Job Summary:</b></p>
<p>
The Hospice Clinical Director is responsible for ensuring the coordination and management of patient care. Assists the Director of Nursing in maintaining the quality of patient care in accordance with the Agency's objectives and policies, through planning, coordination, implementation and evaluation of the hospice services provided. This market services the Kansas City region in Kansas.</p>
<p><b>
RESPONSIBILITES</b></p>
<ul>
<li>Coordinate and oversee direct and indirect patient services provided by clinical personnel.</li>
<li>Provide supervision and guidance to clinical staff to ensure the quality of services and maintenance of standards.</li>
<li>Perform chart audits.</li>
<li>Gather data for QAPI and participates in QAPI activities.</li>
<li>Appraise clinical staff performance during their probationary period and annually, provides the teaching and counseling necessary to enhance the delivery of services and the professional development of nursing and health aide staff.</li>
<li>Identify and evaluate patient needs and appropriateness of home care by reviewing the initial intake screening form before assignment of direct care staff. Assist with the orientation of new staff. See patients in the home setting as needed. Maintains adequate and appropriate Medical Supplies.</li>
<li>Assists with orientation of new personnel as well as in-services for current staff.</li>
<li>Responsible for ensuring Medicare, State and ACHC regulations are followed by all clinical staff.</li>
<li>Assists the Director of Nursing with responsibilities as requested.</li>
<li>Assume responsibility for self-development by continually striving to improve his/her Nursing/management practice through formal education, attendance at workshops and conferences, active participation in professional and related organizational meetings, and/or individual research and reading.</li>

<p><b>JOB CONDITIONS</b></p>
<ul>
<li>Position may be stressful.</li>
<li>Primarily a desk job which involves minimal and occasional lifting of medical supplies and charts.</li>
<li>On occasion, one may be required to bend, stoop, reach and move patient weight up to 250 pounds lift and/or carry up to 30 pounds.</li>
<li>Travel is required to and from patient's home when supervision of care is scheduled.</li>
<li>The ability to access patients' homes which may not be routinely wheelchair accessible is required.</li>
<li>Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform patient care.</li>
<li>Use of computer, multi- line telephone, calculator, copier, and other office and medical equipment.</li>

<li>The Clinical Supervisor must possess at least one of the following:</li>
<li>A Bachelor of Science in nursing and two (2) years combined public health nursing and progressive professional responsivities in public health nursing OR Three (3) years combined public nursing and progressive professional responsibilities in public health nursing.</li>
<li>Must have a minimum of 2 years nursing experience in home hospice or skilled home care. (hospice preferred)</li>
<li>Ability to market and deal tactfully with customers and the community.</li>

We&rsquo;re looking for a compassionate Hospice Chaplain to provide non-denominational spiritual and emotional support to patients and families facing end-of-life care. As a valued member of our interdisciplinary team, you&rsquo;ll help ensure that no one walks this path alone.</div><div></div><h3 class="jobSectionHeader"><b><br>
What You&rsquo;ll Do as a Hospice Chaplain:</b></h3><ul><li><div>
Provide spiritual and emotional support to patients, families, and caregivers in alignment with their beliefs and values.</div></li><li><div>
Offer non-denominational, inclusive care that respects all faiths, spiritual traditions, and cultural backgrounds.</div></li><li><div>
Serve as an integral member of the interdisciplinary team, participating in patient care conferences and collaborating with nurses, social workers, aides, and medical staff.</div></li><li><div>
Assess spiritual needs of patients and families and develop individualized spiritual care plans.</div></li><li><div>
Conduct visits, prayers, blessings, and rituals as appropriate and requested by the patient or family.</div></li><li><div>
Assist families during times of grief, loss, or transition, and provide bereavement support and counseling.</div></li><li><div>
Build relationships with local faith leaders and community resources to support patients' and families' unique spiritual needs.</div></li><li><div>
Maintain timely and accurate documentation of spiritual assessments, interventions, and outcomes.</div></li></ul>
About You:
<div><b>
The candidate must meet one of the following educational requirements:</b></div><ul><li>
Bachelor&rsquo;s degree in theology, religion, human services, counseling, psychology, or sociology (or as required by state-specific regulations)</li><li>
A minimum of one unit of ACPE-accredited Clinical Pastoral Education (CPE)</li></ul><div>
Note: Degrees from accredited institutions are preferred. If a degree is from a non-accredited college or university, AVPO approval is required prior to hire.</div>
