You will be responsible for the assessment, diagnosis, and treatment of assigned patients.POSITION RESPONSIBILITES:Patient Care<ul><li>Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es).</li><li>Provides professional nursing care by utilizing all elements of nursing process and as defined in the state Nurse Practice Act.</li><li>Assesses and evaluates patient&rsquo;s status by:</li></ul>A) Writing and initiating plan of careB) Regularly re-evaluating patient and family/caregiver needsC) Participating in revising the plan of care as necessary<ul><li>Initiates the plan of care and makes necessary revisions as patient status and needs change.</li><li>Uses health assessment data to determine nursing diagnosis.</li><li>Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.</li><li>Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician&rsquo;s plan of care.</li><li>Counsels the patient and family in meeting nursing and related needs.</li><li>Provides health care instructions to the patient as appropriate per assessment and plan.</li><li>Assists the patient with the activities of daily living and facilitates the patient&rsquo;s efforts toward self-sufficiency and optional comfort care. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. As a Hospice RN, you will:</div><ul><li><div>
Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, functional, psychosocial, and environmental needs as evidenced by documentation, clinical record, case conference, team report, evaluations, and ADLs, (i.e., risk for grief, cultural and spiritual, verbal and non-verbal).</div></li><li><div>
Apply specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and the patient&rsquo;s eligibility.</div></li><li><div>
Implement/develop/document the plan of care to ensure quality and continuity of care and recommend revisions to the plan as necessary.</div></li><li><div>
Consult with and educate the patient and family regarding disease process, self-care techniques, end-of-life care, nutrition and dietary needs. Prioritize any needs with the members of the IDG. Provide training to other staff as needed.</div></li><li><div>
Determine scope and frequency of services needed based on acuity and patient/family needs. Assess the ability of the caregiver to meet the patient&rsquo;s immediate needs upon admission and throughout care.</div></li><li><div>
Provide clinical directions to the Hospice Aide and LPN/LVN to ensure quality and continuity of service provided.</div></li><li><div>
Prepare clinical and progress notes that demonstrate progress toward established goals. Ensure continuity of quality patient care delivered with appropriate documentation.</div></li><li><div>
Inform physician and other personnel of changes in the patient&rsquo;s needs and outcomes of intervention, while evaluating patient and family response to care.</div></li><li><div>
