Case Manager, Transitional Housing
<br><br>
Reporting to the Clinical Coordinator, the Case Manager, Transitional Housing will provide case management services to people experiencing homelessness, some with mental illness and/or substance use disorders, in order to assist the individual in the transition to permanent housing.
<br><br>
This position has a 35-hour work week.
<br><br>
<b>ESSENTIAL DUTIES:</b><br>
<ul>
<li>Provide direct services to a caseload of approximately 15 clients, including conducting necessary assessments, benefits assistance, and other services necessary to support the client in achieving housing and recovery goals</li>
<li>Conduct psychosocial evaluations</li>
<li>Connect clients to psychiatric services</li>
<li>Connect clients to medical treatment and substance use treatment facilities when applicable</li>
<li>Obtain identification, public benefits, and other applicable income sources for clients</li>
<li>Meet weekly with clients to assess needs, encourage harm reduction, and develop service plans</li>
<li>Prepare all documentation for housing applications</li>
<li>Assist and counsel clients in the process of obtaining permanent housing</li>
<li>Maintain record keeping in compliance with agency standards as well as those of City and State agencies</li>
<li>Intervene in crisis situations</li>
<li>Escort clients to appointments when needed</li>
<li>Upon training, provide assistance with self-administration of medication consistent with Breaking Ground's policies and procedures</li>

<b>MINIMUM QUALIFICATIONS:</b><br>

<li>Bachelor’s Degree or equivalent work experience</li>

The Supportive Services for Veteran Families (SSVF) SSVF Case Manager will be responsible for the case management of a portfolio of Veteran families who have experienced or are at risk of homelessness. The SSVF Case Manager will work closely with each family on their caseload to help them achieve long-term housing stability in the shortest time possible. Supportive Services for Veteran Families (SSVF) is a case management program for homeless, or at-risk for homelessness, Veterans and their families. SSVF Case Managers will assist you with identifying and addressing barriers, increasing self-sufficiency, and securing permanent housing. We work closely with the VA and other community service providers to ensure that our clients are connected with any and all resources available to them.</p><p>

Maintains a caseload of households in the SSVF Program. Works with households to assess housing needs, and provides case management and financial assistance to address those needs. Assists clients to secure housing if they are experiencing homelessness.</p><p>
Utilizes Housing First and Motivational Interviewing approaches to engage and work with Veteran households. In collaboration with Veteran households, develops Housing Stability Plans, and connects households with additional VA and community resources to meet their identified needs in the shortest time possible. Assists clients in accessing and maintaining entitlements and benefits.</p><p>
Establishes and maintains collaborative working relationship with community resources; attends and participates in appropriate community resource meetings and events.</p><p>
Participates in street and shelter outreach to identify homeless Veterans and present the SSVF Program as an alternative to homelessness.</p><p>
Completes thorough intakes and eligibility screenings with referred clients.</p><p>
Enters client data and case notes into the Homeless Management Information System (HMIS).</p><p>

Under supervision of the Program Manager, the Case Manager is responsible for the provision of case management and housing stabilization services to homeless Veteran families. Translatable military experience, relevant internship and volunteer experience, or administrative experience will be considered. SUMMARY OF POSITION:</b></p><p>
The Housing Navigation Case Manager (HNCM) Co-Op will work within a team of case managers to ensure that we develop and execute sustainable and expeditious housing plans for currently homeless guests. The Housing Navigation Case Manager Co-Op will engage with homeless guests to reduce barriers to housing and develop individual housing plans that will rapidly lead to safe, affordable housing with the supports necessary to sustain achieved housing. The HNCM Co-Op and their team will work under tight timeframes for accurately entering guest information into the Pine Street Inn&rsquo;s approved Homeless Management Information System (HMIS). Attention to detail and accuracy is critical in this position, The Housing Navigation Case Manager Co-Op will provide exceptional customer service both internally and externally and will work in partnership with other PSI departments to provide integrated service delivery, which furthers our mission to eliminate homelessness.</p><p><b>

Homeward Bound is the leading nonprofit organization in Buncombe County focused on preventing and ending homelessness using the best practice Housing First model. We work collaboratively with community partners and have a proven track record of helping individuals and families achieve housing stability. Since 2006, Homeward Bound has moved more than 2,615 people off the streets, out of cars, camps, and shelters and into permanent housing.</p>

Overview: </b>Homeward Bound&rsquo;s outreach team works to connect homeless individuals to housing and community resources. The Outreach Case Manager is a source of support and guidance to individuals, trained to assist individuals with identifying barriers to housing and engage in goal planning that aims to link individuals to the resources and skills that they need in order to achieve long-term housing. Most importantly, we are looking for individuals that have passion for the work and compassion for all people experiencing homelessness in our community, no exceptions.</p>

This Outreach Case Manager is primarily focused downtown and serves as a liaison between people experiencing homelessness, faith communities, businesses, residents, and City of Asheville emergency services staff, offering harm reduction strategies and problem-solving support to all parties.</p>
