Position Overview</b></p><p>
The Hospice Aide is a Certified Nursing Assistant and paraprofessional member of the interdisciplinary care team who works under the supervision of a Registered Nurse. The Hospice Aide provides direct support and personal care for hospice patients, to assist in their day-to-day needs, provide comfort and enhance their quality of life. The Hospice Aide is responsible for following patients&rsquo; individual Plan of Care, providing in-person care, maintaining documentation and coordinating with the patient&rsquo;s RN Case Manager.</p><p><b>
Essential Functions and Skills</b></p><ul><li>
Provide personal care, including bathing, dressing, oral hygiene, and bed linen changes.</li><li>
Assist patients with mobility, transfers, and exercises as instructed by nursing staff.</li><li>
Offer assistance with meals, feeding, and light household tasks as assigned.</li><li>
Monitor and report patient condition changes to the RN Case Manager.</li><li>
Administer routine skin care to prevent breakdown and ensure patient comfort.</li><li>
Support medication reminders per state regulations.</li><li>
Maintain a clean, safe, and organized patient environment.</li><li>
Provide emotional support and respite for family caregivers.</li><li>
Use equipment safely and appropriately for patient care needs.</li><li>
Adhere to all documentation and HIPAA compliance standards.</li><li>
Participate in quality assessment and performance improvement activities.</li><li>

Requirements/Qualifications</b></p><ul><li>
Must have an active Certified Nursing Assistant (CNA) license in the state of practice.</li><li>
Must be at least 18 years of age.</li><li>
Ability to pass background study, consumer report and drug test.</li><li>
Valid State Driver&rsquo;s License, access to an automobile in good working order, automobile insurance in accordance with state and organizational requirements, and an acceptable driving record required.</li></ul><p><b>
