and coordinating plan of care.
10. Coordinates all information on patient referrals, plan of care, patient care and physician orders.
11. Assists in establishing the initial Plan of Care and provides supplies and equipment arrangements for the client.
12. Ensures implementation of a correct, timely and complete clinical documentation in accordance with the Agency's Policies and Procedures and State and Federal regulatory requirements.
13. Communicates with clinicians and provides training and guidance following compliance with the Agency Policies and Procedures.
14. Participates in home health case conferences and other Agency meetings.
15. Provides guidance to Agency home health staff, other health care professionals, patients, and families related to appropriate and available health care resources.
16. Adheres and implements compliance to Health Insurance Portability and Accountability Act of 1996 (HIPAA) for protection of confidentiality of all written and computer files.
17. Promotes and participates in the development and execution of the Quality Assurance Performance Improvement (QAPI) Program and activities.
18. Other related duties and responsibilities that may become necessary or appropriate to meet the clinical care and compliance needs of agency.<h4 class="jobSectionHeader">
Knowledge, Skill and Experience:</h4>
1. Registered Nurse with current license to practice in the State of Arizona
2. Minimum Experience required: Minimum of 1 year of home health case management experience preferred;
3. Knowledge of home health regulatory and reimbursement requirements preferred;
4. Strong coordination, multitasking, telephone skills, organizational and interpersonal skills with sensitivity toward Medicare population required;
5. Computer skills required both clinical and clerical programs; With Computer and Microsoft Office Skills; Must be able to type a minimum of 45 words per minute and have a working knowledge on medical terminology;
6. Excellent verbal and written communication skills;
7. Ability to work independently on assigned tasks as well as accept direction on given assignments;
8. Ability to supervise and direct members of home health care team;
9. With organizational and time management skills.
Typical Working Conditions: Typical office environment - no unusual exposures.

Equipment Used: Computer keyboard, telephone, Fax Machine, Copier, Projector

Essential Physical Tasks: Must occasionally carry boxes of patient information documents weighing approximately 30 lbs. Work at computer monitors for prolonged periods must guard against eyestrain and muscle pain.

MAJOR RESPONSIBILITIES
 
<ul>
<li>Reflects the company’s mission and values in developing and maintaining professional working relationship with others.</li>
<li>Ensures that client complaints, issues and concerns are communicated to the COO and the Administrator.</li>
<li>Ensures compliance at assigned location with established professional standards of practice, state and federal regulations, and company policies, procedures, and processes, employee management and key quality measures.</li>
<li>Ensures client care is delivered to established care guidelines, Plan of Care, Physician Orders, state and federal regulations, and company policies, procedures, and processes and meets payer requirements.</li>
<li>Oversees and ensures appropriate patient and personnel assignments</li>
<li>Oversees the referral process. Ensures clients are accepted for care according to company admission criteria.</li>
<li>Ensures client needs are continually assessed</li>
<li>Ensures the development, implementation, and updates of the client’s individualized Plan of Care</li>
<li>Ensures all Vulnerable Adult and Child Protection reports are conducted following the company's policies and procedures.</li>
<li>Ensures clients are discharged from the company according to established policies and procedures.</li>
<li>Participates in and oversees the on-call process to ensure client care policies and procedures are followed.</li>
<li>Implements company Performance Improvement Plan (PIP) established by the company Director of Quality</li>
<li>Implements strategies and initiatives to achieve operational goals and objectives.</li>
<li>Role fulfills the Clinical Manager requirements of the Medicare Conditions of Participation for the assigned area.</li>
<li>Ensures onboarding, supervision, evaluation, training, assignment and discharge of staff is done according to established company policies and procedures. Notifies the Director of Human Resources of any employee issues or concerns.</li>
<li>Mentors, and supervises staff, holding them accountable for role responsibilities, competencies, individual performance and goals.</li>
<li>Ensures ongoing training, in-service education requirements, and performance management for staff are met according to company policies and procedures.</li>
<li>Oversees the compensation policies and procedures to ensure guidelines are met.</li>
<li>Implements assigned location’s safety programs and ensures completion of reports for Workers' Compensation.</li>
<li>May fulfill key roles during temporary vacancies.</li>
<li>Participates in strategic company growth and performance improvement measures.</li>
<li>Provides recommendations on assigned location’s sales and growth strategies.</li>
<li>Participates in the implementation of long range plans for expenditures and resources based on the business plan established by the parent, operational needs, and the safety of patients and employees.</li>
<li>Develops and maintains relationships with community agencies and referral sources that nurture and promote company growth and expansion.</li>
<li>Monitors key financial indicators for assigned location.</li>
<li>Monitors financial performance and key metrics for assigned location.</li>
<li>Ensures services are provided following established payer guidelines</li>
<li>Compiles and submits required documentation to parent accounting department for reimbursement of account payables</li>
<li>Ensures completion of necessary reports and documentation. Submits monthly reports as requested to parent.</li>
<li>Ensures payroll of employees meets established policy, procedures and guidelines.</li>
<li>Ensures all equipment/supplies required for employees to perform their roles are in good working condition and available.</li>
<li>Manages work assignments effectively, remains flexible with unexpected changes, and may be assigned additional duties.</li>
</ul>
PHYSICAL/ENVIRONMENTAL DEMANDS
 
Primarily office work that requires frequent sitting, standing, bending, and stretching. Must be able to drive or use public transportation in all types of weather.
 
Light. Amount of weight moved; up to 20 pounds occasionally.
 
QUALIFICATIONS
 
<ul>
<li>Currently licensed as a Registered Nurse (RN) or Graduate of accredited Physical Therapy program and currently licensed in the state of Minnesota; currently licensed as a Physical Therapist in the state of Minnesota and registered with the Board of Medical practice.</li>
<li>The license must not have been revoked, suspended, and without limitations or restrictions.</li>
<li>Proof of negative Tuberculosis testing: mantoux skin test, or documentation of negative chest X-ray.</li>
<li>Access to a dependable automobile in order to travel to multiple company business stops a day.</li>
<li>A valid driver’s license and proof of car insurance when using a personal vehicle for company business.</li>
<li>Have U.S. Citizenship or evidence of valid Alien Work Permit.</li>
<li>Disclose any conviction and criminal history records pertaining to any crime related to the provision of health services. A candidate who has been convicted of a crime will not be hired.</li>
<li>Pass initial and ongoing background studies and screenings including but not limited to those of the Minnesota Department of Health and the Federal Office of the Inspector General’s List of Excluded Individuals and Entities.</li>
</ul>
SKILLS AND EXPERIENCE
 
<ul>
<li>Five years of progressive health care setting with management experience required. Home health Care experience preferred.</li>
<li>Demonstrates critical thinking skills, the ability to follow standards/procedures, and make appropriate clinical judgments to support the provision of safe client care.</li>
<li>Ability to work independently with minimal supervision, adapt to change, and manage a variety of assignments while establishing priorities and ensuring the necessary duties are completed.</li>
<li>Excellent written and oral communication skills, interpersonal communication and teaching skills.</li>
<li>Ability to mentor and develop leadership and employees for assigned location.</li>
<li>Basic computer skills and willing to learn new software programs; experience with Electronic Medical Records and Microsoft Outlook preferred.</li>
<li>Current CPR certification highly recommended.</li>

Adara Home Health, Inc. is committed to fair and equitable compensation practices. The base salary range for this position in the U.S. is $100,000- $118,000 per year plus qualifies for a bonus incentive program. </div><div>As a Care Transitions Coordinator (Clinical Liaison), you will assist patients with the process of navigating post-acute care. Your goal is to create a positive impact on patient outcomes while increasing referral source satisfaction with Enhabit's services.</div> 
Responsibilities 
<ul><li>
Integrate evidence-based clinical guidelines, preventative guidelines, protocols, and other metrics in the development of transition plans that are patient-centered and promote quality and efficiency in the delivery of post-acute care.
</li><li>Represent the area home health branches in strategic relationships with health systems, hospitals, inpatient facilities, physicians and physician groups, and executive level opportunities.</li></ul><div></div> 
Qualifications 
<ul><li>
Must be a graduate of an approved school of nursing or therapy.</li><li>
Must be licensed in the state where they currently practice.
</li><li>Must have two (2) years of demonstrated field experience.
</li><li>Must have demonstrated experience and understand federal, state, and local laws and regulatory guidelines governing the operations of Medicare certified home health and hospice.</li><li>
Must have basic demonstrated technology skills, including operation of a mobile device.</li></ul>
Education and experience, preferred<ul><li>
A Registered Nurse RN or Physical Therapist PT is required.
</li><li>Three (3) years of field experience is preferred.</li><li>
Previous experience in home health or healthcare sales is preferred.</li></ul>
