</b></p><p></p><p>The Weekend Clinical Care Coordinator, is responsible for monitoring and responding to referrals through Allscripts, E-fax and 4 Next and data entering referral information provided in the EMR, answering incoming telephone calls from patients/patients' families and referral sources; making patient care decisions in response to telephone calls; coordinating and assigning visits to clinicians as deemed medically necessary to meet patients' needs; and accepting reports from and responding to questions from home care clinicians who are in the field providing care.<br>
</p><p></p>Position Responsibilities Include:<br>
<p></p><ul><li><p>Respond to incoming telephone calls and other inquiries from patient/patients' families, referral sources and other healthcare professionals.
</p></li><li><p>Document incoming patient telephone calls, response or direction, and the coordination of services using the appropriate processes and systems in the EMR
</p></li><li><p>Make patient care decisions in response to incoming patient telephone calls, using professional nursing judgment. Such responses may include providing advice or direction to the patient/cg, contacting the physician or scheduling a visit.
</p></li><li><p>Provide guidance and direction to home care providers regarding patient care and clinical issues including patient/staff incidents and/or injuries.
</p></li><li><p>Regularly communicates with and works in cooperation with appropriate personnel to coordinate patient care services.
</p></li><li><p>Oversees weekend schedules, and makes adjustments to schedule in response to patient and and/or staffing changes.
</p></li></ul><p>&middot; Perform the duties and responsibilities of the Intake Coordinator position, by monitoring and responding to all referrals through Allscripts, E-Fax and 4 Next, in addition to reviewing content of referrals and data entering referral information, and current orders into EMR.

</p><ul><li><p>Coordinates and assigns admission visit to appropriate staff when weekend or Monday admission is indicated.
</p></li><li><p>Check Common Working File for MC patients for Admissions as required by the payer as able, given potential limitations of accessibility during weekend days.
</p></li><li><p>Check managed care to verify coverage for new referrals.
</p></li><li><p>Will be responsible to receive hand off communication from Clinical Manager regarding any potential issues (patient related or staff related) that may possibly occur on the weekend.
</p></li><li><p>Provide weekend summary report to intake department, Sr. Director of Home and Community-Based Services and Home Health and Hospice Clinical Manager for their review on Monday mornings.
</p></li></ul><ul><li>As directed, collaborates to create an agency environment that is in compliance with federal, state, and local regulatory agencies. Complies with HIPAA regulations and practice policies for the privacy and security of patient information.

As a Nurse Practitioner, you will:

Take responsibility for patient palliative care from the initial assessment through discharge. Assessment at a minimum should include: disease-specific changes, decline or change in function, nutrition and/or cognition, inadequately controlled pain or other distressing symptoms, falls, infections or similar events, the impact of the disease burden on the patient's condition, caregiver and/or family's quality of life.</li><li>
Establish the palliative plan of care, individualized for each patient, in consultation with the patient, family and other members of the Palliative Care Team.</li><li>
Facilitate obtaining care for patient when a need is identified in accordance with the Plan of Care.</li><li>
Participates in evening/weekend call as required, conducting on-call services in a clinically competent and responsive manner.</li><li>
Prescribe medications including controlled substances to the extent delegated and licensed, in accordance with state law. Order and interpret laboratory and other diagnostic tests within the scope of professional practice. Order treatments and durable medical equipment as indicated.</li><li>
Communicate clinical findings to the Palliative Medical Director as needed, but no less than bi-weekly. Re-assess effectiveness of care plan on a regular basis and modify as needed.</li><li>
Maintain effective working relationships with supportive care team (e.g., Social Worker, Spiritual Care Coordinator), as applicable, and involve team when patient's care plan warrants team participation to achieve desired outcome.</li><li>
Establish and maintain effective working relationships with those contacted in the course of work.</li><li>
Communicate and document clinical findings, treatment plan and care provided in the patient's medical record in a manner consistent with acceptable standards in order to support sound medical practice and reimbursement for services provided.</li><li>
Knowledgeable of Palliative Medicine Quality Measures collection, documentation and reporting.</li></ul>

The Board and executive leadership of Community Hospice & Palliative Care formed Alivia Care Inc. in 2020 with the strategic intent to provide a structure in support of the expanded mission and vision of Community Hospice & Palliative Care and prepare it to respond to changes in care delivery and reimbursement for patients living with advanced or chronic illnesses. Under the direction of the Care Navigation Center Manager, the Care Navigation Coordinator provides support guiding incoming Community Hospice and Palliative Care, PACE and Home health referrals to the appropriate personnel or other healthcare resource.
<br>S/he completes the referral intake process via telephonic and non-telephonic communications, including but not limited to areas such as insurance verification, scheduling patient and personnel assessments, providing patient and affiliated family members with requested information, and conducting routine follow up check-ins with patients and referral sources ensuring seamless patient admissions to services.

<li>Processes referrals with accuracy and in a timely manner.</li>

We are currently seeking <b>compassionate and dedicated Certified Home Health Aides (CHHAs)</b> to join our growing team in the<b> Essex </b><b>County area</b>. This position focuses on delivering high-quality, dignified care to patients during the <b>evening weekend shift (3:00 PM &ndash; 11:00 PM)</b>.</p>

<li><p>Provide compassionate, hands-on care to hospice patients as needed</p>
</li>
<li><p>Assist patients with activities of daily living (ADLs)</p>
</li>
<li><p>Follow individualized care plans to ensure the highest level of comfort and support</p>
</li>
<li><p>Collaborate with the interdisciplinary hospice care team</p>
</li>
<li><p>Communicate effectively with patients and their families</p>
</li>

<li><p>Valid <b>Certified Home Health Aide (CHHA) license</b> in the state of New Jersey</p>

<li><p>Prior <b>hospice experience is a plus</b>, but not required</p>

<li><p>Reliable transportation and dependability</p>
</li>
<li><p>Strong communication skills and a compassionate approach to care</p>
