SUMMARY OF JOB RESPONSIBILITIES</b><b>:</b></div><div>
The Clinical Manager is the heart of the interdisciplinary team ensuring that HopeWest delivers high-quality, compassionate hospice and palliative care to patients living in various community settings. From welcoming new team members to fostering growth through regular one-on-one sessions and dynamic group meetings, the Clinical Manager serves as the driving force for staff excellence. This role is responsible for ensuring that every team member feels empowered to deliver the highest standard of care through a hands-on leadership approach, including staying connected and addressing any needs promptly. Serving as a guardian of sustainability, the Clinical Manager works collaboratively with teams to meet monthly contribution margins while never losing a commitment to quality care.</div><div><b><br>
QUALIFICATIONS</b><b>:</b></div><div><ul><li>
RN with current Colorado licensure; graduate from an accredited school of nursing, BSN preferred.</li><li>
Must have and maintain hospice certification (CHPN) or obtain no later than the end of the second year of employment.</li><li>
At least 3 years of experience in home health, hospital, or hospice setting.</li><li>
Demonstrated management and/or leadership experience.</li><li>
Valid Colorado driver&rsquo;s license and unlimited access to reliable transportation.</li><li>
Adequate automobile insurance.</li></ul></div><div><b>

HOW YOU'LL WORK:</b><br>
You'll be responsible for providing end-of-life care and Hospice services to patients in the Health System Inpatient Hospice setting in a manner consistent with standards of practice. You will assist in determining the need to change the level of care based upon assessment and collaboration with the Attending Physician or Medical Director.</p><p></p><p><b>

Plan of Care:</i> Implement care to achieve outcomes of comfortable and safe dying, self-determined life closure and effective grieving for patient and family.</p></li><li><p><i>
Family Educator/Advocate:</i> Maintain the dignity of the dying patient while supporting their unique spiritual and cultural beliefs, provide appropriate support at the time of death, including disposition of medications, notification of funeral home, etc. according to agency policy.</p></li><li><p><i>
Policies: </i>Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.</p></li><li><p><i>
Communication:</i> Provide referral sources with information on hospice programs and the coordination of patient care for optimal transition from the hospital to discharge destination in a hospice care setting wherever the patient calls home.</p></li><li><p><i>
Operations:</i> Attend interdisciplinary team meetings for full team discussions, consultation and evaluation of patient/family needs.</p></li></ul><p></p><p><b>
