 &bull; Liaison between Crisis Housing Time Limited Subsidy, Problem Solving, CES Matching, Housing Navigation &amp; Family Advocate components of the Family Solution Center (FSC) to ensure that families receive appropriate services.
 
 
 
 
 
 
 
 
 
 &bull; Maintain confidentiality in all matters pertaining to the participants and their families.
 
 
 
 
 
 
 
 
 
 &bull; Develop effective and trusting professional relationships with participants to facilitate provision of services.
 
 
 
 
 
 
 
 
 
 &bull; Maintain a comprehensive caseload of participants. Effective and timely follow ups with head of household on a weekly basis to develop housing plans based upon the housing &amp; service plan that identified barriers.

WHAT YOU WILL DO:

Provides direct case management services to individuals and families who are homeless or at risk of becoming homeless, focusing on helping them achieve housing stability. Uses Family Development and Housing First models of case management that is client driven, solution focused and facilitates change and movement toward self-sufficiency. Specific duties may include, but are not limited to home visits, intakes, the tracking of services, and assisting in program development. 


WORK SCHEDULE:

Monday through Friday: 9 hour shifts Mon-Thu, 4 hours on Friday. 


ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

Provides homeless housing related case management services using the Family Development Model to families participating in homeless housing programs.</li><li>

OUR IDEAL CANDIDATE WILL HAVE:

Minimum four-year degree and six months relevant experience; or year for year relevant experience in place of degree.</li><li>
Case management experience in social service employment setting, and a working knowledge of issues and resources in relation to housing services for low-income families preferred.</li><li>
Experience working with persons living with mental illness, experiencing chronic homelessness and/or experiencing Domestic Violence and its long term affects</li><li>
Knowledge of the disease of Chemical Dependency and its signs and symptoms.</li><li>
Experience with crisis prevention and intervention and crisis management; including resolution and recovery.</li><li>
Valid Washington State driver&rsquo;s license and five-year driving history within Opportunity</li><li>

<div>GENERAL DESCRIPTION
The Rapid Rehousing Case Manager is a grant-funded position focused on providing case management and rental assistance to homeless individuals, families, and youth in Marion and Polk Counties. The person in this role will provide direct services to individuals, which include; eligibility screening, program enrollment, participant follow-ups and check-ins. Case managers will also be responsible for processing required paperwork, data entry and other general program supports. This position will directly align with the Housing First initiatives.

<li>Ensure overall compliance with organizational and funder regulations. Adhere to documentation requirements as assigned by leadership and funder regulation/standard.</li>
<li>Coordinate aftercare services and follow-up contacts for post-discharge updates. &Oslash; Conduct outreach and engagement including apartment visits and inspections.</li>
<li>Participate in community development efforts such as Community Meetings and supporting the residence in establishing partnerships with other providers in the community.</li>

<li>Property Management staff to assist tenant to meet terms of their lease including timely payment of rent and avoidance of any nuisance behaviors.Mediate landlord-tenant disputes that threaten housing stability.</li>
<li>Utilize trauma informed interventions and culturally responsive engagement techniques to assist tenants with overcoming barriers to housing stability.</li>
<li>Participate in community initiatives to fulfil our mission to end homelessness</li>
<li>Provide crisis intervention as needed.</li>
<li>Provide all services in a manner that respects cultural differences amongst tenants.</li>
<li>Carry out other duties as assigned by leadership.</li>

<li>Requirements:
</li><li>A Bachelor's degree in human services or related field.</li>
<li>Must possess strong organizational, writing, communication, and computer skills.</li>
<li>Some relevant experience working with low-income or homeless populations.</li>
<li>Fluent in Spanish is strongly preferred.</li>
<li>Candidates must be self-starters with the skills and energy to work within the community.</li>
<li>Case Manager is regularly required to, type, use hands and fingers, reach with hands and arm. The Case Manager is regularly required to walk, stoop, and kneel. The Case Manager is often required to spend up to six (6) hours sitting at a desk during a normal eight (8) hour shift. Occasionally the Case Manager is required to lift items less than 5 pounds. The Case Manager is regularly required to move throughout the facility while carrying out job duties.</li></ul>

<div>Minimum Qualifications

Education and Experience:
<ul><li>A bachelor's degree which includes a practicum encompassing a substantial number of activities with the target population or which features a major or concentration (concentration = a minimum of 24 credits in the noted disciplines) in: social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation/recreation therapy, counseling, community mental health, child and family studies, sociology, speech and hearing plus one year of case management or other relevant human services experience. A bachelor's degree in any field plus two years of case management or relevant human services experience.</li></ul>

About the Role

The Case Manager will have the prime responsibility for organizing comprehensive service planning and assessment activity and assuring service plan implementation through direct case management or co-provider services. The Case Manager will also have either primary or shared responsibility, depending upon the specialty/discipline, in providing the residents with specific linkages/advocacy services with co-providers of housing, mental health, substance abuse, and healthcare services etc. as well as with parole boards. Together with the residents, the Case Manager will formulate residents' comprehensive service plan in conjunction with specialty program staff. For some residents the Case Manager will be the major provider of a particular service and, for others, a secondary support agent.

Primary Responsibilities
<ul><li>Participate in resident orientation.</li><li>Complete assessments and maintain service plans using integrated healthcare approaches.</li><li>Provide counseling, daily living instruction, and crisis intervention.</li><li>Transport residents to appointments and support income optimization.</li><li>Facilitate recreational and vocational linkages.</li><li>Conduct inspections, document progress, and complete incident reports.</li><li>Participate in quality improvement and maintain accurate records.</li><li>Attend required meetings, trainings, and supervisions.</li><li>Execute emergency plans including CPR/First Aid.</li><li>Perform additional job-related duties as assigned.</li></ul>
